Before the vessel leaves port and at all times during the voyage, each lifesaving appliance must be in working order and ready for immediate use. Vessels #7 lifeboat lowering wire parted during routine maintenance and the #9 lifeboat davit will not lower. Vessels total passenger count is reduced by 240 passengers making the total passenger count allowed until repairs are made 2,200 passengers.

On Monday, February 9, 2026, Sector Mobile was notified of leaking sewage pipe inside of the 4C ballast tank, which caused the sewage tank to fill up when the 4C ballast tank was filled to over 91% full. The suspect pipe was determined to be located in the shower drain in 4-62-2. The company is making preparations to attend dry dock in March 2026 and requested a temporary repair which was approved by ABS with Sector Mobile concurrence. Vessel must complete temporary repair proposal and monitoring plan submitted on 09FEB26. Permanent repairs shall be made prior to completion of next dry dock due 31MAR2026. 46 CFR 72.01-15(a)

Vessel has severe damage between the 4C ballast tank and anti-roll tank that has been temporarily repaired to the satisfaction of the USCG. Complete permanent repairs during the 2025 dry docking. Vessel must acquire a permit-to-proceed prior to departing the San Diego inspection zone to complete repairs. Cite: 46 CFR 92.01-10

Starboard tender's powered bilge pumps are not operational. Conduct repairs of the affected pumps and demonstrate satisfactory operation for attending USCG marine inspector. The starboard tender may not conduct tendering operations until this deficiency is rectified. Cite: 46 CFR 182.520.

Both tenders do not have serviceable fire-buckets. Replace both tender's fire buckets in accordance with 46 CFR 181.610. Neither tender may not conduct tendering operations until this deficiency is rectified. Cite: 46 CFR 181.610.

Before the vessel leaves port and at all times during the voyage, each lifesaving appliance must be in working order and ready for immediate use. The No. 7 Liferaft Davit was found to be inoperable due to excessive wastage of the gear box.

The #5 lifeboat's engine failed multiple times during in-water lifeboat testing. Identify cause of failure and repair. The #5 lifeboat may not be used to determine the vessel's total person count until this deficiency has been rectified to the satisfaction of the attending USCG marine inspector. Cite: 74 SOLAS (1997) Reg 20.1.1.1

While vessel personnel were conducting weekly testing of lifeboat engine, found the engine would not turnover. Further inspection found the engine oil sump to be full of water. Repair engine. Passenger count shall be reduced by 133 until repairs are made.

During abandon ship drills, it was discovered that the No. 1 Tender Vessel's davit was not fully operational, with electrical, hydraulic, and brake issues. The No. 1 Tender Vessel may not be used to determine the vessel's total persons count until this deficiency has been rectified to the satisfaction of the attending Marine Inspector.

Received a failed foam analysis report for the AFFF stored in the flight deck and machinery space storage tanks. Both AFFF samples did not meet the required expansion ratio requirements of 4.5%. Please submit a satisfactory foam analysis report within 30-days.

Each bus-tie between a main switchboard and an emergency switchboard must: (a) Disconnect automatically upon loss of potential at the emergency switchboard; (b) Be arranged to prevent parallel operation of an emergency power source with any other source of electric power, except for interlock systems for momentary transfer of loads. Vessel reported that its EDG Auto-Manual selector switch is malfunctioning to include its Auto mode function. During return voyage to San Diego, CA, vessel shall provide a watch stander in the EDG space (with sufficient communications equipment) to activate the EDG for any possible loss of power. If manning is insufficient to maintain this additional watch station, provide a proposal of equivalent safety. During transit within port (from sea buoy to final berth) vessel shall maintain the EDG ready on standby to take electrical load in case of loss of power. Upon safe return vessel shall make permanent repairs to EDG.

All emergency equipment shall be maintained in an operative condition. Vessel reported GMDSS SAT C system cannot log in or connect to any region. System will not sych with NSC or NCC. Upon return to home port vessel vessel will make permanent repairs.

Each fall used in a launching appliance must be renewed when necessary due to deterioration or at intervals of not more than 5 years, whichever is earlier. Both tender boat davits are past their 5 year maintenance window. Both tenders may not be utilized until appropriate maintenance of the davits has been completed. The tenders may not be used to determine the vessel's total person count until deficiency is rectified.

Accommodations provided for officers and crew on all vessels shall be securely constructed, properly lighted, heated, drained, ventilated, equipped, located, arranged, and insulated from undue noise, heat, and odors. Seals on the vessel's deck drains connected to the sewage system have been reported as not being sufficiently maintained, resulting in releases of hydrogen sulfide and VOCs to accommodation spaces. Complete adequate maintenance of deck drain seals to prevent further sewage vapor releases.

Fire door F.S.D. 70 gets caught on a nearby escape scuttle during closure. Adjust F.S.D. 70 as necessary to prevent it from getting caught.

02 open cable transits were discovered in stair tower 4-68-2. Properly re-pack both cable transits with approved materials.

The #9 lifeboat stalls during operation and has difficulty starting. Diagnose and repair the cause of the failure and conduct a satisfactory operational test in the presence of a USCG marine inspector. Total persons authorized on board reduced to 2,184.

The emergency switchboard was found with a 100 amp breaker installed for the keep warm emergency generator heater. The circuit conductor size is rated for 64 amps. Tag-out current circuit breaker, provide keep warm heater with adequate over current protection and update ship's drawings.

Approximately 80% of all life rafts were fitted with non-approved weak links. Provide approved weak links for all life rafts.

The door of freeze stores reefer 01-38-2 has an inoperable door handle mechanism inside the reefer, which will result in it locking personnel inside. Repair/replace the door opening mechanism so that it may be operated from both sides.

Each launching appliance for lifeboats shall be arranged that the fully equipped lifeboat can be safely launched when boarded with not more than the required operating crew on board. Vessel reported lifeboat #3 launching appliances failed to launch boat during testing. Inspection shall be made by an approved ABS/OEM lifesaving servicing technician. Provide date of scheduled inspection and technician report with root cause analysis to Sector San Diego. Total persons authorized on board reduced to 2,312.

HP Turbine experienced a fire during its voyage. Damage was minimal to HP turbine unit. Machinery was placed back on line with no limitations.

SA6 watertight door spring failed internally. Complete permanent repairs to the SA6 watertight door.

It was reported that the vessel's port boiler has a leaking boiler tube. Firing the affected boiler is prohibited until repairs have been completed. The boiler shall be opened for a fireside inspection of the affected boiler, under suitable hydrostatic pressure, to determine the location of the leak (this may require the removal of lagging and/or refractory to locate the origin of the leak). The vessel shall arrange for a Marine Chemist to certify the affected boiler as "safe for workers" prior to USCG attendance - this certificate shall be maintained throughout the repair process. Vessel shall make permanent repairs to leaking boiler tube(s) where identified (plan approval may be required in accordance with 46CFR59.01-5). Upon completion of the repairs, provisions shall be made to conduct Hydrostatic test of the repaired boiler at 1.5 MAWP in accordance with 46CFR61.05 with a Coast Guard Marine Inspector in attendance.

While conducting hydrostatic test of the repaired port boiler superheater tubes, additional leaks were observed in-way-of the water wall header. Identify exact location and submit repair proposal for leak(s) in-way-of water wall header. The vessel shall arrange for a Marine Chemist to certify the affected boiler as "safe for workers" prior to USCG attendance - this certificate shall be maintained throughout the repair process. Vessel shall make permanent repairs to leaking boiler tube(s) where identified. Upon completion of the repairs, provisions shall be made to conduct Hydrostatic test of the repaired boiler at 1.25 MAWP in accordance with 46CFR61.05 with a Coast Guard Marine Inspector in attendance. Firing the affected boiler is prohibited until repairs have been completed.

Make permanent repairs to holed spool piece on sea-chest piping for the #3 and #4 evaporators.

The brakes must be capable of stopping the descent of the survival craft and holding the survival craft loaded with its full complement of persons and equipment. The brake on lifeboat/tender #1 did not hold the survival craft while conducting drills. Make repairs to brake to hold "A" and "B" weight with Coast Guard in attendance. Reduction of passenger by 150 on starboard side. Total persons allowed 2290.

Each lifeboat must be plainly marked with the number of persons for which the boat is equipped and must be clearly marked in permanent characters. Lifeboat/Tender #1 did not have making showing number of persons. Mark lifeboat/tender with appropriate persons for which the boat is equipped.

Each lifejacket must have a lifejacket light that is approved, operational and not expired. Numerous lifejacket lights where found to be expired. Verify all 2576 lifejackets have lights that are not expired.

Replace embarkation ladders at lifeboat station #3 and liferaft station #9. Due 17Jun2019. Extended to 01Jul2019, parts on order.

Passenger and crew spaces shall have sufficient illumination provided by the emergency lighting source under emergency conditions to provide for safe egress from each space. Emergency lighting located in the enlisted male and female berthing spaces (A through H) does not allow for safe egress under emergency conditions. Ship is to install sufficient emergency lighting in berthing spaces to allow for safe egress prior to billeting persons in the spaces identified.

Repair rescue boats door seals. Due 17Jun2019.

Replace port side tender floating water light. Due 17Jun2019.

Demonstrate the proper operation of all teh open lifeboats by launching, manuevering and recovering them to/from the water prior to departure and NLT 01MAY2018.

A dime sized hole was discovered in the firemain overboard discharge pipe. Provide repair proposal for approval to repair fire main discharge pipe as original. Repair proposal shall include all materials used to include welding materials, weld procedure, and proposed method of testing after repair. Ref: 46 CFR 71.55-1

Repair burnt/damaged lagging on the steam piping on teh port ship's boiler next to the superheater safety valve.

Demonstrate proper operation of the boiler safety valves prior to boiler operations/sailing.
