PSCO’s observed the starboard side hatch cover had a 1” hole allowing water to penetrate the hatch rendering the hatchway to not be weathertight.

Additionally, the fire control plan stated, an additional fire control plan was to be located on the starboard bridge wing and PSCO’s did not observe one in that location.

Fuel oil sight glass - Safe and efficient means of ascertaining the amount of oil fuel contained in any oil fuel tank shall be provided. The Administration may permit the use of oil level gauges with flat glasses and self-closing valves between the gauge glasses and the oil tanks. Cylindrical gauge glasses may also be permitted in free standing oil fuel tanks provided that they are suitably protected and fitted with self-closing valves to the satisfaction of the Administration.
Fuel oil sight glass used appears to be plastic and is NOT equipped with a self-closing valve between the gauge and the sight glass.
