A hatch exposed to the weather must be watertight, except that the following hatches may be weathertight:(1) A hatch on a watertight trunk that extends at least 305 millimeters (12 inches) above the weather deck;(2) A hatch in a cabin top; and (3) A hatch on a vessel that operates only on protected waters. Engineroom and Lazzarette flush deck hatches did not function as designed and had wasted gasket material. Make hatches watertight and replace gaskets to the satisfaction of the attending inspector.

An inflatable life raft or inflatable buoyant apparatus must be serviced at a facility specifically approved by the Commandant for the particular brand, and in accordance with servicing procedures meeting the requirements of part 160, subpart 160.151, of this chapter—(1) No later than the month and year on its servicing sticker affixed under 46 CFR 160.151-57(n). Life raft servicing was expired. Service life rafts at an approved facility and provide evidence to attending marine inspector.

On a vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in each of the following normally unmanned spaces:(1) A space with a through-hull fitting below the deepest load waterline, such as a lazarette;(2) A machinery space bilge, bilge well, shaft alley bilge, or other spaces subject to flooding from sea water piping within the space; Bilge alarms for the engine room could not be located and both visual and audible alarms at operating were not verified. Provide SAT bilge alarm test to the attending marine inspector.

A vessel must comply with the applicable requirements for any radio and Electronic Position Indicating Radiobeacon (EPIRB) installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. Vessel was missing valid Safety radiotelephony and Bridge to Bridge radio certificates. Get FCC licensed radio inspeciton for certificates and provide valid VHF-DCS MMSI and GPS capability to the attending marine inspector.

A vessel of 12 meters or more in length shall be provided with a whistle, a vessel of 20 meters or more in length shall be provided with a bell in addition to a whistle. Vessel's whistle did not work at the time of inspection. Provide satisfactory whistle test to the attending marine inspector.

A hinged watertight bulkhead door must be fitted with a quick action closing devise operable from both sides of the door and indicator lights at the operating station showing whether the door is open or closed. WTD indicator lights did not operate when tested at the operator station to show door position changes. Provide sat door indicator test to the attending inspector.

An exposed hazard, such as gears or rotating machinery, must be properly protected by a cover, guard, or rail. Stbd machinery space hydraulic pump coupling was missing a protective device. Install protective device for the hydraulic pumps exposed rotating parts.

A voltmeter and an ammeter, which can be used for measur ing voltage and current of a generator that is in operation, must be provided for a generator rated at 50 volts or more. #1 Generator voltmeter did not indicate rated voltage during test.

The EPIRB test required by paragraph (a) shall be logged or otherwise documented, as applicable. EPRIB monthly test log not available for inspection. Provide monthly EPRIB inspection log. 46 CFR 185.728(c)

Monthly inspections required by NFPA 10 may be conducted by the owner, operator, person-in-charge, or a designated member of the crew. Monthly crew portable fire extinguisher inspection log not available for inspection. Provide monthly fire extinguisher inspection log IAW NFPA 10. 46 CFR 186.710(b)(ii)

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. Analgesic medication was expired. Replace analgesic medication. 46 CFR 184.710

The owner or operator must conduct all tests as required by the OCMI, and make the vessel available for all specific inspections and drills required by subpart H of this part. At each initial and subsequent inspection for certification, the marine inspector may require that an abandon ship or man overboard drill be held under simulated emergency conditions specified by the inspector. At each initial and subsequent inspection for certification, the marine inspector may require that a fire drill be held under simulated emergency conditions to be specified by the inspector. At time of inspection a crew was not provided. Prior to carrying passengers or cargo, provide properly licensed and trained crew who are enrolled in company's drug testing program, and complete fire, man overboard and abandon ship drills to the satisfaction of the attending USCG Marine Inspector.

An inspection for renewal of a Certificate of Inspection is conducted to determine if the vessel is in satisfactory condition, fit for the service intended, and complies with all applicable regulations. It normally includes inspection and testing of the structure, machinery, equipment, and on a sailing vessel. Vessel reported that the stbd fuel tank had bad fuel and at the time of inspection and had the shutoff valve closed. F/O shutoff valves were not tested due to potential fuel contamination. Vessel will need to prove operational capability of valves before carriage of passengers or cargo.

All unfired pressure vessels must be installed to the satisfaction of the cognizant OCMI. The design, construction, and original testing of such unfired pressure vessels must meet the applicable requirements of subchapter F (Marine Engineering) of this chapter. PRVs did not relieve at designated pressure of 150 p.s.i. Replace PRVs and prove to marine inspector a satisfactory test.

Each vessel that relies on electricity to power the following loads must be arranged so that the loads can be energized from two sources of electricity. Stbd generator was out of service due to fuel pump repair. Repair generator and provide marine inspector with proof of satisfactory repair and service test.

You are responsible for all actions of your officials, r epresentatives, and agents in carrying out the requireme nts of the DOT agency regulations. Vessel did not have enrollment with service agent for drug testing. Provide proof of CY24 enrollment with consortium or valid testing program and enrollment.

To qualify for a medical certificate, a mariner must provide evidence of meeting the medical and physical standards in this section on a CG–719–K or CG–719–K/E, as appropriate. Vessel master did not have medical certificate present at the time of inspection. Provide proof of valid medical certificate to marine inspector.

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an emergency. During inspection the following items were found requiring repair. Repairs must be made prior to credit for next scheduled dry docking. (1) Stbd forward gangway entrance aft open portion has a 12-inch crack from the deck to the mid portion of the bulwark. Fully clean area to determine length and depth of weld crack. Provide marine inspector with repair proposal.

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an emergency. During inspection the following items were found requiring repair. Repairs must be made prior to credit for next scheduled dry docking. Pilothouse deck had approximate 6-inch diameter area of exposed and pitted through deck plating. Provide marine inspector with repair proposal.

A hinged watertight bulkhead door must be fitted with a quick action closing devise operable from both sides of the door and indicator lights at the operating station showing whether the door is open or closed. The indicator for the doors located at frames 18 and 20 was inoperable. Repair and demonstrate proper operation.

The emergency lighting required by paragraph (a) of this section must automatically actuate upon failure of the main lighting system. If a vessel is not equipped with a single source of power for emergency lighting, it must have individual battery powered lights that: (1) Are automatically actuated upon loss of normal power; (2) Are not readily portable; (3) Are connected to an automatic battery charger; and (4) Have sufficient capacity for a minimum of 2 hours of continuous operation. Battle lantern type emergency light located on the forwad engine room bulkhead was inoperable. Demonstrate proper operation of emergency lighting.

Each pressure type wire connector and lug must meet UL 486A (incorporated by reference; see 46 CFR 175.600) or other standard specified by the Commandant. The use of twist-on type wire nuts is permitted under the following conditions: (1) The connections must be made within an enclosure and the insulated cap of the connector must be secured to prevent loosening due to vibration; and (2) Twist-on type connectors may not be used for making joints in cables, facilitating a conductor splice, or extending the length of a circuit. An improper wire nut splice was found in way of the potable water pump in the fore belly. Provide proper splice.

Vessel punctured/cracked their port side fuel oil tank. O/O to Provide a repair proposal and make permanent repairs to the fuel oil tank to the satisfaction of the attending MI.

Provide complete and current first aid kit.

Properly mark water tight door at frame 18.

Provide water tight door alarm for water tight door at frame 20.

Provide non-return ball for forward starboard vent on main deck.

Provide screen for potable water tank vent.

Provide 04 alcohol swabs.

Properly install weak link and hydrostatic release on liferafts.

Replace junction box in port aft laz.

Remove dead end wire in laz.

Cap unused through hull valve stbd side E/R.

Open up for further inspection discussed deck leak into portside E/R IWO deck chain locker.

Clad weld isolated pits as marked in forward salon void portside hull plating.

Remove extension cord in stateroom and install an additional outlet.

Properly secure refrigerator in the galley.

Hydro 04 CO2 extinguishers or replace with servicable B-IIs.
