Except when individual pumps are provided for separate spaces, individual bilge suction lines must be led to a central control point or manifold and provided with a stop valve at the control point or manifold and a check valve at some accessible point in the bilge line. A stop-check valve located at a control point or manifold will meet the requirements for both a stop valve and a check valve. Lazarette, E/R, and E/R alleyway bilge suction piping are not fitted with check valves.

Separation of machinery and fuel tank spaces from accommodation spaces. Machinery and fuel tank spaces must be separated from accommodation spaces by boundaries that prevent the passage of vapors.

A vessel of more than 19.8 meters (65 feet) in length must have at least one fire axe located in or adjacent to the primary operating station. Fire axe was found in main deck gear locker and not kept in or adjacent to the primary operation station.

Complete but simple instructions for the operation of a extinguishing system must be located in a conspicuous place at or near each pull box and stop valve control and in the space where the extinguishing agent cylinders are stored. If the storage cylinders are separate from the protected space, the instructions must also include a schematic diagram of the system and instructions detailing alternate methods of releasing the extinguishing agent should the local manual release or stop valve controls fail to operate. Each control valve to a distribution line must be marked to indicate the space served. Local and remote actuator pull box for fixed CO2 system did not have instructions posted.

The name of the vessel must be marked on some clearly visible exterior part of the port and starboard bow and the stern of the vessel. Vessel's name was not marked on port and starboard bow.

Annual inspection. Your vessel must undergo an annual inspection within the 3 months before or after each anniversary date. Vessel has not scheduled an Annual Inspection and has lapsed. 46 CFR 176.500(b)(1)

Vessels described by 46 CFR 175.110(c) must have an interconnected fire detection system in compliance with § 181.450 installed in all enclosed areas where passengers and crew have routine access, including accommodation spaces and machinery spaces. Vessel does not have installed detectors in required spaces by 27Dec2022. Vessel has been limited to a Limited Coastwise route until detector installation has been verified by a Marine Inspector.

On vessels described by 46 CFR 175.110(c), flammable items not otherwise covered by the regulations of this subchapter, such as rechargeable batteries, including lithium ion batteries utilized for commercial purposes, must be handled, stored, and operated in a way that mitigates the risk of hazardous conditions. Vessel has not submitted plan for lithium ion battery charging. Vessel has been limited to a Limited Coastwise route and no overnight passengers until lithium ion charging plan has been verified by a Marine Inspector. 46 CFR 185.364

For a vessel described by 46 CFR 175.110(c), the training program in paragraph (a) of this section must address firefighting proficiency and must include, but need not be limited to (Listed in cite). Vessel has not shown to have met training requirements. Vessel has been limited to a Limited Coastwise route and no overnight passengers until training has been verified by a Marine Inspector. 36 CFR 185.420(b)

Vessels described by 46 CFR 175.110(d) must submit plans to the cognizant OCMI, in accordance with 46 CFR 176.700, for the installation and use of monitoring device to ensure the wakefulness of the watchmen required in paragraph (a) of this section. The Coast Guard will work with the vessel operators to determine a reasonable implementation schedule once the plans are accepted. The monitoring device(s) must:(1) Ensure the wakefulness of the crew in the event that the watchman required in paragraph (a) of this section is unresponsive; (2) Remain operable during the nighttime watch; and (3) Be arranged to ensure proper coverage of the passenger accommodation spaces, common areas, and spaces with potential fire hazards. Vessel has been limited to no overnight passengers until monitoring device has been verified by a Marine Inspector. 46 CFR 185.410(b)

The master of a vessel described by 46 CFR 175.110(d) must conduct passenger emergency egress drills from the passengers' assigned overnight accommodation spaces prior to beginning an excursion with new passengers. (1)If the passengers are not assigned an overnight accommodation space, the master of a vessel described by 46 CFR 175.110(d) must conduct passenger emergency egress drills from an accommodation space prior to beginning an excursion with new passengers. (2) For the purposes of this section, excursion includes anytime the the vessel gets underway, or anytime passengers remain overnight on the vessel. Provide plan for conducting passenger egress to OCMI. Vessel has been limited to no overnight passengers until passenger egress plan has been verified by a Marine Inspector. 46 CFR 185.507

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. Vessel reported a loss of propulsion due to starboard engine failure. Verify cause of engine failure and make necessary repairs. 46 CFR 182.200(a)

Adequate emergency lighting automatically actuated upon failure of the main lighting system shall be fitted along the line of escape in vessels having lounge areas below the main deck. The emergency lighting in the passageway leading to the engine room was not functioning and needs to be repaired or replaced. 46 CFR 184.30-5(a).

Complete but simple instructions for the operation of a fixed gas fire extinguishing system must be located in a conspicuous place at or near each pull box and in the space where the extinguishing agent cylinders are stored. If the storage cylinders are separate from the protected space, the instructions must also include a schematic diagram of the system and instructions detailing alternate methods of release. There were no operating instructions posted for the fixed CO2 system installed on the vessel. 46 CFR 181.20-15(d).

Watertight doors and watertight hatches shall be marked on both sides in at least 1-inch letters. Update the verbage on the watertight doors to read "Watertight door-keep closed while underway" so that it reflects the requirements of the stability booklet. 46 CFR 185.30-25(a).

As appropriate for the intended voyage, all vessels must carry adequate up-to-date charts and nautical publications. The vessel's rules of the road publication is from 2003 and needs to be replaced with an updated version. 46 CFR 185.20-35.

At each initial and subsequent inspection for certification the marine inspector shall examine and test all boilers and unifired pressure vessels to the extent necessary to determine that their condition is satisfactory and that they are fit for the service for which intended. The pressure gauge on the vessel's air receiver is inoperable and needs to be replaced. 46 CFR 176.25-30.

At each inspection the following examinations and tests shall be held: The setting of safety valves or relief valves on boilers and unfired pressure vessels shall be checked. The relief valve for the air receiver did not properly operate and was unable to reseat when lifted. 46 CFR 176.25-30(b)(2).
