Radio stations in the maritime service must be licensed by the FCC. One ship station license will be granted for operation of all maritime services transmitting equipment on board a vessel. The vessel radio station license expired. Provide evidence of valid radio station license for MARY M IV. CITE: 47 CFR 80.13

Pendants must be fitted around the outside of each buoyant apparatus. Each pendant must be made up in a hank, and the hank secured by not more than two turns of light twine or similarly easy to break means of securing such as a rubber band. Hanks are made up w/ wire that exceeds breaking strength. Replace w/breakable securing means. CITE: 46 CFR 180.200

Each ring life buoy on a vessel must: Be stowed in a way that it can be rapidly cast loose. Operational readiness of life rings impacted by wire ties around lifeline and disconnected water light. 46 CFR 180.70(b)(3)

A vessel must carry adequate and up-to-date: Charts of large enough scale to make safe navigation possible; U.S. Coast Pilot or similar publication; Coast Guard Light List; Tide tables; and Current tables. No updated charts, light list, or coast pilot on board. 46 CFR 184.20

Each launching appliance and each survival craft and rescue boat on a vessel must be in good working order and ready for immediate use. Life floats had areas in need of repair, including retro reflective material. 46 CFR 185.700(a)

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. Medical kit had expired medicine. Provide medical kit in accordance with ISO 18813:2006(E) paragraph 4.12. 46 CFR 184.710

Each hatch exposed to the weather must be watertight. Inspector observed worn gasket on forward compartment. 46 CFR 171.124(a)

When operating on a body of water where the discharge of untreated sewage is prohibited, the operator must secure each Type III device in a manner which prevents discharge of sewage. Acceptable methods include: Closing and removing the handle; Padlocking; Using a non-releasable wire-tie. MSD valve was not properly secured. 33 CFR 159.7(c)

Suitable covers, guards, or rails shall be installed in way of all exposed and hazardous places such as gears, machinery, etc. Steering gear at hard starboard extended and made contact with port muffler causing damage. 46 CFR 177.35-15 "Old T"

Batteries terminals shall be of the soldered type and all connections shall be made to the battery terminals with permanent type connectors. Spring clips or other temporary type clamps are not permitted. Wing nuts were found on battery terminals. Provide permanent type connectors. 46 CFR 183.05-20(c) "Old T"

Examine all weathertight closures above weather deck. 03 windows on the main deck are damaged. Replace all windows with cracks. 46 CFR 176.25-5(5) "Old T"

No vessel subject to inspection under 46 U.S.C. 3301 will be navigated unless it is under the direction and control of an individual who holds an appropriate license or officer endorsement on his or her MMC. Master of the vessel was unable to provide original MMC license on board during inspection. 46 CFR 15.515

The Coast Guard will issue a medical certificate to a mariner meeting the medical and physical standards for merchant mariners. Master of the vessel was unable to provide a valid medical certificate during inspection. 46 CFR 10.301

Each ship transporting more than six passengers for hire equipped with a radiotelephone station in accordance with Part III of Title III of the Communications Act must carry a radio operator who meets the following requirements. Master of the vessel was unable to provide a valid FCC radio operator permit during inspection. 47 CFR 80.159 (e)

A separate circuit, fused at the main distribution panel shall be provided for each radio-phone installation. Both installed radios where on the same circuit breaker. Install so radios are on different circuit breakers for overload protection. 46 CFR 183.05-25 Old "T"

Suitable covers, guards, or rails shall be installed in way of all exposed and hazardous places such as gears, machinery, etc. Port and starboard main engines has missing guards over rotating belts on forward assemblies. Install adequate safety guards for both engines. 46 CFR 177.35-15 Old "T"

Penetrations of required watertight decks at bulkheads by any rigid plastic pipe systems are prohibited, except when each penetration is accomplished using an acceptable metallic fitting, welded or otherwise attached to the bulkhead AND an acceptable metallic shut-off valve(s) is installed. Option 1: 1 shutoff valve installed in space that can be operable from the above bulkhead deck. Option 2: 2 shutoff valves installed on either side of the bulkhead. Sewage system PVC piping was installed through bulkhead without metallic fitting and without shutoff valve(s). Piping goes from fuel tank space to engine room compartment. Install properly with metallic fitting through bulkhead and appropriate option of shutoff valve(s) installation. 46 CFR 182.40-1 Old "T"

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. Vessel was observed with an exhaust leak, that appeared to be coming from the STBD engine's turbo. This exhaust leak caused the smoke detector to go off. Vessel is require to identify and correct the engine room exhaust leak prior to carrying passenger, to the satisfaction of the attending marine inspector. (46 CFR 182.05-1(a) Old-T)

02 fuel hose clamps were broken on generator at fuel pump and fuel filter. Replace broken hose clamps to the satisfaction of the attending Marine Inspector.

Life float daisy chains are too short. Daisy chains need to be lengthened to allow for each life float to be deployed without coming in contact with each other. To the satisfaction of the attending Marine Inspector

Exhaust leak found on Starboard M/E at the turbo. Repair exhaust leak to the satisfaction of the attending Marine Inspector.

Missing donning procedures placard for the child life jackets that are carried onboard. Require vessel to post donning procedures placard for all life jackets that are carried onboard to the satisfaction of the attending Marine Inspector.

Ships generator did not secure during test of fireboy fire suppression system activation. Generator is not vital system for ships operation, require generator to be locked out/tagged out until fire suppression system is serviced and then complete a satisfactory test of the system to the satisfaction of a attending Marine Inspector.

Vessel’s sewage overboard discharge valve does not have a proper locking device. Install satisfactory locking device to valve, to the satisfaction of the attending Marine Inspector.

Vessel does not have current charts for its operating area. Provide proper updated charts for the operating area of the vessel to the satisfaction of the attending Marine Inspector.

Inflatable buoyant apparatus not onboard at time of inspection.

Mechanisms for securing engine room ventilation closures from outside the space were found to be ineffective.

Fuel shutoff valve hatch for generator corroded and not easily accessible.

Port and Starboard fuel tank filling pipes are missing bonding wire.

Sewage overboard hose broken at thru-hull valve.

Generator/shore power interlock switch at operating station is loose.

Electrical outlet on the aft starboard bulkhead in the wheelhouse is not connected to power source.

The vessel was found to be operating with passengers on board without a properly credentialed master as required on the COI and for all Inspected Small Passenger Vessels. Prior to operating with passengers provide proof of a valid master endorsed MMC for under 100GT self propelled vessels. 46 CFR 15.805 (a) (4)

The vessel was carrying outdated paper charts and does not have the proper digital charts onboard. Updates to the existing chart shall be made or a new updated chart put onboard. 46 CFR 184.402

The vessel is required to have 12 flares. The vessel had 11 non-expired flares onboard. The vessel is limited to a Lakes Bays and Sounds inland route until an additional flare is put on the vessel. 46 CFR 180.68

The vessel had 04 non operational water lights on lifejackets, 03 adult and 01 child. The vessel is restricted to carrying no more than 67 total people and no more than 07 people under 90lbs, as the vessel had 08 child jackets onboard but is only required 07. The jackets shall remain removed until new lights are fitted. 46 CFR 180.71

The vessel fire extinguishers have not been completed the required annual servicing. The vessel is not permitted to operate with passengers until proof is submitted, received, and confirmed to have been reviewed by a CG inspector. 46 CFR 181.500

Grind down two areas identified on the stem to good glass. Once ground down, make areas available for USCG inspector. Once repair method is approved by USCG, make proper repairs to the area.

Make fasteners attaching the spray rail water tight.

Replace the wire connection to the newly installed LED light in the frw storage area where the connector has exposed metal.

re-connect or renew the bonding strip within the engine room connecting the radio to the stbd engine foundation.

Replace the cracked jacket water head tank on the stbd engine. The current tank has a crack along the outboard side.

Install engine guards around belts within the engine room to mitigate pinch hazards.

Install a strainer at the bilge pick-up within the tank space.

Renew/replace steering gear hardware as discussed. make area available once hardware and surrounding mounting location is removed. Make proper repairs and prove steering gear functionality.

Remove dead-end wiring and prove operation of the steering gear high bilge alarm.

Make repairs or replace the self holding the engine room battery bank.

A life float is required primary lifesaving as stated on the vessel's COI. The life float requires repair. Provide repair proposal in accordance with OEM. - 46 CFR 180.200(a)(2)

The vessel is required navigation lights. The MI found the vessel's navigation lights were inoperable. - 46 CFR 183.420

The life buoy and life float are required to be equipped with floating water lights. The MI found the water light batteries for the life float and life buoy were expired. Limited to daytime operations. - 46 CFR 180.70(d)(1) 46 CFR 180.175(d)

The vessel must have a sufficient number of alcohol testing devices onboard in the event of a Serious Marine Incident. The MI found the alcohol testing strips were expired. - 46 CFR 4.06-15(a)(1)

All life jackets are required to be marked with Type I retroreflective material. Two child sized life jackets were found with deteriorated retroreflective material. Limited to carrying 6 children. - 46 CFR 185.604(h)

The machinery space is required to be equipped with a fixed fire extinguishing system. The vessel has a pre-engineered sytem. The MI found a hose obstructing actuation of the system. - 46 CFR 181.420(a)

The vessel has water ingress into the tank space. Drydock vessel for hull inspection prior to carrying passengers. - 46 CFR 176.600(d)

Prove watertightness of lazarette forward bulkhead. - 46 CFR 176.600(d)

Tabbing around clamp on port and starboard side is delaminated. Provide repair proposal and make available for MI. - 46 CFR 176.600(d)

Vessel has no FCC station license as required, must obtain lic.

Vessels P/A system needs an aft speaker that works for emergency situations, clear communication between pilot house and main deck

Vessel must put distress flares in a water tight container of bright color and mark distress signals.

Vessels vent closure will not close as required must repair to like new condition and close properly with CGMI.

Vessels fixed fire system (fireboy) does not alarm and engine will not restart in manual override. Must fix/correct system to operate as designed.

Vessel needs a nozzle on the fire hose that is corrosion resistant

Installed inflatable apparatus (IBA) is over 200 lb and inboard of a 36" high railing. IBA's over 200 lb need mechanical lifting devices if they must be lifted more than one vertical foot. Submit plan to comply with regs to satisfaction of Coast G uard Marine Inspector.

Provide EPRIB (was sent out for new battery). Vessel is restricted from operating on the high seas.

Crew shall conduct underway drills.
