Resolution: Attended OSV LOLA. Witnessed proper function and repair of the weathertight egress from the machinery space on the starboard side.
Each administration shall hold companies responsible for documentation and data relevant to all seafarers employed on its ships are maintained and readily accessible. Ship did not have flag state endorsements available for examination by PSCOS.
