SOLAS 1974 II-2 Regulation 21 Currently, All the crew berthing areas (below the bulkhead deck) have two means of escape. However, the second means of escape are in crewmember berthing rooms above the top bunks. Furthermore, the two means of escape for each berthing area lead to a common space (i.e. Lounge/lounges) above the crew berthing areas. Using the forward crew berth as an example: If a fire were to break out in the forward bar (located in the forward lounge) the crew below would have no escape route. The same scenario for the large aft crew berths under the Galley and main passenger lounge. * Submit plans to Coast Guard Marine Safety Center to correct the deficient means of escape from all three crew berths prior to sailing foreign. Until the means of escape meet SOLAS 1974 II-2 Regulations 21, the vessel is restricted to domestic voyages only.

The fuel oil / lube oil transfer pump was found without safety guard over the motor/pump coupling. Provide the pump with a protective guard to prevent personnel injury.

The vessel's fire nozzles were found without retaining devices on the nozzle caps. Provide each fire nozzle with a permanent retaining chain.

Observed a vessel repair plan for 8"X7' on the STBD side shell from frame 29-30'. Repairs are to completed as per plan, due to hole in side shell.

Observed repair plan and insert repair shall continue on portside 3' mark, via plan.

Grey water overboard discharge valve was inoperable and would not close all the way. Replace or repair the overboard discharge valve, prove proper operation afterwards.

Observed wasted steel on deck by STBD side shell from frame 29. Crop out steel, Coast Guard inspector must be present after cut-out to assess steel, once cleared to continue repairs may continue, Coast Guard inspector must be present for NDT prior to painting.

Vessel has a temporary doubler repair on both STBD and portside mid body of the keel. Final repairs are to be made at next dry dock.

Vessel is to complete final repairs to FWD F/O TK by next dry dock.

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant [name withheld] autopilot failure steering procedure is for an analog system and not for the digital system installed on-board. Requested specific procedure for autopilot failure be amended and/or changed and resubmitted to MSC for approval. 46CFR115.700

Vessels described by 46 CFR 114.110(f) must submit plans to the cognizant OCMI, in accordance with 46 CFR 115.700, for the installation and use of monitoring device(s) to ensure the wakefulness of the watchmen required in paragraph (a) of this section. Vessels with a keel laid date after March 28, 2022, must include plans for the monitoring device(s) within the plan submissions required in 46 CFR 116.202. The Coast Guard will work with the vessel operators to determine a reasonable implementation schedule once the plans are accepted. The monitoring device(s) must: (1) Ensure the wakefulness of the crew in the event that the watchman required in paragraph (a) of this section is unresponsive; (2) Remain operable during the nighttime watch; and (3) Be arranged to ensure proper coverage of the passenger accommodation spaces, common areas, and spaces with potential fire hazards. Vessel does not have an installed monitoring device on-board. Provide the following: (1) Submit plans to SEAK OCMI in accordance with 46CFR 115.700, for the installation and use of monitoring within 20 days.(2) Upon review of plan by a Marine Inspector, install and test system in accordiance with the submitted plan. 46CFR122.410

A hold back mechanism for a door must allow the door to be released upon a signal from a control space. Marine Inspector found fire doors were not able to close remotely from the bridge and system could not indicate when the doors were closed. 46 CFR 116.435(c)(6)(ii)(B) 16c

All engines must have at least two means of stopping the engine(s) under any operating conditions. Marine inspector found emergency stop on FWD Ships Service Diesel Generator was not able to secure the engine. 46 CFR 119.200(b) 17c

At each inspection for certification and at every other vessel inspection the bilges and other spaces may be examined to see that there is no excessive accumulation of oil, trash, debris, or other matter that might create a fire hazard, clog bilge pumping systems, or block emergency escapes. Marine Inspector found oily water, oily rags, oil containers and paint cans in the engine room bilges. 46 CFR 115.830(b) 17c

Each item of lifesaving equipment determined by the marine inspector to not be in serviceable condition must be repaired or replaced. Marine Inspector found 02 immersion suits in need of replacement due to the age and condition of the suit. 46 CFR 115.808(b) 17c

The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection. Marine Inspector found plexiglass covers on sewage and treatment tanks.

Reparis and Alterations: Newly installed steering system is not approved by the Coast Guard. Owners have 07 days to address comments from MSC letter dated 05 JAN 2023. Provide proof of correspondence with MCS. Owners have 07 days to submit QFA/DVTP to MSC for approval as per CG-CVC policy letters 17-07 and 17-08. Owners have 30 days to test approved DVTP's and to incorporate in preventive maintenance system testing. 46 CFR 115.700(a) 705c

An abandon ship drill must be conducted as follows: Each member of the crew shall participate in at least one drill each month. Marine Inspector found abandon ship was not completed in November 2023. As per vessel's Safety Management System, create a non-conformity and corrective action plan and submit to the OCMI for review in order to clear deficiency. 46 CFR 122.520(b)(1) 16c

The following tests ans inspections must be carried out weekly on a vessel: Each rescue boat must be visually inspected, engine must be run ahead and astern for not less than 3 min, and each battery for rescue boat engine starting must be brought up to fullcharge at least once each week. MI noted that rescue boat weekly maintenance/inspections were missed on the following dates - November 12, 16 and 19 2023. As per vessel's Safety Management System, create a non-conformity and corrective action plan and submit to the OCMI for review in order to clear deficiency. 46 CFR 122.720 16c

Administrators shall require that records of daily hours of rest of seafarers be maintained in a standardized format to allow monitoring and verification of compliance with the provisions of this section. Marine Inspector found records for October 2023 were not onboard. As per vessel's Safety Management System, create a non-conformity and corrective action plan and submit to the OCMI for review to correct the deficiency. STCW A-VIII/1.7 74 SOLAS AMEND IX/3 ISM Code 10.2.4 16c

Each vessel must be provided with a sound powered telephone system, complying with subpart 113.30 of this chapter, to communicate between the pilothouse and steering compartment. Sound powered phone from pilothouse to steering compartment is not functioning as designed. 46CFR119.600 46CFR58.25-15 16c

The owner, managing operator, or a qualified servicing facility as applicable shall conduct the following inspections and tests: (1) For portable fire extinguishers, the inspections, maintenance procedures and hydrostatic pressure tests required by Chapter 4 of NFPA 10, (incorporated by reference, see § 114.600 of this chapter) with the frequency specified by NFPA 10. In addition, carbon dioxide and halon portable fire extinguishers must be refilled when the net content weight loss exceeds that specified for fixed systems by Table 115.810(b). The owner or managing operator shall provide satisfactory evidence of the required servicing to the marine inspector. If any of the equipment or records have not been properly maintained, a qualified servicing facility may be required to perform the required inspections, maintenance procedures, and hydrostatic pressure tests. A tag issued by a qualified servicing organization, and attached to each extinguisher, may be accepted as evidence that the necessary maintenance procedures have been conducted. (2) For semiportable and fixed gas fire extinguishing systems, the inspections and tests required by Table 115.810(b), in addition to the tests required by §§ 147.60 and 147.65 of this chapter. The owner or managing operator shall provide satisfactory evidence of the required servicing to the marine inspector. If any of the equipment or records have not been properly maintained, a qualified servicing facility may be required to perform the required inspections, maintenance procedures, and hydrostatic pressure tests. Marine Inspector found fire protection and detection equipment expired for 7 days while vessel was in operation in transit to Panama. As per vessel's Safety Management System, create a non-conformity and corrective action plan and submit to the OCMI for review in order to clear deficiency. 46 CFR 115.810(b) 74 SOLAS AMEND IX/3 ISM CODE 10.2.1 10c

All persons employed or engaged onboard vessels to which STCW applies will be issued a medical certificate valid for two years unless the mariner is under the age of 18, in which the maximum period of validity will be 1 year. Marine Inspector found expired medical certificate for crew member assigned for Chief Engineer position. As per vessel's Safety Management System, create a non-conformity and corrective action plan and submit to the OCMI for review to correct the deficiency. 46 CFR 10.301(b)(1) 16c

Except as otherwise provided in this section, all vessels shall be provided with not less than two avenues of escape from all general areas accessible to the passengers or where the crew may be quartered or normally employed, so located that if one is not available the other may be. At least one of the avenues of escape shall be independent of watertight doors. Crew berthing's secondary means of escape currently has plywood and/or couch cushions sitting on top of escape hatches, obstructing escape. Illustrate an effection means of unobstructed escape.

Tubular gauge glasses must be of heat resistant materials and be provided with devices that will automatically close at the tank connection. Closing valve on gauge glass for the fuel tank at emergency generator was broken-off. Must replace to satisfaction of CG Marine Inspector.

Electrical equipment on a vessel must be installed and maintained to prevent electrical ignition of flammable vapors. Wiring going into control relay box on port main engine had exposed wiring and face plate on electrical outlet next to No. One Generator must be replaced.

All newly installed control systems for steering and propulsion tested sat for DVTP/QFA. Vessel operators submitted plans to MSC and waiting approval. Vessel required to have QFA/DTVP for new control systems by next COI exam.

MI identified vessel does not have required radios for firemen suits. Vessel to provide radios.

Vessel to demonstrate crew knowledge and skills for the following drills: fire, abandon ship, man overboard, and security drill.

Vessel to prove accuracey of fire control plan.

All CO2 dead-end lines shall extend at least 2 inches beyond the last orifice and shall be closed with a cap or plug. The two end bells on the fixed CO2 system in the engine room have no extension. Install required extension to the dead-ends of the CO2 system in the engine room. 46 CFR 181.20-20(d)

Connections must be made to battery terminals with permanent type connectors. 04 batteries in engine room were discovered to have wing nut terminal connections. Replace wing nut connections with permanent type connectors. 46 CFR 120.350(d)

All-round lights shall be so located as not to be obscured by masts, topmasts, or structures within angular sectors of more than 6 degrees, except anchor lights prescribed in Rule 30. The placement of the all-around anchor light does not have 360 degrees of visibility due to obstructions on the vessel. Anchor light shall be placed so that it is no longer obstructed.

Each vessel inspected and certificated under the provisions of this subchapter must, when any passengers are aboard during the tenure of the certificate, be in full compliance with the terms of the certificate. Plan review and official certification for DIBS and Zodiacs authorized as tenders on vessels COI are incomplete. Tenders are ineligible for operation as certificated small passenger vessels until completion of certification and therefor are restricted to carrying no more than six passengers in accordance with applicable regualtions for uninspected passenger vessels.

Repair gaps at bottom of galley fire screen doors - entry and bottom of sliding window door.

Replace paint locker with full steel locker or line wholly with steel or other equivilant material.

Properly store all lifejackets, construction materials, equipment, etc ... prior to passenger operation.

AIS must be maintained in effective operating condition, which includes the accurate input and upkeep of all AIS data fields. The vessel is transmitting inaccurate AIS data. 33 CFR 164.46(d)(2).

Mount hanging light fixtures in engine room.

Remove plastic garbage containers and replace with non combustible containers. 46CFR116.405(i).

Wasted deck around vent pipe located under starboard interior benches in forward lounge going down into forward crew berthing. The vessel made temporary repairs with epoxy and will renew metal during next dry dock; vessel requested to make epoxy repair this dry-dock, because major steel and hull replacement was scheduled for 2011 dry-dock period. Special note added to MISLE.

Provide proof that all mattresses onboard meet 46 CFR 116.405(j).

Replace turbo lube oil drain flexible hose on both MDE's.

Lazerette bilge suction strainer was missing. Required vessel to install strainer.

Flexible loop hoses on fixed CO2 bottles were cracked/checked, required vsl to renew hoses.

Install flame screen in emergency generator fuel vent.

Chest Freezer on exterior 2nd deck is plugged into an extension cord that runs into a storage space.

Starboard fuel remote shutdown valve was inoperable.

Excessive wastage was noted at the base of the machinery space ventilation intake bulkhead on the 2nd deck. The wasted bulkhead area was not structural, but prevented the bulkhead from being weathertight and allowed the ingress of water into the ventilation duct and machinery space.

Found several electrical outlets and juntion boxes in the engine room that were not properly mounted. Securely mount these electrical boxes.

Found that two emergency escape routes below the main deck pass through crew staterooms which have locking door knobs, potentially prohibiting escape. Provide method of bypassing locks in an emergency or remove locking devices.

Provide 8 additional adult PFD's for a total of 125 adult PFD's onboard.

Provide plans for appoval for the installation of sea chest for underwater bow camera. Include associated wiring diagram and specs and sea valve specs.

Provide plans for the installation of bridge deck awning and structure. Provide proof that installation will not effect the vessels existing stability.

Provide proof that new carpet installation meets ASTM fire test standards.

Conduct non destructive testing of hull plating in the following identified areas; port shaft log hull plating, 900 voids, and 400 voids. Crop and renew the 5/16" plating that is 25% wasted to the satisfaction of an attending USCG MI or ABS surveyor.

Prove operation of inflatable boat navigation lights.

Crop, renew, and prove watertight the identified area of the fantail watertight plating on the fantail starboard air vent that penetrates into the lazarete

Replace expired items in first aid kit.

Provide fire-stop in all wire penetratrations in fire tight bulkheads.

Replace damaged fuel/oil transfer hose in engine room.

Provide metallic cable hangers in engine room.

Make permanent repairs to starboard aft galley bulkhead at next dry dock.

Provide proof that the carpet pad installed on the vessel is the pad stated on the certificates provided to the attending Marine Inspector, or provide certificates for the installed pad. If the carpet pad does not meet the requirements specified in NVIC 9-97, the carpet pad must be removed and replaced with an approved type.

Replace retroreflective tape on 1 immersion suit.

Replace lights on 2 immersion suites.

Service dry chemical fire extinguisher on 1 RHIB.

Prove proper operation of the emergency generator.

Prove proper operation of the stbd and stern navigation lights.

RENEW 4 FUEL OIL HOSES ON THE RACOR PRESSURE GUAGES ON BOTH MAIN ENGINES AND BOTH AUXILIARY GENERATORS USING AN APPROVED TYPE OF HOSE.

RENEW THE DETERIORATED FUEL OIL SUPPLY HOSE ON THE RACOR FILTER FOR THE FORWARD GENERATOR.

PROVIDE 2 CHILDRENS LIFE PRESERVERS. UNTIL THIS ITEM IS CORRECTED, THE MAXIMUM NUMBER OF CHILDREN PERMITTED IS 15.
