Each ungrounded conductor of circuit supplying lights, motors or appliances shall be equipped with a circuit breaker or switch and fuses at the point of attachment to the power source. This applies also to added circuits. Battery installation supplying power to the aft deck davit from the engine room and batteries supplying power to the wheel house communications equipment lacks circuit breakers or switches and fuses. 46CFR183.05-15(c)(Old T)

Provide a minimum standard of regulations for electrical installations and equipment that will result in reasonably safe from electrical shock of personnel. Installed 220 volt fans in lazarette, stbd side overhead have exposed wiring creating shock hazard. 46CFR183.01-5 (Old T)

Cables entering boxes or fittings shall be protected from abrasion, and shall conform with the following requirements:(1) Openings through which conductors enter shall be adequately closed. (2) The cable armor shall be secured to the box or fitting. Box penetrations throughout vessel and behind air compressor panel in engine room are missing abrasion protection & unsecured.

Receptacle outlets and attachment plugs for the attachment of portable lamps, tools, and similar apparatus supplies as ship's equipment and operating at 100 volts or more, shall provide a grounding pole and a grounding conductor in the portable cord, to ground the non-current carrying metal parts of the portable apparatus. The below outlets are improperly wired: 1.Berthing outlets missing ground connection. 2.STBD Wheel House and STBD Head have open neutral circ. 3.E/R outlet below compressor panel has hot & neutral reversed. 46CFR183.10-25(o) (Old T)

Wiring joints and splices shall be mechanically secure and made in junction boxes. Connections throughout vessel not in junction boxes. 46CFR183.05-50(b)(Old T)

Fire hazards to be minimized. The general construction of the vessel must be such as to minimize fire hazards insofar as it is reasonable and practicable. Blower fan motor in engine room, overhead stbd compressor is saturated with excessive oily residues and requires de-greasing. 46CFR177.405(a)

Valves for removing water or impurities from fuel in watertraps or strainers will be permitted. Such valves shall be provided with caps or plugs to prevent fuel leakage. Port Generator filter flame bowl missing plug. 46CFR182.20-40(b)(7)

This part is intended to provide a minimum standard of regulations for electrical installations and equipment that will result in a vessel that is reasonably safe from fire caused by and/or originating in electrical equipment, and reasonably safe from electrical shock of personnel. Outlet boxes in lazarette found with improper/poorly fitted faceplates. Excessive dead end wires found in wheel house.

Where ends of stranded conductors are to be clamped under terminal screws, they shall be formed and soldered unless fitted with pressure terminal connectors listed by Underwriters' Laboratories, Inc. Conductors to compressor soft start screw terminals are missing solder or UL certified terminal connectors.

A vessel must be operated in accordance with applicable laws and regulations in such a manner as to afford adequate precaution against hazards that might endanger the vessel and the persons to be transported. Deficiencies 02-07 provide objective evidence that the vessel is substandard and not proactively managing technical and operational requirements. Vessel must complete a satisfactory COI re-inspection prior to any movement.

For fixed-gas fire extinguishing systems, the inspections and tests required by Table 176.810(b) of this section, in addition to the tests required by 46 CFR 147.60 and 147.65. The owner or managing operator must provide satisfactory evidence of the required servicing to the marine inspector. During annual inspection, service technicians found a faulty heat detector disabling the systems automatic activation. Provide a satisfactory technician's servicing report.

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, and the vessel from electrical hazards, including fire and electrical shock. The vessel had numerous wiring issues which posed a major hazard to passengers and crew. The hazards identified include but were not limited to: Various exposed and dead end wiring, broken, improperly mounted electrical switches, missing over-current protection on numerous conductors, numerous fire/ electrical shock electrical situations, non-UL tested lighting fixtures found throughout the vessel, permanently mounted battery chargers not of marine type and are equipped with spring type terminal connections, broken light fixtures in the engine-room, portable household type fans set up throughout vessel for permanent use. Vessel owner is required to submit a third-party survey report from a certified marine electrician to the OCMI.

Repairs or alterations to... equipment that affect the safety of the vessel must not be made without the approval of the cognizant [name withheld] or alterations that affect the safety of the vessel include but are not limited to:..damage repair or replacement, other than replacement in kind, of electrical wiring. Numerous modifications have been made throughout both the vessels AC and DC electrical systems. Vessel owner is required to submit updated electrical plans including: Elementary one-line diagram of the power system; Cable lists; Bills of materials; Type and size of generators and prime movers; Type and size of generator cables, bus-tie cables, feeders, and branch circuit cables; Power, lighting, and interior communication panelboards with number of circuits and rating of energy consuming devices; Type of capacity of storage batteries; Rating of circuit breakers and switches, interrupting capacity of circuit breakers, and rating and setting of overcurrent devices; and Electrical plant load analysis to the Marine Safety Center for review and approval.

A vessel not required to have a power-driven fire pump by 181.300 must have at least three 2.5 gallon buckets with an attached lanyard, placed so as to be easily available during an emergency. One of the onboard fire buckets was rotted and cracked. Bucket to be replaced.

The individual bilge suction lines shall be led to a central point or manifold. Each line…shall be provided with a check valve at some accessible point in the bilge line. The check valve for the engine room bilge line was frozen, prohibiting the bilge system from operating as designed in the engine room. Prove proper operation.

The floating waterlight must be attached around the body of the life float or buoyant apparatus by a 10 mm (3/8 inch) lanyard, resistant to deterioration from ultraviolet light, and at least 5.5 meters (18 feet) in length. The waterlight lanyard was severely deteriorated and requires replacement.

Vessels described by 46 CFR 175.110(c) must have an interconnected fire detection system in compliance with § 181.450 installed in all enclosed areas where passengers and crew have routine access, including accommodation spaces and machinery spaces. Engine room interconnected smoke detector was missing batteries.

Every vessel subject to Part III of Title III of the Communications Act must have a detailed inspection of the radio installation by an FFC-licensed technician in accordance with §80.59 once every five years. If the ship passes the inspection, the technician will issue a Communications Act Safety Radiotelephony Certificate. Vessel did not have a valid safety radiotelephony certificate. Vessel restricted to 1000 feet from shore.

A vessel must comply with the applicable requirements for any radio… including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. Vessel did not have a valid FCC station license. Vessel restricted to 1000 feet from shore.

Each launching appliance and each survival craft and rescue boat on a vessel must be in good working order and ready for immediate use before the vessel leaves port and at all times when the vessel is underway. The required retro-reflective material on both life floats was found to be worn and wasted and requires replacement.

Complete but simple instructions for the operation of a fixed gas fire extinguishing system must be located in a conspicuous place at or near each pull box and stop valve control and in the space where the extinguishing agent cylinders are stored. If the storage cylinders are separate from the protected space, the instructions must also include a schematic diagram of the system and instructions detailing alternate methods of releasing the extinguishing agent should the local manual release or stop valve controls fail to operate. Each control valve to a distribution line must be marked to indicate the space served. The installed CO2 system did not have required instructions. 46CFR185.612(a)

Each entrance to a space storing carbon dioxide cylinders, a space protected by carbon dioxide systems, or any space into which carbon dioxide might migrate must be conspicuously marked as follows: (1) Spaces storing carbon dioxide—“CARBON DIOXIDE GAS CAN CAUSE INJURY OR DEATH. VENTILATE THE AREA BEFORE ENTERING. A HIGH CONCENTRATION CAN OCCUR IN THIS AREA AND CAN CAUSE SUFFOCATION.” and (2) Spaces protected by carbon dioxide—“CARBON DIOXIDE GAS CAN CAUSE INJURY OR DEATH. WHEN ALARM OPERATES DO NOT ENTER UNTIL VENTILATED. LOCK OUT SYSTEM WHEN SERVICING.”.(g)Both the Engine Room and CO2 storage space are missing required warning signage and require proper signage. 46CFR185.612

Remote fuel shutoff stations must be marked in clearly legible letters at least 25 millimeters (1 inch) high indicating purpose of the valve and direction of operation. Emergency fuel shut-off markings warn and require renewal. 46CFR185.608

An interconnected detection and alarm system must be installed such that the actuation of alarm in one area results in both audible and visual alarms in all areas required by 46 CFR 181.405(c) or 118.400(d) to be protected by the interconnected detection and alarm system. Fire detection system on board was not interconnected and activation of one detector did not activate alarms in other spaces. Prove proper operation of interconnectivity of alarms.

Portable fire extinguishers must be located so that they are clearly visible and readily accessible from the space being protected. The installation and location must be to the satisfaction of the Officer in Charge, Marine Inspection. Portable fire extinguishers were not installed in appropriate bracket as prescribed by manufacturer to meet USCG approval. 46CFR181.520

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date: U.S. Coast Pilot or similar publication; Coast Guard Light List ; tide tables; and current tables. Extracts from the publications listed above for the areas to be transited may be provided instead of the complete publication. Publications were out of date. 46CFR184.430

Each battery installation must be located in a well ventilated space and protected from falling objects. Batteries were not covered or adequately secured. Cover and properly secure all batteries in engine room.

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected. Large tool box on engine room deck was found unsecured posing a hazardous situation in heavy seas. Remove or secure tool box. 46CFR176.830

Each connection to a conductor or terminal part of a conductor must be made within an enclosure and have either: (1) A pressure type connector on each conductor; (2) A solder lug on each conductor; (3) A splice made with a pressure type connector to a flexible lead or conductor; or (4) A splice that is soldered, brazed, or welded to a flexible lead or conductor. Shore power cable connection is not in a required enclosure. 46CFR183(g)

Electrical equipment and installations must be suitable for the roll, pitch, and vibration of the vessel underway. Vessel has battery chargers installed that are not marine grade nor suitable for commercial vessel service. Battery chargers shall be of marine grade and permanently mounted. 46CFR183.220

The owner or managing operator shall submit drawings, sketches, or written specifications describing the details of any proposed alterations to the cognizant [name withheld] alterations must be approved by the OCMI before work is started. Electrical installation including, but not limited to: (i) Elementary one-line diagram of the power system; (ii) Cable lists; (iii) Bills of materials; (v) Type and size of cables, bus-tie cables, feeders, and branch circuit cables; (viii) Rating of circuit breakers and switches, interrupting capacity of circuit breakers, and rating and setting of overcurrent devices. During an annual inspection it was found that several small ventilation fans were installed below the bunks in the berthing area. This installation was completed with-out plan review or USCG oversight. This system must remain disconnected from a power source until plan review and approval is carried out by the USCG [name withheld] plans as listed above to the USCG for review. 46CFR176.700(b) and 46CFR177.202(b)

Not later than April 1, 2024, an operator of an overnight fishing charter not in compliance with such section 3306(n)(3)(A)(v) shall submit to the Commandant a plan for complying with such requirements. A plan for IFR compliance has not been submitted to the cognizant [name withheld] are prohibited access to overnight accommodation spaces. NDAA FY24, Section 3531(b)

The master shall conduct sufficient fire drills to make sure that each crewmember is familiar with his or hers duties in case of fire. Operator was medically unable to conduct drills. Vsl cannot carry passerngers until drills are completed in the presense of a Marine Inspector.

A vessel must carry either a first aid kit approved. The contents of the first aid kit were expired.

Rules concerning lights shall be complied with from sunset to sunrise. Electrical connections to the port and stbd navigation lights were lose causing the lights to work intermittently.

Owner was not familiar or comfortable with conducting QFA/DVTP's post repower. The operator who normally runs the vessel was medically unable to attend the inspection. QFA/DVTP's must be completed prior to repower credit being issued. Vessel in not allowed to carry passengers until the QFA/DVTP's have been satisfacotily witnessed by an attending Marine Inspector.

Vessels described by 46 CFR 175.110(d) must submit plans to the cognizant OCMI, in accordance with 46 CFR 176.700, for the installation and use of monitoring device to ensure the wakefulness of the watchmen required in paragraph (a) of this section. Vessel has not submitted Watchmen Plans to the OCMI for review and approval. Vessel is prohibited from carrying passergers overnight until Watchmen Plans have been submitted and approved by OCMI.

Shutoff valves for fuel tanks shall be provided with suitable arrangements to be able to stop fuel in the event of emergency from outside the space. Shutoff valves for aft port and starboard fuel tanks were found missing the valve handles and unable to be shutoff from outside the space. Vessel is restricted from use of port and starboard aft fuel tanks until suitable arrangements can be provided to shutoff fuel from weather deck. OLD T 46CFR182.20-40(a)(3)

Covered metal trash cans shall be provided in lounge areas and maintinaed to minimize fire hazards. PPlastic uncovered trash cans (02) were found in galley/lounge areas. OLD T 46CFR177.30-7(d)

An exposed hazard, such as gears or rotating machinery, must be properly protected by a cover, guard, or rail. MI found 03 rotating couplings with no guarding in place as well as no guarding around the STBD main diesel engine belt drive for the installed bilge pump. Install guarding around the four hazards to the satisfaction the the USCG MI.

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected. MI found 01 Bait pump not secured to the deck. Damage from movement/vibrations from the pump could cause a possible flooding situation. Secure the bait pump to the deck to the satisfaction of the USCG MI.

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected. MI found excessive amounts of combustibles and gear adrift in the engine room. Organize, secure and remove combustibles from space to the satisfaction of the USCG MI.

At each initial and subsequent inspection for certification, the owner or managing operator must be prepared to conduct tests and have the vessel ready for its inspection of its fire protection equipment. The servicing certificates for the fixed and portable extinguishers are expired. Vessel to provide proof of proper inspection.

A suitable metal marine type strainer meeting the requirements, must be fitted in the fuel supply line. Vessel to provide flame bowl for missing flame bowl on Generator #1.

The master shall conduct sufficient drills to make sure that all crew members are familiar with their duties during emergencies. Due to weather, unable to get vessel underway to conduct drills. Vessel to conduct drills for credit COI.

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. Vessel reported starboard engine would not start. Vessel has scheduled a tech to troubleshoot starboard engine. Vessel is to provide tech report and prove proper operation of starboard main diesel engine prior to carrying passengers.

Due to COVID-19 circumstances beyond the control of the vessel, the vessel will need be reinspected to meet 46 CFR 176.500(b). Vessel is allowed 90 days to continue operations in accordance with the COI, and is required to make itself available for inspection prior to 28 JULY 20.

46 CFR 177.410(a) 705c Vertical or horizontal surfaces within 3 feet of cooking appliances must have an ASTM E-84 flame spread rating of not more than 75. Oven in galley is surrounded by wood. Provide required structural fire protection around oven.

46 CFR 176.700(a) 705c Repairs or alterations to the hull, machinery, or equipment must not be made without approval of the cognizant [name withheld] compressor was found in STBD lazarette installed without approval. Submit electrical plans to MSC for approval of stbd air compressor in lazarette.

46 CFR 178.330(a)(4)(v) 705c If vessel carries passengers on dive excursions, not less than 80lb should be assumed for each person. Stability letter does not account for dive gear. Vessel cannot exceed a total weight of 7,480 lbs. 1 diver = 265lbs, 1 non-diver = 185 lbs.

All other vessel must undergo an inspection for certification as specified in 176.404 and annual inspection as specified in paragraph (b)(1) of this section. (b)(1) Annual Inspection. Your vessel must undergo an annual inspection within the 3 months before or after each anniversary date. Vessel is currently overdue for required annual inspection. Vessel is required to complete a satisfactory annual inspection prior to carrying passengers for hire.

Found 1 of 5 chiild life jackets to be inserviceable. A child life jacket must be provided for at least 10% of the number of persons permitted on board. Provide adequate number of serviceable child life jackets to the MI.

Replace 05 adult lifejackets due to excessive wear/wasting. Replace retroreflective material on 04 additional childrens life jackets. Limited total passenger count to 37 with no more than one child until corrected.

Painter lines not permanently spliced to buoyant apparatus or vessel.

Water lights were missing prongs and in need of replacement.

Provide passenger count for embarking and disembarking passengers.
