Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI. Vessel is undergoing repairs following a partial sinking at the dock. All spaces to be internally inspected and all machinery to be tested and an annual inspection must be completed to the satisfaction of the attending CG marine inspector prior to the carriage of passengers.

Each floating waterlight must be approved in accordance with § 161.010 in subchapter Q of this chapter, or other standard specified by the Commandant, including, but not limited to, approval series 160.110 Batteries installed were expired and shall be replaced.

For the purpose of flexibility only, flexible hose installed in lengths of not more than 760 millimeters (30 inches) and subject to pressures of not more than 35 kPa (5 psig), may meet the following requirements, Flexible hose designed for use with hose clamps may be installed with two clamps, at both ends of the hose. All main engine R/W intake hoses only had one clamp on each side of intake hose.

Nonmetallic flexible hose Nonmetallic flexible hose must be in accordance with SAE J1942 (incorporated by reference; see 46 CFR 56.01-2) and may be installed only in vital and nonvital fresh and salt water systems, nonvital pneumatic systems, lube oil and fuel systems, and fluid power systems. Nonmetallic flexible hose must have factory-assembled end fittings requiring no further adjustment or field attachable fittings. Hose end fittings must comply with SAE J1475. flex hoses in pressure side of electric fire pump do not meet subchapter F requirements for hoses for factory assembled end fittings and are not exempted by the less than five PSI rule in 46 CFR 182.720 (e) (3) (v). Two flexible non-metallic hoses for the winch/capstan penetrate the watertight main deck. Each penetration must be accomplished using an acceptable metallic through deck fitting that is welded or otherwise attached to the deck by an accepted method; and one metallic shutoff valve must be installed if operable from above the bulkhead deck.

Vessels that have overnight accommodations for passengers must submit plans to the cognizant OCMI by 28Mar2022 for the installation and use of a monitoring device to ensure the wakefulness of the watchmen. Submit detailed plans for your proposal to include general arrangement/locations and bill of materials for the installation to include cable sizes/length for plan approval and install the system prior to 30Sep2022.

Provide data plate information and make available for testing/inspection the auxiliary boiler in the pilothouse to the satisfaction of the attending USCG Marine Inspector prior to placing into service.

Vessels must submit plans to the cognizant OCMI for the installation and use of monitoring device to ensure wakefulness of the watchmen required in 46 CFR 185.410(a). The monitoring device must: (1) Ensure the wakefulness of the crew in the event that watchman is unresponsive. (2) Remains operable during nighttime watch. (3) Be arranged to ensure proper coverage of the passenger accommodation spaces, common areas, and spaces with potential fire hazards. Submit plans to Sector SENE for watchstanding monitoring device; plans to include, but not limited to: device spec sheet, how device is to be installed/powered, how & when device is to be implemented, device locations, testing procedures & any other maintenance requirements for the device.

Wire on top of pilothouse is exposed from protective sheathing. Properly replace or remove wiring/cable to the satisfaction of marine inspector. 46 CFR 183.01-15 (OLD T)

The galley lower electrical panel has no blanks and is an exposed electrical hazard.

There is no CO2 sign at the fixed fire system pull station.

Secure stern light and fix wiring.

Repaint navigation light screens matte black.

Repair or replace port and starboard running navigation lights.

One of the vessel's ring buoys was worn and in need of replacement. Found a different ring buoy, restenciled.

Vessl reported a total loss of steering after a "t" connection in the hydraulic line bursted. Per 46 CFR 182.610(a)(1), a vessel must be provided with a main steering gear that is of adequate stregnth and capable of steering the vessel at all service speeds. Vessel is directed to effect repairs and prove satisfactory operation to a CG MI, prior to embarkation of passengers.

Update emergency instructions/station bill to include designated crew member as a watch patrolman.

Update log book to organize weekly, monthly, quarterly, and annual inspections.

Provide a means to log watch patrolman rounds when watch patrolman is required to be conducting rounds.

Create a voyage plan to be included with passenger count/lists when making overnight voyages.

Provide documentation/check list to include records of instruction of each crew member of their duties, responsiblities, and ability/knowledge/familiarization of the location and to utilize all emergency equipment as per 46 CFR 185.420.

Update passenger safety orientation to include the request of passengers to don a life jacket when on a voyage of more than 24 hrs as per 46 CFR 185.506(e).

Provide documentation that smoke detectors meet UL 217 standard as per 46 CFR 181.450.

VHF radios we're not able to receive GPS information with Lat and Long. Prove proper operation of VHF radios.

Vessel's name, retro tape, and number of persons were worn off life floats and paddles. Remark accordingly.

Properly stow painter line for the lifefloat located on the pilot house.

Properly mark life jacket stowage locations to include type and number of life jackets.

Make approved wood repairs to port quarter where main deck meets the side shell where deck beams and ceiling plywood were rotted. Prior to credit DD.

Vsl is broadcasting: No static draft information, no dimension information (AIS Antenna Position ABCD values equals 0), and not broadcasting an AIS Ship Type.

Plug lighting fixture over STBD MDE into a grounded outlet.

Properly mount lighting fixtures in Engine room over STBD MDE and Port generator.

Renew exhaust lagging on center MDE.

Remove terminated leads on battery charger.

Install additional hose clamp on exhaust flex hose port side of Engine room.

Install handle for ball valve on manifold for tank room bilge suction.

Clean growth in fuel system treat as needed.

Fix broken handrail port bow.
