When minor alterations or minor repairs of tank vessels become necessary such work shall be under the direction of the Officer in Charge, Marine Inspection, and shall be in accordance with the regulations in effect at the time the vessel was contracted for or built, or in accordance with the regulations in effect for new construction insofar as possible. Vessel shall provide damage survey, transit plan and/or salvage plan report to the satisfaction of the CG prior to movement.

Product has leaked into the #2 stbd wing void from either the #2 or #3 cargo tank.

Make permanent repairs to the previously reported TBSIP requirement in the #3 stbd side shell.

Submit repair plan for side shell replacement of entire barge to local OCMI for approval.

Vsl notified USCG NOLA of damage found in # stbd wing tank, temp repairs in place. complete final repairs to USCG satisfaction w/in 45 days per TBSIP policy and prior to loading after current cargo discharged.

#3 stbd wing void, 2" hairline plate fracture, 6'6" fwd of aft bulkhead at 5'6" draft.

Make permanent repairs to the pin hole leak in the #2 stbd wing void at approx the 8' draft level just fwd of the aft bulkhead. No further cargo operations after discharge.

3" crack in #2 stbd wing void 6' aft of the fwd blkhd at the 7.5' draft mark.

1/4" hole in the #2 stbd wing void, all the way, at the 7.5' draft mark.

Vessel permitted to transit from current location MM161 LMR LDB to MM164 LMR RDB with a portable pump onboard and hooked up. A watchman to be stationed at the void to monitor and pump out the stern void, if necessary. Vessel to remain at the CGB164 Yard for completion of temp repairs.

Make temp repairs to the port stern corner wrapper upper section with molded steel welded in place to cover the 3' x 2' holed and damaged corner. Corner to be made watertight and inspected by the USCG prior to moving the vessel.

After temp repairs in Item #2 have been completed, vessel permitted to transit to LaPlace DuPont facility to offload cargo of Caustic. Portable pump to remain onboard vessel and void to be examined every 4 hours.

After discharge of cargo at DuPont, vessel permitted to transit back to CGB164 for complete and permanent repairs to the port stern corner. Repairs to be inspected by USCG Marine Inspector.

T/B IB 953 took on water in the # 3 void through a 6 inch fracture 2 ft above the bilge knuckle starboard side. A satisfactory temporary repair was made by divers. After discharging present cargo at the Houston Ship Channel, proceed directly to dry-dock at Channelview Shipyard for repair. Repair to the satisfaction of attending USCG Marine Inspector prior to re-loading.

6" Fracture found in way of the #1 port void. Shipyard placed a temporary patch (concrete) to seal fracture. Issued CG835 to make permanent repairs prior to loading any cargo.

Identify cause of product leakage into the #2 wing void and effect permanent repairs to the satisfaction of the attending CG marine inspector prior to the carriage of cargo.

PRIOR TO LOADING CARGO INTO CARGO TANK NO. 3, PROVIDE PROPER LIQUID LEVEL GAUGING DEVICE IAW 46 CFR 38.10-20 TO THE SATISFACTION OF THE ATTENDING MARINE INSPECTOR.

Repaint missing draft marks on port and stbd side aft.

Repair/replace hydraulic starting unit for cargo pump engine.

Provide guard for exhaust pipe to diesel cargo engine.

Replace damaged flame screen on P/V valve tank #3 prior to cargo operations.

PROVIDE ON BOARD BARGE, LOAD LIMITS DIAGRAM AS REQUIRED BY CERTIFICATE OF INSPECTION PRIOR TO LOADING CARGOES REQUIRING A TYPE II OR TYPE III HULL BUT NOT LATER THAN 30 NOV 87.

PROVIDE ONBOARD BARGE, USCG APPROVED OIL TRANSFER PROCEDURES AND PIPING DIAGRAM FOR THE HYDRAULIC ACTUATED STRIPPING SYSTEM PRIOR TO UTILIZING THIS SYSTEM BUT NOT LATER THAN 30 APRIL 88.

INSTALL CARGO OIL DISCHARGE CONTAINMENT AS REQUIRED IN 33 CFR 155.310 IN WAY OF HYDRAULIC ACTUATED STRIPPING SYSTEM DISCHARGE FITTING PRIOR TO UTILIZING THIS SYSTEM BUT NOT LATER THAN 30 APRIL 88.

PRIOR TO UTILIZING STEAM SYSTEM, RELOCATE RELIEF VALVE DISCHARGE SO THAT ITIS FACING DOWN.

REPACK THE NO.(2) AND (3) CARGO TANK REACH ROD ASSEMBLIES.

PRIOR TO UTILIZING STRIPPING SYSTEM INSTALL SAFETY GUARD OVER STRIPPING PUMP DRIVE SHAFT.

INSTALL PIPE CAPS OVER BOTH OPENINGS LOCATED STRIPPING PIPING SYSTEM.

REPAIR, REPLACE OR REMOVE PORT NAVIGATIONAL LIGHT SCREEN.

REMOVE CARGO FROM DECK, FIND SOURCE OF LEAK WHICH APPEARS TO BE COMING FROMCARGO PUMP DRIVE SHAFT AND REPAIR PRIOR TO UTILIZING CARGO PUMP.

PLAINLY AND LEGIBLY MARK DRAFT MARKS.

INSTALL FLAME SCREEN ON FUEL OIL SUPPLY TANK GOOSENECK VENT PIPE.

INSTALL PLUG IN FUEL OIL SUPPLY TANK DRAIN LINE.
