If divers are utilized to conduct the underwater survey as part of the examination process, Alternate Hull Examinations (AHEs) must be performed twice within every five-year period, with no more than three years allowed between any two AHEs. This required three-year interval was exceeded on May 8, 2025. You must provide our office, Officer in Charge, Marine Inspection (OCMI), with a written hull examination report to include: thickness gauging results, bearing clearances, a copy of audio and video recordings, and any other information that will help the OCMI evaluate your vessel for a credit hull exam. This report must be signed by the third party examiner and confirm the validity of its contents.

You must submit your written application for renewal of a Certificate of Inspection to the OCMI at least 30 days prior to the expiration date of the Certificate of Inspection. Vessel allowed their COI to lapse with no notice to OCMI.

Any vessel of at least five net tons which engages in [...] coastwise trade (Defined as the transportation of passengers or merchandise between points embraced within the coastwise laws of the United States). Vessel had an expired Certificate of Documentation, reapplied to NVDC.

The owner or managing operator must provide satisfactory evidence of the required servicing to the marine inspector. Vessel just came due for annual servicing, scheduled for the following week. Must provide documentation showing portable extinguishers servicing. Vessel also has pre-engineered system installed in a locker, overdue for servicing by 2 years. Vessel to remove or provide documentation of servicing.

Placards containing instructions for the donning and use of the life jackets aboard the vessel must be posted in conspicuous places that are regularly accessible and visible to the crew and passengers. Vessel had no donning placards.

The master shall conduct sufficient drills and give sufficient instructions to make sure that all crew members are familiar with their duties during emergencies. Abandon ship and man overboard drills and training shall be logged or otherwise documented. Vessel had no running drill logs.

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment. Vessel had daisy-chained power strips on bridge to power radios, cameras, and A/C unit. Vessel to find suitable length cables and ensure outlet is rated to provide the demanded electrical load.

A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. Vessel first aid kits had expired and missing contents.

Each EAP education program must include at least the following elements: display and distribution of informational material; display and distribution of a community service hot-line telephone number for crewmember assistance, and display and distribution of the employer's policy regarding drug and alcohol use in the workplace. Vessel had no hotline or educational information posted.

The number and identification of the items stowed inside, and their sizes, must be marked in clearly legible letters and numbers on each container for life jackets. Vessel had an old locker with faded lettering identifying it as for life jackets, no longer used for life jackets.

Replace generator fuel oil hose run from supply piping to generator housing with approved hose IAW 46CFR182.455.

Replace generator fuel oil hose and clamp from low pressure fuel oil pump to fuel manifold IAW 46CFR182.455.

Replace 02 excessively worn life ring buoys on the vehicle deck IAW 46CFR176.808(b).
