instruction placard should contain at least the
applicable portions of the “Emergency Instructions”

alcohol testing devices readily accessible on board the
vessel to determine the presence of alcohol in the
system of each individual who was directly involved in
the SMI. Vessel's testing devices were expired. Until
such time that valid testing devices are available
onboard,vessel must stay within 2 hours of a location to perform the required alcohol tests from the time of
occurrence of the SMI. Vessel will need to replace
testing devices as required and or show written
policy attesting to how the vessel will obtain the
devices and conduct the required alcohol tests within 2
hours from the time of occurrence of the SMI.

by suppling the vessel with satisfactory life jackets.

Deck rails must be designed and constructed to withstand a point load of 91 kilograms (200 pounds) applied at any point in any direction, and a uniform load of 74 kilograms per meter (50 pounds per foot) applied to the top rail in any direction. The point and uniform loads do not need to be applied simultaneously.
Rail stanchions on the port aft and starboard mid-ships were found with epoxied repairs at the base. Remove epoxy and reweld to ther satisfaction of the attending USCG marine inspector.
