Fire extinguishing equipment installed on a vessel in excess of the requirements of §§ 181.400 and 181.500 must be designed, constructed, installed, and maintained in accordance with a recognized industry standard acceptable to the Commandant (CG-ENG-4).Vessel does not have annual fire detection service certificate.

For vessels other than those covered in paragraphs (b) and (c) of this section, the name of the vessel must be marked on some clearly visible exterior part of the port and starboard bow and the stern of the vessel. The hailing port of the vessel must be marked on some clearly visible exterior part of the stern of the vessel. Paint name on stern to be contrasting in color.

Each ring life buoy on a vessel must: Be approved in accordance with § 160.050 in subchapter Q of this chapter, or other standard specified by the Commandant, including, but not limited to, approval series 160.150. Each ring life buoy needs CG approval sticker. Also renew Worn reflective tape.

A hatch cover must: Have securing devices; Escape hatch on bow missing a dog and had a loose dog.

Each life float must be fitted with a lifeline, pendants, two paddles, a painter, and a light. Life float paddles not secured, retro-reflective tape worn, light missing battery and closure device.

Each vessel must have adequate emergency lighting fitted along the line of escape to the main deck from all passenger and crew accommodation spaces located below the main deck. Provide adequate lighting for aft engine room escape access.

At each initial and subsequent inspection for certification and re inspection the marine inspector shall examine the passenger and crew quarters, toilet and washing spaces, galleys, serving pantries, lockers, etc., to determine that they are serviceable and in a sanitary condition. Secure stove in the galley to prevent movement during rough seas or emergencies.

Where rigid nonmetallic material is permitted for use in piping systems by this subpart, the following restrictions shall be adhered to: Protection from mechanical damage shall be specially considered. Protective covering or shields shall be installed to the satisfaction of the Officer in Charge, Marine Inspection. Replace chaffed oil line to the #1 MDE reduction gear unit.

Remove paper towel used to bypass fire alarm located in the galley overhead.

If your vessel's Certificate of Inspection is renewed annually, the marine inspector must check the settings of the safety or relief valves on all pressure vessels, except cargo tanks, at each inspection for certification. Relief valve did not reset on its own.

Propulsion machinery shall be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. Repair fuel control arm tensioner on #3 Main Diesel Engine.

Battery terminals shall be of the soldered type and all connections shall be made to the battery terminals with permanent type connectors. Spring clips or other temporary type clamps are not permitted. Properly secure battery charger leads to battery terminals.

Vessels described by 46 CFR 175.110(c) must have an interconnected fire detection system in compliance with § 181.450 installed in all enclosed areas where passengers and crew have routine access, including accommodation spaces and machinery spaces. Vessel did not have smoke detection in all accommodation spaces. Ensure smoke detectors are provided in all accommodation spaces and proved technician service report to satisfaction of attending marine inspector.

Each life float and buoyant apparatus must be individually attached to the painter by a line meeting § 180.175(f)(3) (ii), (iii), and (iv) and long enough that each life float or buoyant apparatus can float without contacting any other life float or buoyant apparatus in the group. Life float arrangement had tangled lines and may not properly float free. Ensure life floats and lines are properly installed/arranged per NVIC 01-83.

All observed unsafe practices and hazardous situations shall be corrected. Washer and dryer are not secured. Properly secure washer and dryer to satisfaction of attending marine inspector.

Each Class 1 door of a vessel on ocean service must be designed to withstand a head of water equivalent to 10 feet. Fastening devices on the forward escape door in the passenger area need to be adjusted to proved a tighter closure.

Vessels shall be equipped with a suitable number of portable battery lights. Install an additional emergency light in the engine room entrance.

Protective gratings shall be provided at such locations where persons or gear might come into contact with the exhaust pipe. Exhaust pipe was exposed, install protection on exhaust pipe.

All observed unsafe practices and hazardous situations shall be corrected. Hot water heater safety relief valve not operational due to corrosion and needs replacement.

46CFR183.200(b): Electrical equipment on a vessel must be installed and maintained to: Protect passengers, crew, other persons and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Vessels Port and Stbd Generator windings were dirty.

46CFR320(e): Each motor must have a nameplate attached to it containing the information required by Article 430 of NFPA 70, and for a motor derated in accordance with paragraph (b) (2) of this section, the derated capacity. Vessel Missing date plate on Stbd generator.

Electrical equipment and installations must be suitable for the roll, pitch and vibration of the vessel underway. Vessel Port and Stbd generator pedestal bolts were loose.

46 CFR 183.200 (b): Electrical equipment on a vessel must be installed and maintained to: Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Vessel port generator air intakes were saturated with oil and spry paint.

Each distribution panel or switchboards must be totally enclosed and of the dead front type. Vessel Switchboard had openings on the side and loose cover.

Each propulsion, power, lighting, or distribution system having a neutral bus or conductor must have the neutral grounded. Vessel Ground detection system test function was inoperable.

A voltmeter and an ammeter, which can be used for measuring voltage and current of a generator that is in operation.

Two clamps must be used on each end of the hose, or hose clamp can be used if the pipe ends are expanded or beaded to provide a positive stop against hose slippage. Vessel missing a number of hose clamp on all MDE's reduction gear.

Except as noted in paragraphs (d) and (e) below, each vessel must be equipped with a public address system. Vessel Public address was inoperable.

Each Battery must be located as high above the bilge as practicable, secured to protect against shifting with the roll and pitch of the vessel. Vessel battery had loose connections and exposed wires.

The Shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located , preferably from an accessible position on the weather deck. Vessel fuel tank return valve handle loose and fuel shut off is bound and leaking.

No repairs or alterations affecting the safety of the vessel or its machinery shall be made unless applicable requirements in this chapter are met. The procedures to be followed in notifying the Coast Guard about vessel repairs vary according to the type of vessel and service in which engaged. Vessel sustained a fire with an associated fuel spill on the afternoon of 04JUN2019. Vessel has extensive damage from smoke and fire primarily in the galley and above the galley in the crew space. Prior to movement, company must provide to the OCMI a satisfactory marine damage survey and a satisfactory transit plan (to repair facility) to be approved by the USCG Marine Inspector. If vessel is to transit to repair facility under own power, prove satisfactory sea trials conducted to [name withheld] not, vessel will use assist vessel(s) to transit to repair facility. After vessel arrives at repair facility, the company must provide a repair plan to be approved by the OCMI prior to start of repairs.

Stbd fire hose failed pressure test.

Lifefloat painter lines were incorrectly tied to lifefloats.

Renew stenciling on starboard lifebuoy

Soft patch IVO starboard cargo deck to facilitate main engine removal was missing fasteners allowing water intrusion.

Starboard remote fuel cut off valve was inoperable.

Additional fire fighting equipment required for carriage of flammable/combustible did not have an approved nozzle.

Improper grouping of lifefloats & attachment to incorrectly sized weak links.

Mast halyard secured to aft rail directly crossing over life float installations causing potential obstruction to float free arangement.
