Rails or equivalent protection must be installed near the periphery of all decks of a vessel accessible to passengers or crew. Equivalent protection may include lifelines, wire rope, chains, and bulwarks, which provide strength and support equivalent to fixed rails. Lifelines were found loosened during inspection. Tighten all lifelines to satisfaction of MI.

Each ship transporting more than six passengers for hire equipped with a radiotelephone station must carry a radio operator who holds a marine radio operator permit. FCC Marine Radio Operator Permit for the Captain was not available during inspection. Provide a copy. 47 CFR 80.159(e)

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date charts of large enough scale to make safe navigation possible. Navigation chart was not up-to-date. Provide updated chart for intended voyage.

The owner or managing operator must provide satisfactory evidence of the required servicing (for fixed-gas fire extinguishing systems) to the marine inspector. Operator was unable to provide annual service certificate for fixed-gas fire extinguishing system. Provide a copy to the MI. 46 CFR 176.810(b)(1)(iv)

Electrical equipment on a vessel must be installed and maintained to minimize accidental personnel contact with energized parts. Live dead-end wiring was found in battery compartment. Secure to satisfaction of MI.

The owner shall, prior to receiving a Certificate of Inspection, submit for approval to the cognizant OCMI, at least two copies of the following plans, manuals, analyses, and calculations that are applicable to the vessel as determined by the OCMI: On sailing vessels:(i) Masts, including integration into the ship's structure; and(ii) Rigging plan showing sail areas and centers of effort as well as the arrangement, dimensions, and connections of the standing rigging. Sail Rigging plan was not available during inspection. Provide to MI. 46 CFR 177.202(b)(12)(ii)

No marine employer shall engage or employ any individual to serve as a crewmember unless the individual passes a chemical test for dangerous drugs for that employee. Vessel was unable to provide evidence of chemical testing for required number of crew members on Certificate of Inspection for Limited Coastwise route. 46 CFR 16.201(a)

Vessels operated solely within 20 nautical miles of land must be equipped with a VHF-DSC radiotelephone installation meeting the requirements of § 80.1101(c)(2). MI observed vessels VHF-DSC radio was not transmitting vessel position. 46 CFR 184.502 47 CFR 80.905(a)(1)

Every vessel subject to Part III of Title III of the Communications Act must have a detailed inspection of the radio installation by an FCC-licensed technician. MI observed that VHF radio on the vessel was not broadcasting all required information. 47 CFR 80.903

Each floating waterlight installed after March 11, 1997, on a vessel carrying only one ring buoy, must be attached to the lanyard with a corrosion-resistant clip. The clip must have a strength of at least 22.7 kilograms (50 pounds), and allow the waterlight to be quickly disconnected from the ring life buoy. MI observed waterlight on ring life buoy is attached to grab line with a splice and no corrosion resistant clip is installed. 46 CFR 180.70(d)(3)

...Each life jacket must be approved in accordance with either § 160.002, § 160.005, or § 160.055 in subchapter Q of this chapter. MI observed deteriorated retroreflective material on life jackets. Additionally several snaps on lifejackets are corroded or deteriorating and need servicing/cleaning. 46 CFR 180.71(c)

Each buoyant apparatus must be fitted with...a light. MI observed light installed on the buoyant apparatus with drained batteries, required lithium battery replacements were unavailable. 46 CFR 180.175(e)

A vessel must undergo an annual inspection within the 3 months before or after each anniversary date. Vessel has not completed annual inspection.

All engines must have at least two means for stopping the engine(s) under any operating conditions. The vessel was unable to operate the engine from the operating station.

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of the hull structure. Vessel had multiple holes due to missing handles, removed equipment, etc. Repair holes, making vessel watertight, to the satisfaction of the attending marine inspector. 46 CFR 176.802(a)

Except as otherwise provided in this section, machinery and fuel tank spaces shall be separated from accommodation spaces by watertight and/or vapor tight bulkheads of double diagonal wood, marine plywood, steel plate, or equivalent construction. Where segregation can be obtained by means of a watertight and/or vapor tight engine box, this will be accepted as a substitute for the required bulkheads. Engine area had large hole where hoses leave space. Make engine area vapor tight to the satisfaction of the marine inspector. 46 CFR 182.35-1 "OLD T"

A marine employer whose vessel is involved in a casualty or incident that is, or is likely to become, a serious marine incident as defined in § 4.03–2 of subchapter A of this chapter shall comply with the requirements of § 4.06 in subchapter A of this chapter. No alcohol testing devices were on board. Obtain alcohol testing devices. 46 CFR 185.212

The cognizant OCMI may require that a vessel and its equipment undergo any additional test or inspection deemed reasonable and necessary to determine that the vessel and its equipment are suitable for the service in which they are to be employed. Hydraulic leak found starboard of engine compartment. Determine cause of leak and repair to the satisfaction of the marine inspector. 46 CFR 176.840

It shall be the duty of the master of any vessel to assure that all exposed hatches are properly secured before getting underway for a voyage on other than protected waters. The hatch aft of the helm does not have a device to properly secure it. Provide way to secure hatch to the satisfaction of the marine inspector. 46 CFR 185.20-15 "OLD T"

A vessel that is exposed to salt water more than three months in any 12 month period since the last examination must undergo a drydock examination and an internal structural at least once every two years. Vessel has not completed worklist items by drydock due date to receive credit for drydock. Complete worklist items to the satisfaction of the attending Marine Inspector.

For fixed-gas fire extinguishing systems, the inspections and tests required by Table 176.810(b) of this section, in addition to the tests required by 46 CFR 147.60 and 147.65. The owner or managing operator must provide satisfactory evidence of the required servicing to the marine inspector. Fire suppression system has outdated annual service. Have system inspected on the vessel and provide the Marine Inspector a copy of the report.

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. Vessel has had an engine casualty and is unable to get underway. Make repairs to the satisfaction of the attending Marine Inspector prior to the carriage of passengers.

A vessel must be operated IAW applicable laws and regulations and in such manner as to afford adequate precaution against hazards that might endanger the vessel and persons being transported. Fire extinguishers and installed fire suppression are beyond service dates. Buoyant apparatus does not have required equipment (Light).

A vessel must be operated in accordance with applicable laws and regulations and in such a manner as to afford adequate precaution against hazards that might endanger the vessel and the persons being transported. MI discovered numerous deficiencies with sail rigging. Seize all shackles. Secure all turnbuckles with cotter pins or similar to prevent inadvertent turning. Secure bolts that have back out in gooseneck fitting. Provide details of findings from monthly rigging inspections in PMP. Update PMP to reflect date of last and next mast unstepping (10 Year cycle).

The weather deck of a flush deck vessel must be watertight. Boot for baby-stay as it enters the deck has failed and needs replacement.

The items in first-aid kit shall be as listed in Table 160.054-4(b). Aspirin in first-aid kit has expired.

For fixed-gas fire extinguishing systems, the inspections required by Table 176.810(b) of this section. The owner or managing operator must provide satisfactory evidence of the required servicing to the marine inspector. If any of the equipment or records have not been properly maintained, a qualified servicing facility may be required to perform required inspections. Fixed fire suppression system has lapsed on annual service. Have system inspected on vessel and provide technician report.

At each initial and subsequent inspection for certification, the owner or managing operator must have the vessel ready for inspection of its fire protection equipment including inspection of each fire extinguisher. All portable fire extinguisher are due for annual servicing. Have inspected and provide servicing report to MI.

Each buoyant apparatus shall be equipped with a water light. Water light was missing at time of inspection. 46 CFR 180.15-1(b)

The intent of this part is to insure that all vessels be operated in accordance with applicable laws and regulations and in such a manner as to afford adequate precaution against hazards of an emergency naturewhich might endanger the vessel and the persons being transpored. Vessel's securing tag line to the back up jib halyard was showing signs of wear and fraying. 46 CFR 185.01-5

The inspection and maintenance of the equipment listed in paragraph (a) of this section shall be logged or otherwise documented for review by the Coast Guard upon request. Vessel's lifesaving maintenance logs lacked entries and details of actual maintenance performed. 46 CFR 176.810(b)(ii)

Monthly inspection required by NFPA 10 may be conducted by the owner, operator, person-in-charge, or a designated member of the crew. Vessel's firefighting maintenance records lacked entries and details of actual maintenance performed. 46 CFR 185.01-5

Unless normal use of the radiotelephone installation demonstrates that the equipment is in proper operating condition, a test communication on a required frequency in the 1605 to 27500kHz band or the 156 to 162MHz band must be made by a qualified operator each day the vessel is navigated. If the equipment is not in proper operating condition, the master must be proptly notified. Vessel's VHF radio transmitted but was unable to receive. Operator identified speaker as the issue and replaced on the spot. Satisfactory test of the VHF radio was completed.

A vessel to which this subchapter applies may not be operated without having on board a valid U.S. Coast Guard Certificate of Inspection (COI). COI has expired.

Annual inspection. Your vessel must undergo an annual inspection within the 3 months before or after each anniversary date. Vessel has failed to schedule an annual inspection and is not in compliance with the annual inspection timeline.

At each initial and subsequent inspection for certification, the owner must be prepared to conduct tests and have the vessel ready for the following: inspection of each portable fire extinguisher. 01 portable extinguisher shown to be in the red gauge and no longer usable, requires recharging. Replace or service fire extinguisher before getting underway.

For fixed-gas fire extinguishing systems, the inspections and tests required by Table 176.810(b) of this section, in addition to the test required by 46 CFR 147.60 and 147.65. The owner/managing operator must provide satisfactory evidence of the required servicing to the marine inspector. Fixed fire fighting lacked current inspection tag/documentation. Provide CG Marine Inspector with current inspection documents for fixed fire fighting before taking passengers.

Abandon ship, man overboard, and firefighting drills and training shall be logged or otherwise documented for review by the CG upon request, with the date of the drill and description. Vessel missing current logs for all drills; begin documenting monthly drills IAW CFR requirements.

Employers must maintain records of chemical tests and must make these records available to CG upon request. The records shall ID the total number of individuals chemically tested annually for dangerous drugs in each of the categories of testing required, including the annual number of individuals failing chemical tests and the number and types of drugs for which individuals tested positive. Vessel lacking DAPI documentation for all crew members; provided updated DAPI for all crew members on board to CG inspector. 46 CFR 16.260 (a)

A space containing diesel machinery must be fitted with two ducts to furnish natural or powered supply and exhaust ventilations. Engine room space lacking proper ventilations; install proper ventilations for engine space IAW CFR.

Adequate emergency lighting automatically actuated upon failure of the main lighting system shall be fitted along the line of escape in vessels having lounge areas below the main deck as per subpart 177.30-7 of this subchapter. Vessels not equipped with a single source emergency lighting system shall have individual storage battery powered automatically operated lights in strategic locations. These lights shall have an automatic battery charger, shall not be readily portable, and shall have sufficient capacity for 6 hours continuous operation. Vessel lacked any emergency lighting for egress and had only handheld flashlights throughout the vessel. Install required emergency lighting for safe egress. Updated due date to additional days from annual inspection.

All electrical installations shall be to the satisfaction of the Officer in Charge, Marine Inspection. Unidentified wire on engine not properly terminated. 46 CFR 183.01-15

The number and identification of the items stowed inside must be marked in clearly legible letters and numbers on each container for life jackets. Label port and starboard stowage locations for life jackets.

On a vessel of at least 26 ft, a visual and audible alarm must be provided at the operating station. The bilge high level alarm was not labeled; add label for clarity.

Portable fire extinguishers shall be installed and located to the satisfaction of the Officer in Charge, Marine Inspection. Label location of fire extinguishers since they are located under the table in the main cabin. 46 CFR 181.30-10(b)
