Vessel reported port controllable pitch propeller is not operating as designed due to possible blocked valve. 46 CFR 91.30-1(a)

The vessel is required a tug escort throughout its transit within the Port of Virginia or until the port controllable pitch propeller is repaired to the satisfaction of the attending Coast Guard Marine Inspector. The number of tugs, horsepower, and bollard pull shall be determined adequate by the [name withheld] the vessel experiences any propulsion or steering issues while underway you are to follow Coast Guard casualty reporting requirements. 33 CFR 160.111(c)

All automatically or remotely controlled or monitored vital systems addressed by part 62 of this subchapter must be subjected to tests and inspections to evaluate the operation and reliability of controls, alarms, safety features, and interlocks. Test procedures must be submitted to the Coast Guard for approval. PSTPs were amended to reflect system upgrades after fire in engine room. Provide approved PSTPs.

Tests must be conducted at periodic intervals specified by the Coast Guard to confirm that vital systems and safety features continue to operate in a safe, reliable manner. Dirty oil tank PSTPs could not be completed due to tank level indicator not installed.

A lockout valve must be provided on any carbon dioxide extinguishing system protecting a space over 6,000 cubic feet in volume and installed or altered after July 9, 2013. “Altered” means modified or refurbished beyond the maintenance required by the manufacturer's design, installation, operation and maintenance manual. Lock out valve was not provided due to supply chain issues. Vessel scheduled to return to BAE in September to install valve. Valve shall be installed by 30Sept2025.

Conduct repairs to the satisfaction of the vessel's Classification Society and attending CG Marine Inspector.

Due to vessel casualty in engine room, prior to movement vessel will be required: 1) maintain a fire safety watch of the effected space and communicate vessel status every 2 hours to Sector Jacksonville via VHF channel 16 or [phone withheld]. 2) Submit an approved tow plan by USCG SECJAX prior movement or towing evolution. 3) Complete and submit a damage survey prior to repairs.

Boundary bulkheads separating control stations and machinery spaces shall be of "A" class construction. ECR port door closes but does not latch.

In the event of alterations involving the safety of the vessel, the applicable plans shall be submitted for approval covering the proposed work. Vessel installed surveillance system to comply with Safer seas act with out approval. Plans were submitted to Marine Safety Center but have yet to be approved and verified by a OCMI. 46 CFR 91.55-10

Each cargo vessel must carry an immersion suit approved under approval series 160.171 of an appropriate size for each person on board. Vessel had 08 immersion suits that were not SOLAS approved. Vessel restricted to domestic voyages. 46CFR199.273(a)

Each vessel must carry life jackets approved under approval series 160.155. Vessels life jackets were not SOLAS approved. Vessel restricted to domestic voyages. 46CFR199.70(b)

Multiple cables should be supported in metal hangers or trays. Metallic band strapping used for cable support should be steel and corrosion treated in not a corrosion resistant material. Vessels navigation light cable runs were supported with zip-ties. 46CFR111.60-5(a) IEEE 45-2002, Section 25

Lifeboat and rescue boat release gear must be operationally tested under a load of 1.1 times the total mass of the lifeboat when loaded with its full complement of persons and equipment whenever overhauled or at least once every 5 years. Vessel completed weight test without a USCG MI present. 46CFR199.190(i)(5)

Annual inspections must include each survival craft to be stripped, cleaned and thoroughly inspected. Maintenance third party servicing report for vessels lifeboats was unclear to what was completed and what was recommended. 46CFR199.190(a)

Water tight doors shall be designed, constructed, tested and marked with ASTM F1196. Door was unable to be opened manually. 46CFR170.270(a)(1)

Non-metallic hose must be in accordance with J1942. Fuel hoses on emergency generator could not be verified that they are designed for use for fuel oil. 46CFR56.60-25(b)

Valves must be designed for the maximum pressure to which they may be subjected, but in no case shall the design pressure be less than 50 lbs per square inch. Evaporator line valve leaking onto cables in overhead above port MDE. 46CFR56.20-9(c)

Wire must be in an enclosure. Dead end wire not terminated in a junction box or removed port side 2nd level in Pump Room. 46CFR111.60-11(a)

Fire mains must be tested to not less than 150psi. Fire main in engine room had pinhole leak. 46CFR56.97-40(a)(7)

Periodic Safety tests must demonstrate the proper operation of the primary and alternate controls. Vessel could not demonstrate PSTPs for Incinerator. Door on incinerator could not be fully closed & engage interlock. 46CFR61.40-6(a)

The Bunker delivery note shall be accompanied by a representative sample of fuel delivered taking into account guidelines developed by the organization. The sample is to be sealed and signed by the supplier’s representative and the master or officer in charge of the bunker operation on completion of the bunkering operations and retained under the ship’s control until fuel is substantially consumed, but in any case for a period of not less than 12 months from the time of delivery. Fuel Samples onboard were only from the last 6 months.

On vessels required to have a final emergency power source by 112.05-5(a) of this chapter, the following emergency lighting must be arranged so they can be energized from the final emergency power source. E-Lights for emergency escape were burnt out. 46CFR112.15-5

The material, workmanship, and arrangement of main propulsion machinery must be at least equivalent to the standards established by ABS Steel Vessel Rules. The vessel secured its port main engine after the turbo was found to be overheating. Metal shavings were subsequently found in the engine's oil. Upon receiving written permission by Sector Jacksonville COTP/[name withheld], vessel is to proceed to berth in Port Canaveral, FL, and effect repairs to the port M/E to the satisfaction of a Marine Inspector. 46 CFR 58.05-1(a)

The general construction of the vessel shall be such as to minimize fire hazards. Oil soaked lagging was found on the M/E exhaust leading to the turbo on the port M/E.

The integrity of a stairtower shall be maintained by "A" Class doors at every level. The doors shall be of self-closing type. Door at Nav Deck does not latch closed and door leading up to the bridge is taped opened due to a broken door knob.

Each lifeboat must be provided with a launching appliance. The launching appliance must be capable of launching and recovering the lifeboat with its crew. Each launching appliance arrangement must be designed to ensure persons can safely disembark from the survival craft prior to its stowage. During Lifeboat drill, noted both Chief Engineer and Second Mate were leading the winch remote control wire with their hands to ensure wire rope stayed on pulleys and unreeled/wound on drum correctly. Vessel to provide third party report from an approved servicing company stating the launching appliances for the lifeboats are operating as intended.

Connections and terminations in all conductors must retain the original electrical, mechanical, flame-retarding properties of the cable. Noted various dead end wires on deck.

The material, design, construction, workmanship, and arrangement of main propulsion machinery and of each auxiliary, directly connected to the engine and supplied as such, must be at least equivalent to the standards established by the ABS Steel Vessel Rules (incorporated by reference, see 46 CFR 58.03–1), except as otherwise provided by this subchapter. ABS; Hot surfaces likely to come into contact with the crew during operation are to be suitably guarded or insulated. Where the temperature of hot surfaces are likely to exceed 220°C (428°F), and where any leakage, under pressure or otherwise, of fuel oil, lubricating oil or other flammable liquid is likely to come into contact with such surfaces, they are to be suitably insulated with non-combustible materials that are impervious to such liquid. Insulation material not impervious to oil is to be encased in sheet metal or an equivalent impervious sheath. Inspector observed oil-soaked lagging on Port MDE Turbo/exhaust piping.

All automatically or remotely controlled or monitored vital systems addressed by part 62 of this subchapter must be subjected to tests and inspections to evaluate the operation and reliability of controls, alarms, safety features, and interlocks. Test procedures must be submitted to the Coast Guard for approval. In accordance with 46 CFR 61.40, design verification and periodic safety testing of automated vital systems verifies system design, construction and operation according to applicable Part 62 requirements. Please note these vessel specific test procedures are not for the purpose of replacing specific personnel or to reduce overall crew requirements.*This is for submittal of the amended test procedures, will amend as necessary once submitted to MSC.

Where a carbon dioxide extinguishing system is installed, the provisions of this subpart, with the exception of § 95.15-90, shall apply. CO2 system to paint locker is in-op and requires repair. Stowage of flammable/combustible products in the Forward paint locker prohibited and space shall be marked as such.

The installation of all systems of an electrical engineering or interior communication nature, together with the details of design, construction, and installation, shall be in accordance with the requirements of Subchapter J (Electrical Engineering) of this chapter. Emergency generator starter has hydraulic leak and combination jacket water/lube oil pressure switch needs replacement (current arrangement has been tested but is not meant for a permanent arrangement), permanent repair required (parts on order).

A nonmetallic expansion joint must be replaced 10 years after it has been placed into service if it is located in a system which penetrates the side of the vessel and both the penetration and the nonmetallic expansion joint are located below the deepest load waterline. MI's unable to verify when the two expansion joints in the pump room were replaced. Provide objective evidence when those were put into service.

Each self-propelled vessel must have a navigation light indicator panel in the navigating bridge to control side, masthead, and stern lights. The panel must visually and audibly signal the failure of each of these navigation lights. Nav light panel audible signal was found inoperable. Repair and prove proper operation.

All ships of 150 GT and upwards shall carry a spare magnetic compass interchangeable with the compass referred to in paragraph 2.1.1. Crew could not locate the spare compass. Provide spare compass. 74SOLAS(20) V/19.2.2.2.1

The master, owner, operator, agent, or person in charge of any vessel equipped with ballast water tanks that operates in the waters of the United States must follow these practices: Maintain a ballast water management (BWM) plan that has been developed specifically for the vessel and that will allow those responsible for the plan's implementation to understand and follow the vessel's BWM strategy and comply with the requirements of this subpart. Vessel's BWM Plan reflects ballast water exchange as a method of BWM. Ballast exchange is no longer an approved method past the vessels compliance date. Amend plan to reflect approved management methods.

Simple operating instructions with block diagram showing the change-over procedures for remote steering control systems and steering gear power units shall be permanently displayed on the navigation bridge and in the steering compartment. Noted that instructions and block diagram were not available at the bridge steering station. Provide instructions and diagram as required. 74SOLAS(20) V/26.3.1

A fixed CO2 system shall be installed in all spaces containing internal combustion main propulsion machinery. CO2 cylinders have been removed for re-placement after accidental discharge. Provide service report for system inspection after installation. 46 CFR 95.05-10(e)

Fuel oil pneumatic shut off valve for the service tank was inoperable. Repair to the satisfaction of the attending USCG Marine Inspector.

During testing, stack fire dampers did not close all the way. Repair and adjust to the satisfaction of the attending USCG Marine Inspector.

Emergency escape hatches throughout the vessel are not functioning as intended and the assist spring needs replaced or serviced. Repair/Replace to the satisfaction of the attending USCG Marine Inspector within 7 days.

During the fire drill, crew did not have required communication devices (radios) or a lifeline for fire team. Crew struggled with testing the low air alarm after the drill. Ensure approved radio communications and a lifeline are put into service on board the vessel and conduct sufficient training with crew to the satisfaction of the attending USCG Marine Inspector within 7 days.

Vessel's CO2 system, MI's observed 2 slices in one of the flexible loops. Dust was found on bottles questioning if they were weighed correctly by 3rd party. Provide a 3rd party servicing report attesting to the maintenance and operation of system within 7 days.

Several fire doors were found to be held back with either rope or a hold back device. In addition, during testing, some fire doors would not close completely. Remove hold back devices and adjust fire doors to ensure the vessel is maintaining fire boundaries to the satisfaction of the attending USCG Marine Inspector within 7 days.

Vessel was missing A60 insulation in the engine room. In addition, insulation was missing in the galley above the air conditioning unit. Replace and ensure all structural fire boundaries are maintained to the satisfaction of the attending USCG Marine Inspector within 7 days.

Galley waiter door has not been tested per manufacturer's recommendation from a 3rd party. Provide a written report attesting to the proper maintenance and testing of the galley waiter door to the satisfaction of the attending USCG Marine Inspector within 7 days.

Excessive grease was found near the deep fat fryer and galley hoods. All electrical components were covered with grease. Clean to the satisfaction of the attending USCG Marine Inspector within 7 days.

Oil found inside a light fixture housing in the engine room and one cracked light housing in the engine room. Identify/secure the leak, clean housing, and replace cracked light cover to the satisfaction of the attending USCG Marine Inspector within 7 days.

CAPAC monitoring and control panel located in engineering control room is showing a fault. Provide servicing report addressing fault and repairs.

Each panelboard switching unit must be numbered and have a circuit directory cardholder; and a circuit directory. Multiple distribution panels throughout, were found with missing breaker numbers and confusing labeling of old and new equipment. Make corrections to breaker numbers and card holders as necessary.

The status of automatically or remotely controlled vital auxiliaries, power sources, switches, and valves must be visually indicated in the machinery spaces or the cognizant remote control location, as applicable. Multiple lights were found extinguished on the ventilation panel. Replace bulbs and necessary.

Electric installations on vessels must ensure Protection of passengers, crew, other persons, and the vessel from electrical hazards. Emergency switchboard and previously installed air compressor control switch were found with missing covers. Install covers as required.

Dead end wires were found for the old compressor push button starting panel. Cap wires to the satisfaction of the attending USCG Marine Inspector.

Lifeboats retro-reflective tape on the upper hemisphere of the lifeboat is wasted and needs to be replaced.

During the examination of the vessel's Garbage Record Book, MI's found that no garbage receipts had been kept since 2011. Conducted detailed training with mate and Captain on the requirements of MARPOL Annex V. In addition, the vessel had an outdated Garbage Management Record Book. Provide proof of an updated record book to the satisfaction of the attending USCG Marine Inspector.

Vessel had outdated life jacket donning placards on board the vessel. Ensure updated lifesaving donning placards are placed on board the vessel and remove the old ones.

Marine Inspectors found several ring buoys (5) that had wasted line, punctured bung holes, and wasted retro tape. Crew had extras and replaced them on the spot.

MI's observed the hospital space being used as berthing for the ship's steward. The hospital space is not authorized for continuous occupancy for any of the crew and needs to remain empty in case of emergency. Clear out the hospital space and ensure each crew member has appropriate berthing.

Observed soft patch on the starboard sea water cooling overboard piping for the starboard main engine.

Quarterly lifeboat launches were not being done as required by subchapter "W". Lifeboats were instead only being brought to the water's edge as per the electronic SMS checklist which was based only on old subchapter "I".

Hospital space was occupied by a crewmember as a regular berth. Space needs to be cleared out & not used as berthing.

No embarkation station ladder or emergency lights were found for the forward 6-man liferaft. Vessel to install adequate embarkation ladder and emergency lighting.

Vessel carries nuclear material onboard as part of a sensor system for dredging ops. Vessel required to have a federal license issued by the Nuclear Regulatory Commission (NRC) or by a State's equivalent agency that also has a reciprocity agreement with FedGov. Only Louisiana license found onboard w/no proof of reciprocity agreement.

THE PORT LIFEBOAT/RESCUE BOAT HAS TWO AREAS OF DAMAGE IN WAY OF THE PORT GUNWALE. FIRST, THERE IS A FORWARD SECTION APPROXIMATELY A FOOT AFT OF THE FRONT BREAST PLATE THAT HAS A 2.5" CRACK. SECOND, UNDER THE AFT GRIPE PLATE, DELAMINATION HAS BEGUN TO TAKE PLACE.

The temporary repair to the starboard shaft is approved, and the vessel must make permanent repairs at the next scheduled dry dock in July 2017. However, should the temporary repair fail in anyway, the vessel is to make report of it to the local OCMI and proceed immediately to dry dock and effect permanent repairs.

Provide approved plans for the newly installed fuel cooler and its piping where it was altered from the existing fuekl piping and the raw water piping.

Prior to Coast Guard review/approval for the shaft repair proposal, DUTRA must provide verification from Wartsilla and the OEM, of their acceptance for the temporary repair. Once reviewed and approved by the OCMI, all NDT testing, installation, and additional tests must be witnessed/accepted to the satisfaction of the attending marine inspector.

Emergency gen would not start when performing Periodic Safety Test Procedures.

Vessel did not want to test thier relief valves.

vessel was unable to get the relief valve to pop off at the marked setting on the valve.

Vessel is required to submit amendments for PSTP to MSC.
