Gap in port side forward machinery space boundary allows the passage of vapors. Make vapor tight to ensure quantity of extinguisher is sufficient for space. Resolve by July 18, 2025 to the satisfaction of the attending marine inspector. 46 CFR 181.420(a)(4)

Spaces containing machinery shall be fitted with at least two ducts to furnish natural or mechanical supply and exhaust ventilation. Operator unable to identify location of the two ducts where port side main engine draws ventilation. Demonstrate adequate ventilation or install in accordance with 46 CFR 182.20-45 (OLD T Regulations). 46 CFR 182.20-45 (c) OLD T

All vessels must have navigation lights that are in compliance with the applicable sections of the International and Inland Navigation Rules. Vessel mast light and stern light is not in compliance with Navigation Rule 23. Install navigation lights in accordance with 33 CFR 83.22(d) (Rule 23) for a vessel of less than 12 meters to the satisfaction of the MI. 46 CFR 183.420

Each vessel must be marked as required by part 67, subpart I, of this chapter. Vessel's name and hailing port markings missing on port side and stern. Replace with markings of appropriate size as described by 46 CFR 67.123(a)(d) to the satisfaction of the attending Marine Inspector. 46 CFR 185.602(a)

A durable placard must be posted next to all radiotelephone installations with the emergency broadcast instructions an information, specific to the individual vessel. Placard not located next to radiotelephone installation. Install next to radiotelephone. 46 CFR 184.506

A person may not employ or engage an individual in a position required to hold and MMC unless that individual maintains a current medical certificate. 01 employee (EMERSON HILLEN) serving as captain unable to produce proof of current medical certificate. Employee may not operate in capacity of captain until valid medical certificate is provided to the attending Marine Inspector. 46 CFR 15.401(c)

Each vessel that relies on electricity to power the following loads must be arranged so that the loads can be energized from two sources of electricity: Navigation equipment and lights. Existing all-around anchor light only powered by battery. Install appropriate navigation light in accordance with this regulation to the satisfaction of attending marine inspector. 46 CFR 183.310(a)(1)(iv)

In general, compliance with the standards established by a recognized classification society will be considered satisfactory evidence of the structural adequacy of the vessel. Area of plywood separation discovered in starboard aft compartment inner hull at waterline. Uncover and present for inspection at drydock. 46 CFR 177.10-1(a)

No major repairs or alterations affecting the safety of the vessel with regard to the hull, machinery, or equipment, shall be made without the knowledge and approval of the [name withheld] repairs present on the port side hull. Uncover and present for inspection at drydock. 46 CFR 176.20-1(a)

Whenever damage or deterioration to hull plating or structural members that may affect the seaworthiness of a vessel is discovered or suspected, the cognizant OCMI may conduct an internal structural examination in any affected space including fuel tanks, and may require the vessel to be drydocked or taken out of service to assess the extent of the damage, and to affect permanent repairs. the OCMI may also decrease the drydock examination intervals to monitor the vessel's structural condition. Vessel has 8 foot crack on exterior of starboard hull in way of wing deck causing water intrusion into starboard hull. Vessel restricted from carrying passengers until adequate repairs are completed during drydock and are accepted by a USCG Marine Inspector. 46 CFR 176.600(d) 701 C

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Unauthorized electrical connections identified between electrical wires. 46 CFR 183.200 (b)

A visual and audible alarm must be provided at the operating station to indicate a high water level in each of the following normally unmanned spaces. Port bilge visual indicator is not operational. 46 CFR 182.530

The marine employer must have a sufficient number of alcohol testing devices readily accessible on board the vessel. Vessel does not have any means to test for presence of alcohol. 46 CFR 4.06-15 (a)

All fuel tanks must be electrically bonded to a common ground. Grounding wire for port fuel tank is missing. 46 CFR 182.440(b)(4)

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Battery was found having six connections on each terminal. Reduce number of connections to a maximum of four. 46 CFR 183.200 (b)

Except as otherwise allowed by this subpart, a vessel must comply with the structural design requirements of one of the standards listed below for the hull material of the vessel. Vessel reported damage to a cleat on the forward part of the starboard sponson. Damage is a potential source of water ingress. Make repairs in accordance with discussed plans. 46 CFR 177.300(c)

Except as otherwise allowed by this subpart, a vessel must comply with the structural design requirements of one of the standards listed below for the hull material of the vessel. Vessel reported damage to the hull where the damaged drain line was leaking. Submit repair proposal.

A pre-engineered fixed gas fire extinguishing system must be installed per the manufacturer's instructions. The provided fixed extinguishing system in the port engine room is rated for 75 cuft. The rear bulkhead of the engine room was not reasonably gas tight and allowed for free communication of atmosphere to the port aft shaft space which increased the volume of the compartment in excess of 75 cuft. Provide a reasonably gas tight separation of the spaces.

A vessel must be provided with a main steering gear that is of adequate strength and capable of steering the vessel at all service speeds. Vessel's steering gear is loose and misaligned in many places. Repair to the satisfaction of the attending marine inspector. 46 CFR 182.610 (a)(1)

Except as otherwise allowed by this subpart, a vessel must comply with the structural design requirements of one of the standards listed below for the hull material of the vessel. Vessel has areas of suspected water damage of fiberglass underneath floor boards in fwd part of vessel, inside cabin area, bow starboard hull, topside aft starboard hull where railing is attached, inside engine room in port hull, and aft right bolt area on starboard side netting. Clean and submit findings with repair proposal. 46 CFR 177.300 (c)

A pre-engineered fix gas fire extinguishing system must have a means to reset devices used to automatically shut down ventilation systems & engines. STBD Fireboy failed to reset; prove proper operation to the attending MI.

A vessel on a limited coastwise route must carry six hand red flare distress signals and six hand orange smoke signals. Provide 06 hand red flares, and 03 hand orange flares. A red hand flare distress signal can be substituted for any of the hand orange smoke distress signals.

Flexible nonmetallic hose may be used in fuel oil systems subject to the following requirements: USCG Type A1-15. The STBD secondary fuel filter to primary filter supply lines are not intended for fuel injected engines. Change to USCG Type A1-15 or SAE J1527 Marine Fuel Hose.

Where a flexible vent pipe section is necessary, suitable flexible hose having high resistance to petroleum oils may be used. The STBD fuel tank vent hose is not designed for fuel. Change to USCG Type A-2 or SAE J1527 Marine Fuel Hose.

All vessels must have operable navigation lights IAW the Navigation Rules. Navigation lights are not wired IAW Navigation Rules. The anchor light needs to be rewired separate from other navigation lights.

Machinery and fuel tank spaces must be separated from accommodation spaces by boundaries that prevent the passage of vapors. The following areas were identified to not meet the vapor tight criteria, to include: the starboard engine space and fuel tank space leading to the storage compartments, as well as the port engine room. Provide a proper seal to prevent the passage of vapors.

Every vessel must be fitted with a suitable magnetic compass designed for marine use. Magnetic compass was not mounted; mount suitable magnetic compass in the vicinity of the operating station and illuminate or vessel is restricted to daytime operations.

A vessel must be provided with a main steering gear that is of adequate strength and capable of steering the vessel at service speeds. The fasteners on the steering ram to rudder tie rod mounting block are corroded, and need to be replaced.

A hatch exposed to the weather must be watertight. Some of the weather deck hatches are not watertight, including the port aft hatch. Make all weather deck hatches watertight tight.

A submersible electric bilge pump may be used as a power bilge pump on a vessel of not more than 65 feet in length carrying not more than 49 passengers, other than a ferry, provided that: The pump meets UL 1113. Install UL 1113 submersible electric bilge pumps.

Shutoff valves, installed so as to close against the fuel flow, must be fitted in the fuel supply lines at the tank connection. The shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located. The Port fuel tank fuel shut off valve does not close all the way. Demonstrate that this shut off valve can close entirely.

When the flexible section is a nonconductor of electricity, the metallic sections of the fill pipe separated thereby must be joined by a conductor for protection against generation of a static charge when filling with fuel. The bonding on the port and STBD fuel fill is in correct or missing; install proper bonding wire to the satisfaction of the attending MI.

A vessel inspected for compliance with the standards required by this subchapter. Machinery, equipment, materials, and arrangements not covered by standards covered in this subchapter may be inspected IAW standards acceptable to the cognizant OCMI as good marine practice. The both PSS shaft seal do not have an adequate vent line; provide a suitable vent line IAW the manufacturer installation procedures.

A Certificate of Documentation (COD) is required for the operation of a vessel in certain trades, serves as evidence of vessel nationality, and permits a vessel to be subject to preferred mortgages. Vessel Operator could not produce original COD. Provide original COD.

Each child size life jacket must be stowed in a location that is appropriately marked and separated from adult life jackets so the child size life jackets are not mistaken for adult life jackets. Child size life jackets were not stowed separately from adult life jackets. Stow child size life jackets that is separated from adult size life jackets.

A cable must not be spliced in a hazardous location. STBD engine room 12V red cable is spliced. Install either a single wire or an enclosure IAW 183.340(g).

Each life jacket, immersion suit, and ring lifebuoy must be marked in clearly legible block capital letters with the vessel's name. 01 ring lifebuoy was not properly labeled with the ship's name. Mark with vessel name.

The master of the vessel shall ensure that: the EPIRB test required by paragraph (a) shall be logged or otherwise documented, as applicable. EPIRB testing not conducted and not loggged. Test EPIRB and log.

The employer shall provide an Employee Assistance Program for all crew members that includes both education and training on the effects and consequences of drug and alcohol use. Supervisory personnel must receive at least 60 minutes of training. Vessel's O/O is establishing an Employee Assistance Program as part of the company's chemical testing program, but has not completed the program. Provide satisfactory evidence of an Employee Assistance Program.

A vessel on which a pre-engineered fixed gas fire extinguishing system is installed must have the following equipment at he operating station: A means to reset devices used to automatically shut down engines. Vessels two pre-engineered systems did not function properly and did not allow engines to restart. 46 CFR 176.810(b)

Vessel is required to ensure the EPIRB's battery is replaced after it is used, or before the date required by FCC regulations in 47 CFR part 80, whichever comes sooner. Vessel's EPIRB battery was past its expiration date. Replace battery to satisfaction of attending Coast Guard Marine Inspector. 46 CFR 185.728

Vessels operated solely within 20 nautical miles of land must be equipped with a VHF-DSC radiotelephone installation meeting the requirements of § 80.1101(c)(2). VHF radio missing GPS signal for DSC operation. 47 CFR 80.905(1)

Over current protection must be provided for each ungrounded conductor. An emergency switch must be provided in the normally ungrounded main supply conductor from a battery. Vessel has 12v wire from battery that bypasses emergency switch and has no over current protection. Determine load of conductor and wire appropriately. 46 CFR 183.380

FLEXIBLE NONMETALLIC HOSE PERMITTED FOR USE IN PIPING SYSTEMS MUST MEET SAE J-1942. HYDROLIC HOSES USED IN STEERING SYSTEM DID NOT MEET THE SAE J-1942 STANDARD. REPLACE THE HYDROLIC HOSES USED IN SYSTEM SYSTEM WITH SAE J-1942 STANDARD NONMETALLIC HOSES.

A HATCH EXPOSED TO THE WEATHER MUST BE WATERTIGHT. HATCH COVERS ON OPEN DECK WERE FOUND TO BE MISSING GASKETS RENDURING THEM NOT WATERTIGHT. ENSURE ALL HATCH COVERS HAVE GASKETS IN PLACE AND ENSURE THE WATERTIGHT INTEGRITY OF THE EACH HATCH COVER.

A VISUAL INDICATOR MUST BE PROVIDED AT THE OPERATING STATION TO INDICATE WHEN ANY AUTOMATIC BILGE PUMP IS OPERATING. THE TWO AUTOMATIC BILGE PUMPS ONBOARD THE VESSEL DID NOT HAVE A VISUAL INDICATOR AT THE OPERATING STATION. INSTALL A VISUAL INDICATOR AT THE OPERATING STATION TO INDICATE WHEN AN AUTOMATIC BILGE PUMP IS ACTIVATED.

CHILD SIZE LIFE JACKETS EQUAL TO AT LEAST 10% OF THE NUMBERS OF PERSONS PERMITTED ON BOARD MUST PROVIDED. VESSEL IS PERMITTED 42 PERSONS. PROVIDE 01 ADDITIONAL CHILD SIZED LIFE JACKET FOR A TOTAL OF 05 ONBOARD.

AFT BRACING BAR FOR STARBOARD SIDE HAMMOCK WAS FOUND TO BE IN AN UNSAFE CONDITION. BRACING BAR IS LOOSE. RESECURE THE AFT BRACING BAR FOR STARBOARD HAMMOCK.

VESSEL MUST HAVE NAVIGATION LIGHTS THAT ARE IN COMPLIANCE WITH THE APPLICABLE SECTIONS OF INTERNATIONAL AND INLAND NAVIGATION RULES. VESSEL'S NAVIGATION LIGHTS WERE HAZED REDENURING THEM NOT VISABLE IAW NAVIGATION RULES. REMOVE HAZE FROM NAVIGATION LIGHTS.

BATTERY CONNECTIONS MUST BE AMDE TO BATTERY TERMINALS WITH PERMANENT TYPE CONNECTORS. REPLACE WINGNUT CONNECTORS WITH PERMANENT HEXNUT TYPE CONNECTORS.

07109: A pre-engineered fixed gas fire extinguishing system must automatically shut down all power ventilation systems and all engines that draw intake air from within the protected space. Currently, fire-boy systems are interconnected so when one fire-boy is discharged, all engines are shut down. 46 CFR 181.420

11116: A vessel on a limited coastwise route must carry 6 hand red flares and 6 hand orange smoke distress signals. Vessel is missing one hand red flare. 46 CFR 180.68(a)

10111: Vessel must carry adequate and up-to-date chart, U.S. Coast Pilot, and CG Light List. Vessel does not have an up-to-date chart, CG light list, and Coast Pilot on board. 46 CFR 184.420

13104: A visual indicator must be provided at the operating station to indicate when any automatic bilge pump is operating. A visual indicator is not provided for both bilge pumps on board. 46 CFR 182.530(c)

04113: A visual and audible alarm must be provided at the operating station to indicate a high water level. Port side high water alarm visual indicator is inoperable. 46 CFR 182.530(a)

05199: A durable placard must be posted next to all radio telephone installations with the emergency broadcast instructions and information specific to the vessel. Vessel does not have an emergency broadcast placard next to VHF radio onboard. 46 CFR 184.506

Replace retroreflective tape on identified (5) adult life jackets
