Installation shall be in accordance with 182.15-15. Dry pipe vertical exhausts are permissible if suitably insulated. MI witnessed STBD side exhaust lagging deteriorated.

The discharge ends of vent pipes shall be fitted with removable flamescreens or flame arresters. MI identified through thickness wastage of the fuel vent piping.

Propulsion machinery shall be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. MI observed a coolant leak from the port main engine coolant pipe hose connected to the keel cooler piping.

MI identified oil soaked windings. Clean the windings to the satisfaction of the attending MI.

S and L. Each major generator and motor shall be fitted with a nameplate of corrosion-resistant material marked with the following informations applicable. No data plate was affixed to the generator. Provide documentation attesting to the information required to be on the data plate and proof of debating if applicable

The engine head, block and exhaust manifold shall be water jacked and cooled by water from a pump which operates whenever the engine is operating. Port engine exhaust cooling water leaking from cracked and corroded weld on exhaust piping. 46 CFR 182.20-10(b)(1)

It is further intended that all machinery be installed and maintained in such a manner as to afford adequate protection against fire, explosion, machinery failure and personnel injury. Located fuel oil leak on generator. Generator provides power to Air Conditioning unit on board. 46 CFR 182.01-5

On a vessel of at 26 feet in length , a visual and audible alarm must be provided at the operating station to indicate a high water level. Steering lazarette visual alarm not illuminating repair alarm unit to factory. 46CFR182.530

Each Life jacket, immersion suit, and life buoy must be marked in clearly legible block capital letters with vessels name. Multiple life jackets have retroflective tape showing wear and labeled with wrong vessel name. repair retroflective tape and remove the non applicable vessels name. 46CFR185.604

Each item of survival craft equipment must be of good quality, and efficient for the purpose it is intended to serve. Lifefloat coating faded, replace or recoat lifefloat. 46CFR180.175

A vessel not required to have a power driven fire pump by § 181.300 must have at least three 9.5 liter (2 1/2 gallon) buckets, with an attached lanyard satisfactory to the cognizant OCMI, placed so as to be easily available during an emergency. The words “FIRE BUCKET” must be stenciled in a contrasting color on each bucket. No fire buckets were available upon request by MI. 46CFR181.610 60c.

All exposed materials must be resistant to oil or oil products, salt water and anticipated weather conditions encountered at sea. All components used in construction of buoys and life rings must meet the applicable requirements of subpart 164.019 of this chapter. The grab line shall be 3/8-inch diameter polyethylene, polypropylene, or other suitable buoyant type synthetic material having a minimum breaking strength of 1,350 pounds. MI inspected the life ring and noted grab line in unserviceable condition. 46CFR160.050-3(a)(c) 60c.

A vessel must carry one floating waterlight. Upon examination by MI the waterlight was found unserviceable. 46CFR180.70(d) 60c.

Where flexible nonmetallic hose is permitted for use in piping systems by this section, it must meet SAE J-1942 (incorporated by reference; see 46 CFR 175.600) or be specifically approved by the Commandant. The following restrictions apply: Flexible nonmetallic hose may be used in lube oil, fuel oil and fluid power systems, subject to the following requirements: Flexible hose may only be used at a pressure not to exceed the manufacturer's rating and must have a high resistance to saltwater, petroleum oils, and vibration. Upon inspection of vessels generator MI observed leaking fuel from non metallic hoses. MI is requiring repairs to be completed prior to starting/using generator and to provide proof of repairs to MI prior to restoring generator as operational. 46CFR183.720(e)(3)(i) 705c.

When the exhaust cooling water system is separate from the engine cooling water system, a suitable warning device, visual or audible, must be installed at the operating station to indicate any reduction in normal water flow in the exhaust cooling system. MI observed all warning devices had been removed from exhaust cooling water system. 46CFR182.425(b)(5) 50c.

Flexible hose designed for use with hose clamps may be installed with two clamps, at both ends of the hose. MI observed port main engine exhaust flexible hose missing the required amount of hose clamps. 46CFR182.720(e)(3)(B) 50c.

A self-propelled vessel must comply with the provisions of this subpart. MI observed bilge pipe in lazerette not properly secured due to missing u-bolt fasteners. 46CFR182.600 50c.

A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. For equivalent kits, the contents must be stowed in a suitable, watertight container that is marked “First-Aid Kit”. A first-aid kit must be easily visible and readily available to the crew. MI observed required items missing in addition to expired medications in the first aid kit. 46CFR184.710 50c

A hatch exposed to the weather must be watertight, except that the following hatches may be weathertight: A hatch on a vessel that operates only on protected waters. MI observed Port engine hatch gasket in unserviceable condition. 46CFR179.360 (a)(3) 50c.

Engine exhaust flexible coupling needs to be double clamped, it currently has single clamp installed.

Port side fwd engine room bulkhead cable penetration is loose and has a large gap around wires and engine control cable.

Vessel needs to have portable light mounted near entrance to engine room.

Provide exhaust lagging for #1 and #2 Engines.

Provide non-chaffing clamps to secure Port engine fuel supply and return lines.

Replace float-free link for life float.

Provide proof of enrollment in Drug and Alcohol test program.

Provide not under command lights prior to operating in low visibility conditions.

Mount EPIRB in a location where it can float free.

Demonstrate proper operation of rudder stops.

Cap off Water Trap for Fuel tank due to small leak.

Rudder arm makes hard contact with rudder stops, renew limit switch.

Repair crack in port exhaust piping.

Compass light has a blown fuse.

Found an area where soot was collecting near the generator exhaust. Requested vsl rep inspect for possible exhaust leak.

Missing emergency broadcast placard.

Weather stripping around the aft passenger door is missing.

First aid kit had expired eye wash solution and only contained small bandages.

Excessive loose gear around the bridge and passenger space.

Generator needs piping/hoses checked due to excessive oil leakage.

Cracking in bottom plating where keel cooler line ties in. The problem is recurring evidenced by piled up weldment on deck as previous repair. Recommend 18x18in insert.

Plug 2 thru fittings in forward engine room bulkhead (starboard side).

Label emergency stops at aft steering station.

Gouge and reweld 2 pickup welds in engine room. LEGACY COMPONENT DESC: Weld

Insulation failure along entire length of positive and negative battery cables feeding stbd main engine starter. Vsl captain states failure do to prolonged exposure to bilge cleaning products as cables were lying in the bilge.

Cloth straps on three life jackets failed.

Provide documentation showing compliance with drug testing requirement found in 46CFR16.

Prove proper operation of Marine Sanitation Device.
