A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. For equivalent kits, the contents must be stowed in a suitable, watertight container that is marked “First-Aid Kit”. A first-aid kit must be easily visible and readily available to the crew. Resupply first-aid kit to meet 160.041 and provide proof to USCG inspector.

A vessel must be fitted with ground tackle and mooring lines necessary for the vessel to be safely anchored or moored. The ground tackle and mooring lines provided must be satisfactory for the size of the vessel, the waters on which the vessel operates, subject to the approval of the cognizant [name withheld] midship mooring cross bit had severe corrosion and holes in aft horizontal cross. Vessel is restricted from using this cross portion of the bit until it is repaired to the satisfaction of the OCMI.

The owner shall, prior to receiving a Certificate of Inspection, submit for approval to the cognizant OCMI, at least two copies of the following plans, manuals, analyses, and calculations that are applicable to the vessel as determined by the OCMI:(6) Fire protection equipment installation including, but not limited to: (iii) Fire detecting system and smoke detecting system plans. Examination of fixed fire detection system installation showed that the installed step-up DC-DC converter was not consistent with the one approved in accordance with MSC letter E2-2203604 of November 25, 2022. Specifically, a different make/model was installed with a different output amperage. Amend plans and resubmit to MSC for approval or install approved step-up DC-DC converter.

A portable or temporary electric cord or cable must be constructed and used in compliance with the requirements of § 111.60-13 in subchapter J of this chapter for a flexible electric cord or cable. Newly installed fire detection ssytem was found powered with a portable power strip. Remove power strip. if necessary submit plans to install permanent connections.

Station license on board vessel was expired. Provide valid station license.

On a vessel of more than 19.8 meters (65 feet) in length, the public address system must be a fixed installation and be audible during normal operating conditions throughout the accommodation spaces and all other spaces normally manned by crew members. Public address system is not audible in the galley or accomodation spaces. Provide public address system audible in all required spaces.

On a vessel of at least 26 feet in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in each normally unmanned spaces. Bilge high level alarm in pilothouse not functioning. Repair and demonstrate operation.

Replace plastic sight glass with real glass.

Demonstrate watertight integrity of line way from forecastle to forepeak. Gasket found cracked and dog frozen.

Demonstrate vapor tight closure of engine room ventilation closures. Port side studs need replacement and both need gaskets.

Provide current coast pilot, light list, charts, and tides/currents on board.

Re-stencil emergency escape hatches.

Provide smoke detectors that meet UL 217.

Provide 01 additional adult lifejacket for a total of 18; vessel restricted to 17 persons until corrected.

Prove proper operation of mast head and anchor light; restricted to daylight ops only.

Provide Pipeline consortium letter, employee list, and MIS report.

Make deck hatch to E/R watertight.

Replace wing nuts with standard nuts on batteries.

Replace volt meter on #1 genset.

Re-mount dryer outlet cord to bulkhead.

Hard wire NAVTEX to electrical panel and prove proper operation; vessel restricted to 100nm from shore.

Re-pack port shaft log.

Adjust dogs on main deck WTD to E/R by next drydock.

Adjust emergency escape hatch from E/R to be opened from both sides.

Label power switch for fire pump.

Re-register EPIRB with NOAA with correct beacon ID#.
