I'M ALONE

Except as provided by § 15.725, no vessel may be navigated unless it has in its service and onboard the crew complement required by the COI. Vessel has insufficient manning in accordance with COI. Prior to operating, vessel must prove adequate manning and conduct satisfactory drills in the presence of a Marine Inspector. 46 CFR 15.515(a)

Each vessel that operates on the high seas, or that operates beyond three miles from the coastline of the Great Lakes, must have on board a FCC Type Accepted Category 1, 406 MHz EPIRB, installed to automatically float free and activate. Hydrostatic release for EPIRB was expired. 46 CFR 180.64

Vessels described by 46 CFR 175.110(c) must have an interconnected fire detection system in compliance with § 181.450 installed in all enclosed areas where passengers and crew have routine access, including accommodation spaces and machinery spaces. 02 smoke detectors in the overnight accommodation space were giving error codes and were being replaced. 46 CFR 181.405

Each life float must be fitted with a lifeline, pendants, two paddles, a painter, and a light. 02 water light batteries were expired. 46 CFR 180.175(d)

Each life jacket carried on a vessel on oceans, coastwise, or Great Lakes route, must have a life jacket light approved in accordance with § 161.012 in subchapter Q of this chapter. 02 life jacket lights inop 46 CFR 185.75

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date:(1) Charts of large enough scale to make safe navigation possible; U.S. Coast Pilot or similar publication; Coast Guard Light List; Tide tables. Vessel publications were not up to date. 46 CFR 184.420(a)

Fire fighting and abandon ship drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. Vessel had not logged any drills in over a year. Provided vessel with log booklet. 46 CFR 185.524(d)(1)(2) 46 CFR 185.520(f)(1)(2)

The master of the vessel shall ensure that the EPIRB test required by paragraph (a) shall be logged or otherwise documented, as applicable. Vessel did not have a log of monthly EPIRB tests. Conducted satisfactory test of EPIRB and provided vessel with log booklet. 46 CFR 185.728(c)

A vessel to which this subchapter applies may not be operated without having on board a valid U.S. Coast Guard Certificate of Inspection. Upon inspection, MI determined vessel was not ready for inspection. Ensure vessel is in compliance and reschedule inspection at a later date.

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Inspection found blank spot where breaker was removed from electrical panel in the engine room.

Each rescue boat and ring life buoy must be marked with Type II retroreflective material approved in accordance with 46 CFR 164.018. Inspection found degraded reflective tape on life rings and life floats.

Shutoff valves, installed so as to close against the fuel flow, shall be fitted in the fuel supply line and... shall be operable from outside the tank compartment. Fuel valve servicing the port fuel tank has wire buckle that is catching on the deck plate not allowing the valve to actuate closed. Make repairs to enable the valve to be fully operable.

Each vessel on an Oceans service must have an approved EPIRB, that is stowed in a manner that makes it capable of being float free if the vessel sinks. The hydrostatic release for the EPIRB is expired. Install a unexpired hydrostatic release and mark expiration date upon installation.

Vessel does not have an interconnected smoke detection and alarm system. Vessels on a Coast wise or Oceans route must have an interconnected fire detection system in compliance with § 181.450 installed in all enclosed areas where passengers and crew have routine access, including accommodation spaces and machinery spaces. § 181.450: An interconnected detection and alarm system must: (1) Consist of multiple-station smoke detectors listed by an NRTL, or independent laboratory accepted by the Commandant according to 46 CFR subpart 159.010, as meeting UL 217. (2) Be installed such that the actuation of alarm in one area results in both audible and visual alarms in all areas required by 46 CFR 181.405(c) to be protected by the interconnected detection and alarm system; (3) Contain an independent power source; and (4) Alarm on low power. Provide evidence to the local OCMI that an interconnected detection system meeting the above requirements has been installed and is functional.

Except as otherwise provided, each space accessible to passengers or used by the crew on a regular basis, must have two means of escape... and must be capable of being opened by one person from either side. Vessel's forward escape hatch is only capable of being opened from inside the cabin. Make hatch capable of opening from both sides prior to 27DEC23.

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. 01 smoke detectors were expired. No overnight passengers until detector is replaced. 46 CFR 176.830(a)

Each life float must be fitted with a lifeline, pendants, two paddles, a painter, and a light. 02 batteries from each life float were expired. 46 CFR 180.175(d)

Each floating water light must be approved in accordance with § 161.010 in subchapter Q of this chapter, or other standard specified by the Commandant. 02 batteries from ring life buoy water lights were expired. 46 CFR 180.70(d)(1)

Each survival craft must be: Stowed so that when the vessel sinks the survival craft floats free. Lines for rescue boats obstructed ability for life floats to float free. 46 CFR 180.130(a)(2)

The float-free link required by § 180.130(a)(1) must be: Certified to meet § 160.073 in subchapter Q of this chapter, or other standard specified by the Commandant. Float-free link for each life float was missing tag certifying approval. 46 CFR 180.137(b)(1)

The equipment required for life floats and buoyant apparatus must meet the following specifications: Each paddle must be of at least 1,220 millimeters (4 feet) in length, lashed to the life float to which it belongs and buoyant. Paddles were not lashed to each life float. 46 CFR 180.175(f)(2)

At each inspection for certification and at every other vessel inspection the bilges and other spaces may be examined to see that there is no excessive accumulation of oil, trash, debris, or other matter that might create a fire hazard, clog bilge pumping systems, or block emergency escapes. Vessel had excess engine oil stored in engine room. Remove excess oil or fill storage tank on vessel. 46 CFR 176.830(b)

Marine employers shall establish programs for the chemical testing for dangerous drugs on a random basis of crewmembers on inspected vessels. Vessel could not provide satisfactory proof of random drug test program for all crewmembers. 46 CFR 16.230(a)

To enhance protection of life and property it is mandatory that each 406.0-406.1 MHz EPIRB be registered with NOAA before installation and that information be kept up-to-date. EPRIB registration with NOAA was expired. Vessel is limited to operation up to 3nm offshore. 47 CFR 80.1061(f)

The master of the vessel shall ensure that: Each EPIRB, other than an EPIRB in an inflatable life raft, must be tested monthly, using the integrated test circuit and output indicator, to determine that it is operative; The EPIRB test required by paragraph (a) shall be logged or otherwise documented, as applicable. Monthly tests of EPIRB were not conducted or logged. Provide documentary evidence of updated log inspections to Marine Inspector. 46 CFR 185.728(a)(c)

Portable extinguishers must be inspected monthly as required by NFPA 10 conducted by the owner, operator, person-in-charge, or a designated member of the crew. Monthly inspections of portable fire extinguishers were not conducted. Provide documentary evidence of updated log inspections to Marine Inspector. 46 CFR 176.810(b)(1)

Abandon ship and man overboard drills and training, firefighting drills and training, and inspection and maintenance of survival craft shall be logged or otherwise documented for review by the Coast Guard upon request. Vessel log was missing abandon ship and man overboard drills and training, firefighting drills and training, and inspection and maintenance of survival craft. Provide documentary evidence of updated logs to the Marine Inspector. 46 CFR 185.520(f) 46 CFR 185.524(d) 46 CFR 185.702(c)

A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. First kit onboard had expired contents. Replace aspirin and eye wash in first aid kit. 46 CFR 184.710

The operator of each self-propelled vessel 12 meters or more in length shall carry, on board and maintain for ready reference, a copy of these Rules. Vessel was missing Inland Navigation Rules. 33 CFR 83.01(g)

The EPIRB test required by 46 CFR 180.64 shall be logged or otherwise documented, as applicable. The EPIRB registration document from NOAA was missing. Replace current registration documentation.

No up to date publications were found onboard. Replace required publications with current editions.

Each item of lifesaving equipment determined by the marine inspector to not be in serviceable condition must be repaired or replaced. 02 life-rings found with deteriorated markings and 01 found with deteriorated retroreflective material. Repair deteriorated markings and type II retroreflective material. 46 CFR 176.808

Inspection and maintenance of lifesaving equipment shall be logged. Found no record of inspection or maintenance. Perform required maintenance and inspections and update/create log.

The owner or managing operator must provide evidence of required servicing and inspection of firefighting equipment. Found no record of monthly inspection of portables IAW NFPA 10. Perform inspection and update/create log.

Type 1 EPIRB battery must be replaced after use or prior to battery expiration. Found battery expired. Replace battery. Vessel restricted to not more than 3 miles from shore until rectified.

Each survival craft must be in good working order. Found 02 bouyant apparatus in unrepairable condition. Replace with acceptable survival craft.

Crew drills must be performed and logged at least once every three months. Found log not updated with no drills logged in the last year. Perform required drills and update log.
