The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. The following conditions were observed: (a) The ballast water treatment system motive water pump was found to have a spraying leak coming from the housing. Repair/renew the pump. (b) The auxiliary boiler on the tug was unavailable for testing. Prove proper operation of auxiliary boiler shut downs in accordance with the vessel’s approved Periodic Safety Test Procedures. (c)The port and starboard sea chest valves below the ballast water treatment system on the barge were found to have excessive leaking from the packing glands. Adjust/replace packing on both valves. (d) The anchor wash piping above the CO2 bottles in the barge forward CO2 space was found to be wasted. Provide plan to return piping to original condition. (e) The engine room fixed gas detection system was last serviced in August 2017. Conduct annual servicing of gas detection system and provide servicing report.

All the life-saving appliances shall be kept in working order and available for immediate use. The starboard lifeboat did not start via primary means, but successfully started via secondary means. Demonstrate proper operation of the primary starting means. Additionally, the starboard lifeboat rudder support is loose or damaged. Repair or replace the support.

Retroreflective material must be fitted on lifeboats as specified in Annex 1, Paragraph 1. The retroreflective material on the port lifeboat was found to be degraded. Renew retroreflective material.

Oil filtering equipment shall ensure that any oily mixture discharged into the sea after passing through the system has an oil content not exceeding 15ppm. The oily water separator was unavailable for testing. Prove proper operation of the OWS alarms and shutdowns.

Manholes and flush scuttles on the weather deck shall be closed by substantial covers capable of being made watertight. The line scuttle in overhead of barge forward CO2 room does not close completely. Make scuttle watertight.

The vessel shall be fitted with a daylight signaling lamp, or other means, to communicate by light during day and night. The bridge signal lamp was found not to be working. Demonstrate proper operation of signal lamp.

All electrical apparatus shall be so constructed and so installed that danger of injury in ordinary handling shall not exist. The following electrical hazards were noted during this inspection: In the barge aft electrical room: (a) The aft lighting panel was found with an exposed breaker receptacle. Install a blank to prevent accidental shock. (b) The tug-to-barge power panel cover was found to be secured with only one bolt. Replace the many missing bolts to properly secure the cover. (c) The ground detection light on the 120 volt panel did not illuminate when the test button was pushed. Identify cause of the electrical ground and repair/replace equipment as needed. (d) The ground detection test button on the 440 volt panel remained depressed after being pushed. Repair/replace the test button to ensure proper operation. In the barge forward paint locker/CO2 space: (a) An extension cord was found routed thru a penetration in the bulkhead separating the CO2 Room from the Paint Locker. A black cable was found plugged into the outlet by the main deck access door and routed through the same penetration. Remove unauthorized extension cord and black cable. (b) The bulkhead penetration through which the extension cord and black cable were routed was found to be open. Provide means to ensure the penetration provides an equivalent fire rating to the surrounding bulkhead. (c) An oscillating fan was found mounted above the work bench in the paint locker. A damaged light fixture was found in the aft port overhead of the paint locker. Neither of these were rated for installation in the paint locker. Remove unrated electrical equipment from the paint locker.

Ships shall be provided with a main steering gear to the satisfaction of the Administration. During a test of the steering gear, the rudders failed to demonstrate a full 70 degree range of motion. Prove full range of motion of the rudders.

The steering gear compartments shall be provided with suitable arrangements to ensure working access to steering gear machinery and controls. No means to prevent slips and falls were found in the vicinity of the steering gear. Provide handrails and gratings or other nonslip surfaces.

There must be a fixed container or enclosed deck area under or around each fuel oil or bulk lubricating oil tank vent, overflow, and fill pipe. Several containment areas on both the tug and barge were found to have drain openings with no means of closure. Provide mechanical closure of all containment drains.

For tonnage purposes, exterior openings temporarily closed by cover plates held in place only by hook bolts must comply with 46 CFR 69.117(d)(5). Modify cover on tug aft main deck opening to meet these requirements.

Each cylinder of compressed gas being transported by vessel must be stowed appropriately. Approximately 15 portable propane tanks were found unsecured on the deck of the barge forward paint locker. Stow propane tanks in accordance with 49 CFR 176.200.

Fire protection systems and fire-fighting systems and appliances shall be maintained ready for use. The fire station hose rack in the Engine Room, starboard side, 19’6” deck level, was found not secured to the bulkhead or framing. Properly secure hose rack.

The following basic principles underlie the Regulations in this Chapter: restricted use of combustible materials. Excessive amounts of combustible packing materials and engineering consumables were found stored outside the starboard machine shop. Reduce the amount of combustibles to the extent practicable.

The ring, lines, and letters of the load line mark shall be permanently marked on the sides of ships. The load line markings on the tug were found not to be permanently marked. Permanently affix the load line markings.

Every ship shall have scales of draughts marked clearly at the bow and stern. Several of the starboard aft tug draft marks were not of a contrasting color to the hull. Ensure all draft marks are clearly marked.

LENGTH DIFF: greater/smaller than expected vessel length

During attendance with class, it was observe vessel have one 25-man raft on the STBD. Based on the observation, the stowage location is determine to be NOT on an open deck for easy transfer and launch. Issue 835 to install a second raft to meet 100% requirement of both port and STBD side. Modification will be required to the railing so lifting not more than 300 mm in order to launch.

Prior to departure, make permanent repairs to soft patch on fire main piping on main deck, starboard side, aft of Cargo Hold No.6. to the satisfaction of the attending class society surveyor.

An updated Periodic Safety Test Procedure is to be submitted to Houston Engineering incorporating vessel crew changes to how the power management system and automation testing is performed. The updated procedure is to include updated test methods for the Raytheon steering gear system including all alarms. U.S. Supplement, Section I, 1999, Part 4/11.1.4

Prior to loading dangerous goods as defined in the IMDG Code, the vessel is to be reviewed for compliance with USCG Letter 16700/2017 dated 1 December, 2017. USCG letter 16700/2017-1218.

Provide statement from lifeboat hook OEM (Schat Harding) as to the satisfactory installation of the hooks to specifically address corrosion on aluminum feet and increased separation from hook and topside through plate. ABS supplement Cite III//4

Vessel reported issue with Reverse Power relay upon arrival. Vessel was gicen a one tug escort while operating in port until part is fixed.

Based on the failure of the fire main piping and due to objective evidence discovered during a damage survey on the ships' fire main system, an external audit of the SMS is requested.
