The forward section of the starboard hull has popped tabbing in four areas, along with delamination on one gusset,located on the centerline. The forward section of the port hull has popped tabbing in seven areas. In the space aft of the helm, there is delamination on one gusset. Submit a written repair proposal to the CG marine inspector. Once the repair proposal is approved by the attending Marine Inspector, complete the repairs to the satisfaction of the attending CG marine inspector. 46 CFR 176.700(a)

When operating a vessel on a body of water where the discharge of untreated sewage is prohibited by the Environmental Protection Agency under 40 CFR 140.3, the operator must secure each Type III device in a manner which prevents discharge of sewage. Acceptable methods of securing the device include— (3) Using a non-releasable wire-tie to hold each valve leading to an overboard discharge in the closed position. Vessel has a Type III device on board in the starboard hull. The device does not have a method of securing. Install the above means to secure overboard discharge to the satisfaction of the attending CG marine inspector. 33 CFR 159.7(c)

The swim ladder, located on the port forward side of the vessel, has a wasted plate, securing the ladder to the vessel. Replace in-kind to the satisfaction of the attending CG marine inspector. Passengers prohibited from using swim ladder until satisfactory repairs are made. 46 CFR 176.700(a)

The deck railing on the starboard forward rail has a fractured weld on top deck, affecting the strength of the railing. Submit a written repair proposal to the CG marine inspector. Once the repair proposal is approved by the attending Marine Inspector, complete the repairs to the satisfaction of the attending CG marine inspector. Passengers prohibited from accessing starboard forward section until satisfactory repair has been made. 46 CFR 176.700(a)

On both starboard and port side hatches, leading into the vessel’s hull, wood rot is present on the structural wood surrounding the perimeter of the hatch. Submit a written repair proposal to the CG marine inspector. Once the repair proposal is approved by the attending marine inspector, complete the repairs to the satisfaction of the attending CG marine inspector. 46 CFR 176.700(a)

Acid batteries shall be located in a tray of lead or other suitable material resistant to deteriorating action by the electrolyte. The wood/FRP battery tray in the port hull is wasted. Replace in kind to the satisfaction of the attending CG marine inspector. 46 CFR 183.05-20(d) "Old T"

Fuses must be of the cartridge type only and be listed by Underwriters Laboratories or another independent laboratory recognized by the Commandant. The main fuse in the port hull is an ANL fuse. Replace with a UL cartridge fuse of the same amperage to the satisfaction of the attending CG marine inspector. 46 CFR 183.380(l)

Each small battery installation must be located in a well ventilated space and protected from falling objects. The battery installation, containing two small batteries, in the port hull, do not have covers. Obtain and install battery covers to the satisfaction of the attending CG marine inspector. 46 CFR 183.354(b)

All cable and wire must: be installed with metal supports spaced not more than 610 millimeters (24 inches) apart, and in such a manner as to avoid chafing and other damage. In port hull, by the battery installation, several sections of unsupported and sagging cable are present, running greater than 24 inches. Install suitable supports for cables, so that the supports are no greater than 24 inches apart, and ensure there is no sagging of the cables between supports, to the satisfaction of the attending CG marine inspector. 46 CFR 183.340(b)(4)

A space containing machinery powered by, or fuel tanks for, gasoline must have a ventilation system that complies with this section and consists of: (1)For an enclosed space: (i) At least two natural ventilation supply ducts located at one end of the space and that extend to the lowest part of the space or to the bilge on each side of the space;and (ii) A mechanical exhaust system consisting of at least two ventilation exhaust ducts located at the end of the space opposite from where the supply ducts located at the end of the space opposite from where the supply ducts are fitted, which extend to the lowest part of the bilge of the space on each side of the space, and which are led to one or more powered exhaust blowers. Vessel had 02 gasoline fuel tanks staged inside enclosed compartment with inadequate natural ventilation and no mechanical exhaust system installed. Vessel is prohibited from carrying passenger on board until: 1) Fuel tanks are permanently removed from enclosed space 2) Approved alterations to the compartment are completed, or 3) Approved installations of two natural ventilation supply ducts and a mechanical exhaust system are completed to the satisfaction of the attending Marine Inspector. 46 CRR 182.460(a)

Shutoff valves, installed so as to close against the fuel flow, must be fitted in the fuel supply lines, one at the tank connection and one at the engine end of the fuel line to stop fuel flow when servicing the accessories. Vessel did not have fuel shut off valves installed at the tank connection nor at the engine end of the fuel lines. Install two shut off valves, one at the tank connection and one at the engine end for each fuel tank and outboard engine. 46 CFR 182.455(b)(4)

It shall be the duty of the master of any vessel subject to the regulations in this subchapter to prepare and post emergency checkoff lists in a conspicuous place accessible to crew and passengers. Vessel did not have emergency checkoff list on board. Provide and post emergency checkoff list on board in a conspicuous place accessible to crew and passengers to the satisfaction of the attending Marine Inspector. 46 CFR 176.810

A visual and audible alarm must be provided at the operating station to indicate a high water level in normally unmanned spaces. Stbd bilge alarm is not operational. Repair or replace to the satisfaction of the attending marine inspector. 46 CFR 182.530(a)

Portable extinguishers must be inspected and maintained in accordance with NFPA 10. The required 10-B:C extinguisher was not charged. Place appropriate extinguisher to the satisfaction of the attending marine inspector. 46CFR176.25-25 Old T

Abandon ship and man overboard drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. Vessel unable to provide drill/training logs for the last two months. 46CFR185.25-10 Old T

Sail rigging and components are to be maintained in accordance with good marine practice. Observed main sail boom sheet was frayed and parting, and additionally the maintenance had not been logged IAW the maintenance plan. Repair sheet and document maintenance to the satisfaction of the attending marine inspector. 46CFR176.25-25 Old T

Rail courses must be installed so that no open space exists that is more than 12 inches. The upper course on the starboard bow and top rail just aft of forward is missing. Install railings to the satisfaction of the attending marine inspector. 46CFR177.35-1 Old T

Deck rails must be designed and constructed to withstand a point load of 200 pounds applied at any point in any direction. Two of the forward stanchion support structures are cracked at weld connections. Submit repair proposal to the satisfaction to the satisfaction of the attending marine inspector. 46CFR177.35-1 Old T

Portable and semi-portable extinguishers must be inspected and maintained in accordance with NFPA 10. The two 40-B:C extinguishers are not held by brackets. Install brackets to the satisfaction of the attending marine inspector. 46CFR176.25-25 Old T

Sail rigging and components are to be maintained in accordance with good marine practice. Aluminum sleeve inside foot of main mast was separating from mast interior. Provide report from third party surveyor attesting to serviceability of mast. 46 CFR 176.800 705 C

Except as otherwise allowed by this subpart, a vessel must comply with the structural design requirements of one of the standards listed below for the hull material of the vessel. Vessel has 2 areas of damaged fiberglass on the forward crossbeam, 2 areas of damaged fiberglass amidships on the keel of the stbd hull, and 3 areas of damaged fiberglass on the port hull. Take areas down to good glass and make available for re-inspection. 46 CFR 177.300 (c) 705 C

Except as otherwise allowed by this subpart, a vessel must comply with the structural design requirements of one of the standards listed below for the hull material of the vessel. Identified area of fiberglass in way of chain plate on stbd hull that is suspected of being delaminated from internal structure. Grind back and make available for inspection. 46 CFR 177.300 (c) 705 C

A vessel not required to have a power driven fire pump by 181.300 must have at least three 2 1/2 gallon buckets, with an attached lanyard satisfactory to the cognizant OCMI. The words "FIRE BUCKET" must be stenciled in a contrasting color on each bucket. Two of the vessel's fire buckets are missing lanyards. Attach lanyards and label fire buckets. 46 CFR 181.60

All vessels shall be provided with bilge pumps in accordance with Table 182.25-10(a). Vessel missing bilge pump in starboard hull. Install and demonstrate operation of bilge pump for starboard hull. 46 CFR 182.25-10 (Old T)

At each initial and subsequent inspection for certification, the marine inspector may require that an abandon ship or man overboard drill be held under simulated emergency conditions specified by the inspector. Man overboard drill not conducted. Conducted MOB in the presence of the attending MI. 46 CFR 176.808(g)

At each initial and subsequent inspection for certification, the marine inspector may require that a fire drill be held under simulated emergency conditions to be specified by the inspector. Conducted fire drill in the presence of the attending MI. 46 CFR 176.810(d)

Each life jacket must be marked with Type I retroreflective material approved in 46 CFR 164.018. Thirteen life jackets were not marked with adequate retroreflective material. Provide appropriate markings to the satisfaction of the attending marine inspector. Passenger count reduced to 13. 46 CFR 185.604(h)

The cognizant OCMI may require that a vessel and its equipment undergo any additional test or inspection deemed reasonable and necessary to determine that the vessel and its equipment are suitable for the service in which they are to be employed. Jib boom forward end wasted at saddle fastening attachment point where the goose neck connects to the headstay bridle. Provide for cause third party rigging survey to rectify condition to the attending [name withheld] sail use not permitted until attending MI reviews survey. 46 CFR 176.840

At least one ring life buoy must be fitted with a lifeline. If more than one ring life buoy is carried, at least one must not have a lifeline attached. Each lifeline on a ring life buoy must be of at least 60 feet in length. Ring life buoy lifeline 30 feet. Provide required 60 feet of line that meets the requirements of 46 CFR 180.70(c)(1 and 3 through 6). 46 CFR 180.70(c)(2)

Each ring life buoy on a vessel must be stowed in a way that it can be rapidly cast loose. Ring life buoy attached with several turns of line and clipped to starboard aft railing that enable it to be rapidly cast loose. Provide proper ring life buoy holding bracket. 46 CFR 180.70(b)(3)

Each ring life buoy must be marked in clearly legible block capital letters with the vessel's name. Vessel name not clearly legible. Remark vessel name on ring life buoy. 46 CFR 185.604(b)

Each child size life jacket must be stowed in a location that is appropriately marked and separated from adult life jackets so the child size life jackets are not mistaken for adult life jackets. Child size life jackets location not marked or seperated from adult life jackets. Mark child size life jackets location and separate from the adult life jackets. 46 CFR 180.78(a)(5)

The number and identification of the items stowed inside, and their sizes, must be marked in clearly legible letters and numbers on each container for life jackets. Life jackets stowage container not marked with number and sizes for adult life jackets. Mark stowage container for life jackets with "ADULT" and number stowed inside. 46 CFR 185.604

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date: Charts of large enough scale to make safe navigation possible; U.S. Coast Pilot or similar publication; Coast Guard Light List; and Tide tables. Vessel missing all required publications and chart. Provide to the satisfaction of the attending MI. 46 CFR 184.420(a)

Fuses must be of the cartridge type only and be listed by Underwriters Laboratories or another independent laboratory recognized by the Commandant. Fuses are blade automotive blade type fuses. Replace with the correct amperage cartridge type fuses to the satisfaction of the attending MI. 46 CFR 183.380(l)

All equipment, including switches, fuses, lamp holders, etc., must be suitable for the voltage and current utilized. During the spot inspection of fuses the attending MI discovered the ratings for fuses were not suitable for the service intended. Replace fuses with correct ampere rating. 46 CFR 183.220(b)

Drug testing programs required by this part must be conducted in accordance with 49 CFR part 40, Procedures for Transportation Workplace Testing Programs. Marine employer is DER and enrolled in the companies random testing program. A person subject to testing cannot conduct the random selection and still meet the definition of “unannounced” as it pertains to random tests. Resolve by: finding a disinterested third party to conduct the random selection process or authorize an employee be the DER and provide the name and telephone number to the collector and consortium to the satisfaction of the attending MI. 46 CFR 16.113(a)

Each vessel must be equipment with at least two operable portable battery lights. One of these lights must be located at the operating station and the other at the access to the propulsion machinery. Vessel does not have portable battery lights. Provide two operable portable battery lights for the vessel. 46 CFR 183.430

Placards containing instructions for the donning and use of the life jackets aboard the vessel must be posted in conspicuous places that are regularly accessible and visible to the crew and passengers. Placard was removed to paint vessel. Post life jacket donning placard. 46 CFR 185.516 (a)

The master of a ship shall ensure that one or more placards meeting the requirements of this section are displayed in prominent locations. No garbage placard found on vessel. Post placard meeting requirements of 33 CFR 151.59.

Connections must be made to battery terminals with permanent type connectors. More than four wires are connected to positive battery terminal and not of permanent type. 46 CFR 183.350(d)

An adult life jacket must be provided for each person carried on board a vessel. Vessel is restricted to 27 persons total due to lack of properly outfitted lifejackets. Replace (01) lifejacket.

Each portable fire extinguisher on a vessel must be of an approved type. Portable extinguisher at the operating station was accidentally discharged. Replace with approved fire extinguisher.

Vessels operated solely within 20 nautical miles of land must be equipped with a VHF-DSC radiotelephone installation meeting the requirements of § 80.1101(c)(2). VHF radio missing MMSI and LAT/LONG for DSC operation.

Portable fire extinguishers must be located so that they are clearly visible and readily accessible from the space being protected. Provide proper labeling of fire extinguisher at operating station.

Each vessel must be equipped with at least two operable portable battery lights. One of these lights must be located at the operating station and the other at the access to the propulsion machinery space.

A visual and audible alarm must be provided at the operating station to indicate a high water level in each of the following normally unmanned spaces. Middle compartment bilge alarm light is not operational.

A submersible electric bilge pump may be used as a power bilge pump if a positive shutoff valve is installed at the hull penetration. Positive action valve and associated hose clamps are missing on port and stbd bilge pipe discharge hoses. Install to the satisfaction of attending marine inspector.

When operating a vessel on a body of water where the discharge of untreated sewage is prohibited, the operator must secure each Type III device in a manner which prevents discharge of sewage. Vessel has type III MSD system and system was not secured to prevent discharge.

No marine employer shall engage or employ an individual to serve as a crewmember unless the individual passes a chemical test for dangerous drugs for that employer. One employed deckhand was covered under another operator’s consortium, and not under the vessel’s drug testing program. Add the crew member to the vessel’s drug testing program to ensure pre-employment testing and proper random testing.

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing machinery failure. The fuel supply line and electrical wiring harness for the port outboard engine are submerged and dragging below the waterline when the vessel is underway. Properly secure the fuel line and wiring harness above the waterline to prevent failure.

Each life jacket and ring buoy must be marked in clearly legible block capital letters with the vessel's name. Ring Buoy did not have vessel name. Provide proper marking and identification on ring buoy.

Your vessel must undergo an annual inspection within the 3 months before or after each anniversary date. The vessel has not completed an annual inspection within 3 months after the Certificate of Inspection issuance date of 18 October. The vessel is prohibited from carrying passengers for hire until satisfactory completion of an Annual Inspection. 46CFR176.500(a)(1)

At least one ring life buoy must be fitted with a lifeline that is buoyant, at least 60 feet in length, is non kinking, has a diameter of at least 5/16 inch, have a breaking strength of at least 1,124 pounds, be of a dark color if synthetic or of a type certified to be resistant to deterioration from ultraviolet light. Vessel's ring life buoy lifeline is unserviceable. Provide proper lifeline. 46 CFR 180.70

A vessel must carry one floating waterlight that has a lanyard of at least 3 feet in length but not more than 6 feet securing the waterlight around the body of the ring life buoy and must be attached tot he lanyard with a corrosion-resistant clip with a strength of at least 50 pounds to allow the waterlight to be quickly disconnected from the ring life buoy. Waterlight and battery is water-logged, inoperable, and in unserviceable condition. Provide proper installation and configuration of waterlight. Vessel limited to daytime operations. 46 CFR 180.70

A vessel must be afloat for at least a portion of the inspection as required by the marine inspector. Vessel is currently replacing its sail rigging. Prove proper operation of vessel underway. 46 CFR 176.802

A vessel on a lakes bays and sounds route must carry three hand red flare distress signals and three hand orange smoke distress signals. A hand red flare distress signal may be substituted for any of the hand orange smoke distress signals. Vessel's distress signals were expired. Provide the proper distress signals onboard vessel. Vessel is limited to short runs less than 30 minutes away from the dock. 46 CFR 180.68 (b)

Stations in the maritime service must be licensed by the FCC either individually or by fleet. Vessel's FCC Radio Station Authorization is expired. Provide updated FCC Station License. 47 CFR 80.13

On a vessel of at least 26 feet in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in each normally unmanned space. Vessel has two high water alarms but only one indicator at the operating station. Alarm indicator light is not working. Install proper alarm indicators at the operating station. 46 CFR 182.530

A vessel of more than 26 feet in length, but not more than 65 feet, must carry a minimum of one life buoy of not less than 24 inches in diameter; vessels ring life buoy is wasted. Replace ring life buoy.

A vessel must carry one floating waterlight; vessel's waterlight is inoperable. Replace waterlight.

A vessel not required to have a power driven fire pump must have at least three 2 1/2 gallon buckets, with an attached lanyard satisfactory to the cognizant OCMI, placed so as to be easily available during an emergency. The words "Fire Bucket" must be stenciled in a contrasting color on each bucket; Vessel has two fire buckets instead of three. Have three 2 1/2 gallon buckets stenciled with the words "Fire Bucket" placed so as to be easily available during an emergency on board the vessel.

A vessel is inspected for compliance with the standards required by this subchapter. Machinery, equipment, materials, and arrangements not covered by standards in this subchapter may be inspected in accordance with the standards acceptable to the cognizant OCMI as good marine practice; Vessels float switch to indicate high levels in the bilge were not secured; Secure float switch to deck.

A vessel must be provided with a satisfactory arrangement for draining any watertight compartment, under all practicable conditions; Vessel's starboard bilge pump was not properly arranged for draining compartment. Properly arrange bilge pump.

A durable placard must be posted next to all radiotelephone installations with the emergency broadcast instruction and information, specific to the individual vessel; Vessel is missing placard. Post placard on board vessel

The PMP should include an inventory of all rigging components, including the age of component, size, type of material and a replacement schedule for all rig components based on inspection results, service history, and manufacturer or designer recommendations; Vessel's PMP is missing inventory of rigging components. Provide PMP with contents including inventory of all rigging components in accordance with NVIC 02-16.

The design, materials, and construction of masts, post, yards, booms, bowsprits, and standing rigging on a sailing vessel must be suitable for the intended service; Vessel's sails are torn. Repair or replace sails so vessel will be suitable for the intended service.

Complete emergency drills upon completion of drydock inspection in the presence of a Coast Guard Marine Inspector
