Approval of bowls other than steel construction on fuel filters will specify if a flame shield is required. Additionally, valves for removing impurities or water from the water trap shall be provided with caps or plugs. The RACOR style fuel filters provided require a flame shield and the a means to plug or cap the drain valve provided. 46 CFR 482.455(b)(6&9)

A person may not employ or engaged an individual in a position required to hold an MMS unless that individual maintains a current medical certificate. Master on duty did not have a medical certificate. Provide medical certificate prior to being employed or engaged in a position that requires a merchant mariner credential by law or regulation or by an employer as a condition of employment. 46CFR15.401(c)

Each vessel that operates on the high seas, or that operates beyond three miles from the coastline of the Great Lakes, must have on board a FCC Type Accepted Category 1, 406 MHz EPIRB, installed to automatically float free and activate. The vessel’s EPIRB registration is expired and rated for 12 persons. Provide proper EPIRB registration, and an EPIRB rated for 24 persons. Vessel restricted to stay within 3nm from shore. 46CFR180.64

A vessel on a limited coastwise route must carry six hand red flare distress signals and six hand orange smoke signals. A red hand flare distress signal may be substituted for any of the hand orange smoke distress signals. Two of vessel's orange smoke distress signals are expired. Vessel restricted to a Lakes/Bays/Sounds route. 46 CFR 180.68

Each survival craft must be secured to the vessel by a painter with a float-free link permanently attached to the vessel except that a float-free link is not required if the vessel operates only on waters not as deep as the length of the painter. Life float is secured to vessel using an HRU, HRU was expired. Provide new HRU for life float. Vessel restricted to a Lakes/Bays/Sounds route. 46CFR180.130(a)(1)

Machinery, equipment, materials, and arrangements not covered by standards in this subchapter may be inspected in accordance with standards acceptable to the cognizant OCMI as good marine practice. Three penetrations on forward bulkhead in engine room, starboard side, are not vapor tight. As a good marine practice, provide a proper seal to each penetration to prevent passage of vapors. 46 CFR 176.25-1(a)

Where flexibility is necessary a section of flexible metallic hose may be used. Non-metallic hose may be used provided it is especially adapted to resist the action of oil, acid and heat, and has a wall thickness sufficient to prevent collapsing or panting. The nonmetallic hose used for the generator’s wet exhaust is dry rotted and cracked, exposing rusting metal jacket. Replace hose for generator exhaust. 46 CFR 182.15-20 (c)

Pendants must be fitted approximately 18 inches apart around the outside of each buoyant apparatus. Each pendant must be at least 1/4in diameter, at least 12 feet long, secured in the middle, and have a breaking strength of at least 540lbs. Each pendant must be made up in a hank, and the hank secured by not more than two turns of light twine. The life float pendants are less than 12 feet long. Provide 12 feet long pendants for the life float. 46CFR160.010-4(g)

The number of persons capacity must be marked or painted in clearly legible letters and numbers on each life float. This number must be the number of persons the device is equipped for and not be greater than the number of persons the device is approved for as shown on its nameplate. The number or persons capacity markings was worn on one side of life float. Mark or paint in clearly legible letters and numbers the number of persons capacity on life float. 46CFR185.604(e)

Each buoyant apparatus must have Type II retro-reflective material meeting subpart 164.018 of this chapter on each side and end. The retro-reflective tape on life float no longer adhered to one side. Renew retro-reflective tape. 46CFR160.010-4 (p)

The employer shall provide an Employee Assistance Program for all crew members that includes both education and training on the effects and consequences of drug and alcohol use. Supervisory personnel must receive at least 60 minutes of training. There are no records indicating crew members received education and training on the effects and consequences of drug and alcohol use. Provide a roster of crew member and the details of the education and training on the effects and consequences of drug and alcohol use they received. 46 CFR 16.401(b)

A vessel must carry either a first aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. Vessel first aid kit aspirin was expired. Provide in-date aspirin for first aid kit. 46 CFR 184.710

An inspection for renewal of a Certificate of Inspection is conducted to determine if the vessel is in satisfactory condition, fit for the service intended, and complies with all applicable regulations. It normally includes inspection and testing of the structure, machinery, equipment, and on a sailing vessel, rigging and sails. The owner or operator must conduct all tests as required by the OCMI, and make the vessel available for all specific inspections and drills required by subpart H of this part. In addition, the OCMI may require the vessel to get underway. Prior to carrying passengers conduct man overboard and fire drills. 46 CFR 176.404(a)

A suitable hull strainer must be installed in the circulating raw water intake line of an engine cooling water system. Vessel was missing strainers on both engines. Install strainers on both engines. 46 CFR 182.420

When required by the marine inspector, a portion of the inspection must be conducted while the vessel is underway. Conduct sea trial prior to credit drydock. 46 CFR 176.802 (c)

At each initial and subsequent inspection inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of machinery, fuel, and piping systems. Vessel's port exhaust pipe was leaking due to fractured fiberglass section. Repair damaged section to the satisfaction of attending marine inspector.

A vessel must comply with the applicable requirements for any radio, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. Vessel could not produce current FCC station license during annual inspection. Provide valid FCC license.

When operating a vessel on a body of water where the discharge of treated or untreated sewage is prohibited by the Environmental Protection Agency under 40 CFR 140.3 or 140.4, the operator must secure each Type I or Type II device in a manner which prevents discharge of treated or untreated sewage. Install means to secure overboard discharge.

Each hatch cover must have securing devices. Forward deck hatch lacks securing devices. Install securing devices on hatch cover in accordance with 46 CFR 171.124.

Alternators on port and stbd engine are exposed presenting an electrical hazard. Properly cover or install boot to prevent potential shock or electrical hazard. 46 CFR 183.01-5

This part is intended to provide a minimum standard of regulations for electrical installations and equipment that will result in a vessel that is reasonably safe from electrical shock of personnel. Electrical wiring over port and stbd engine has several exposed and dead end wires, notably the stbd gear oil pressure sensor wire. Properly renew or cover wires to prevent potential shock or fire hazards. 46 CFR 185.01-5

The discharge ends of vent pipes shall be fitted with removable flame screens or flame arrestors. Mesh on stbd fuel vent flame screen is corroded and creates a gap between the pipe and the flame screen. Replace flame screen to eradicate gap. 46 CFR 182.20-35(d)

Each generator, motor, and other major items of power equipment shall be provided with a name plate indicating the maker’s name, its rating in volts and amperes or in volts and watts and when intended for connection to a normally grounded supply, the grounding polarity. Install name plate. 46 CFR 180.25-25(a)

(a) Each life jacket carried on a vessel on oceans, coastwise, or Great Lakes route, must have a life jacket light approved in accordance with § 161.012 in subchapter Q of this chapter, or other standard specified by the Commandant, including, but not limited to, approval series 160.112. Each life jacket light must be securely attached to the front shoulder area of the life jacket; Vessel is on a Coastwise route and lifejackets do not have lights. Add lifejacket lights. 46 CFR 180.75(a)

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an emergency. When repairs are made during an emergency, the owner, managing operator, or master shall notify the OCMI as soon as practicable after such repairs or alternations are made. Repairs or alterations that affect the safety of the vessel include, but are not limited to: replacement, repair, or refastening of deck or hull planking, plating, and structural members; repair of plate or frame cracks; damage repair or replacement, other than replacement in kind, of electrical wiring, fuel lines, tanks, boilers and other pressure vessels, and steering, propulsion and power supply systems; alterations affecting stability; and repair or alteration of lifesaving, fire detecting, or fire extinguishing equipment.
