The owner shall be prepared to conduct tests and have the vessel ready for inspection of the following: On a sailing vessel all spars, standing rigging, running rigging, blocks, fittings, and sails. Support/chaffing plate for starboard superior diamond stay found with cathodic corrosion causing plate to bend and pull away from mast. Operator shall be required to follow recommendation of 3rd party rigging survey report dated 8/10/2025 to replace standing rigging and turnbuckles no later than October 1, 2025, remove all diamond chain plates for inspection/replacement when the mast is on the ground and coordinate with USCG to receive a 6-yr mast-out inspection credit accordance with USCG NVIC 02-16. 46 CFR 176.802(a)(3)

No marine employer shall engage or employ any individual to serve as a crew member unless the individual passes a chemical test for dangerous drugs for that employer. Employer did not have pre-employment test results and proof of drug awareness training for 02 crew members. Provide satisfactory proof that all crew members have passed pre-employment testing and completed drug awareness training prior to those individuals serving in a crew member role on the vessel. 46 CFR 16.210(a)

A vessel must be fitted with ground tackle and mooring lines necessary for the vessel to be safely anchored or moored. Vessel reported broken anchor chain during storm. Anchor chain requires replacement. 46 CFR 183.400

The following spaces must be equipped with a fixed gas fire extinguishing system in compliance with 181.410: A space containing machinery powered be gasoline. Fixed gas fire extinguishing system (Fireboy) for port amidship generator space. Provide a certified technician service report & demonstrate all shutdowns, alarms, and overrides to the attending USCG marine inspector. 46 CFR 181.400(a)(4)

Machinery and equipment installed on each vessel must be suitable for the vessel and its operation and for the purpose intended. Generator inoperable from start up panel. Make generator operational as designed. 46 CFR 182.100

Operator shall schedule a 3rd party rigging surveyor to adequately assess the safe condition of their sail rigging system in accordance with USCG NVIC 02-16, provide the survey report prior to operating under sail. 46 CFR 176.802(a)(3)

A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. First aid kit is incomplete with expired contents. Provide USCG-approved kit or a kit that complies with the list in 46 CFR 160.041. 46 CFR 184.710

The owner shall submit the following plans,that are applicable to the vessel as determined by the OCMI: On sailing vessels: (i) Masts, including integration into the ship's structure; and (ii) Rigging plan showing sail areas and centers of effort as well as the arrangement, dimensions, and connections of the standing rigging. Provide complete updated 2024 Rigging PMP for vessel. 46 CFR 177.202(b)(12)

Electrical equipment must be installed to protect from electrical hazards. Grounding wires broken on single side band antenna and mast radio/lighting require proper reattachment. Conduit for mast wiring also broken exposing wiring. Requires replacement. 46 CFR 183.200

Each EAP education program must include display and distribution of informational material; a community service hot-line telephone number for crew member assistance, and display and distribution of the employer's policy regarding drug and alcohol use in the workplace. Service hot-line numbers required to be posted on vessel. 46 CFR 16.401

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. Exhaust leak located on port engine exhaust system. Repair exhaust piping to provide leak free exhaust system from port engine. 46 CFR 182.100

Portable and semi-portable extinguishers must be inspected and maintained in accordance with NFPA 10. The owner or managing operator must provide satisfactory evidence of the required servicing to the marine inspector. 46 CFR 176.810(b)(1)

The owner or managing operator of a vessel shall ensure that the vessel complies with the laws and regulations applicable to the vessel and that the vessel is otherwise satisfactory for the intended service. The inspection may include an inspection of the following items: Certificates issued by the FCC. FCC Safety Radio Certificate not valid. Provide inspection and valid FCC Safety Radio Certificate. 46 CFR 176.402

The OCMI may require the vessel to get underway. Get underway in presence of marine inspector to conduct drills. 46 CFR 176.404(a)

Vessel is required to ensure the EPIRB's battery is replaced after it is used, or before the date required by FCC regulations in 47 CFR part 80, whichever comes sooner. Vessel's EPIRB battery was past its expiration date. Replace battery to satisfaction of attending Coast Guard Marine Inspector. Vessel restricted to within three mile of shore. 46 CFR 185.728

The EPIRB test required shall be logged or otherwise documented monthly. Vessel unable to provide log for EPIRB tests. Provide monthly EPIRB test log. 46 CFR 185.728(c)

Firefighting drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. The drill entry shall include the following information: Date of the drill and training; and general description of the drill scenario and training topics. Vessel unable to provide logs for fire fighting drills and training. 46 CFR 185.524(d)

A vessel must be provided with a satisfactory arrangement for draining any watertight compartment, other than small buoyancy compartments, under all practicable conditions. Bilge system inoperable in starboard midship. Make bilge pumps operate as designed from bilge float. 46 CFR 182.500 (a)

Conditions of operations as may be determined by the cognizant Officer in Charge Marine Inspection (OCMI). Grills on vessel built of general purpose resin must have a dedicated 40-B:C fire extinguisher and grill shall be installed so that it can be jettisoned from the vessel with a single action while on fire, such as a pull cable to detach it from the deck railing. See Work Instruction 46 for details. 46 CFR 176.103

In systems and applications where flexible hoses are permitted to be clamped: Double hose clamping is required where practicable. Missing double hose clamps on bilge suction and discharge. Provide double hose clamps on all bilge lines. 46 CFR 182.410 (d) (1)

Shutoff valves, installed so as to close against the fuel flow, must be fitted in the fuel supply lines at the tank connection. The shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located. The vessel's port fuel shutoff valve was not fully operable. Service and prove proper operation of the port fuel shutoff valve. 46 CFR 182.455(b)(4)

All fiber reinforced plastic vessels constructed with general purpose resins must be fitted with a smoke activated fire detection system of an approved type (meeting 46 CFR 181.450), installed in accordance with 76.27 in subchapter H of this chapter in accommodation spaces and service spaces. Vessel does not have interconnected fire detection system for required spaces. Interconnected fire detection system must be in compliance with 46 CFR 181.450. Provide approved interconnected fire detection system and install to the satisfaction of the attending marine inspector. 46 CFR 177.410(c)(3)(ii)

The cognizant OCMI may require that a vessel and its equipment undergo any additional test or inspection deemed reasonable and necessary to determine that the vessel and its equipment are suitable for the service in which they are to be employed. Annual inspection completed on 12 Oct 2022 with deficiency issued to provide full rigging inventory of all old and new rigging components, inspections, and rigging details. Ensure to log all inspections in accordance with vessel PMP. Keep copy of PMP on board vessel at all times and submit revised version of PMP to [name withheld] The deficiency from 12 Oct 2022 has not been cleared. Updated and accurate PMP has not been provided. The vessel shall not fly sails after 31 March 2023 with passengers onboard until a third party rig inspection is completed IAW NVIC 02-16. 46 CFR 176.840

Vessel that operates on the high seas must have on board a FCC EPIRB installed to automatically float free. Hydrostatic release was expired. Provide valid hydrostatic release. Vessel restricted to operating within three miles from land. 46 CFR 180.64

Each ring buoy with floating light must have lanyard of at least 3 ft but not more than 6 ft. Lanyard for ring buoy floating light too long. Provide lanyard of proper length. 46 CFR 180.70

Each life jacket must be marked with type 1 retro reflective material. 13 life jackets had worn/aged retro reflective materials. Provide new retro reflective materiel for 13 life jackets. 46 CFR 185.604(h)

The name of the vessel must be marked on each side of the life float. The The number of persons capacity must be marked on each life float. Name of vessel and number missing from each life float. Provide vessel name and number capacity. 46 CFR 185.604(a) 46 CFR 185.604(e)

Each ring life buoy must be marked with retro reflective material. Worn/aged retro reflective materials on ring. Provide new retro reflective materiel for ring buoy. 46 CFR 185.604(i)

The fuel shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located. Fuel shutoff would not fully close. Provide fully operational fuel shutoff. 46 CFR 182.455(b)(4)

The number and identification of the items stowed inside and their sizes, must be marked in clearly legible letters and number on each container for life jackets. Marking for life jacket storage missing. Provide marking for life jacket stowage. 46 CFR 185.604(f)

A pre-engineered fixed gas fire extinguishing system must be installed in accordance with the manufactures instructions. Port side engine room pre-engineered fixed gas fire extinguishing system was discharged during inspection. Provide new pre-engineered fixed gas fire extinguishing system for port side engine room. 46 CFR 181.420(a)

Sail Rigging Preventative Maintenance Plan (PMP) must be updated and onboard vessel at all times. PMP not currently updated. Provide full rigging inventory of all old and new rigging components, inspections, and rigging details. Ensure all to log all inspections in accordance with vessel PMP. Keep copy of PMP on board vessel at all times and submit revised version of PMP to [name withheld]. 46 CFR 176.802

All Fiber Reinforced Plastic(FRP) vessels constructed with general purpose resins must be fitted with a smoke activated fire detection system of an approved type. Due to generator on vessel being non operational, fire detection system is non operational. AC systems that rely on generator are not operational including general alarm, fire detection, emergency lighting. Repair generator and perform function tests on all systems that run on AC prior to taking passengers. 46 CFR 177.410(c)(3)

A visual indicator must be provided at the operating station to indicate when any automatic bilge pump is operating. Mid ship port and starboard bilge pump indication not operational at operating station. Provide operational auto bilge indicators. 46 CFR 182.530(c)

All escape hatches used by means of escape must be marked on both side in clearly legible 2 inch letter "EMERGENCY EXIT, KEEP CLEAR". Interior emergency escape marking in both port and starboard state room missing. Provide proper emergency escape marking. 46 CFR 185. 606

A pre-engineered fixed gas fire extinguishing system must be installed in accordance with the manufacturers instructions. New Fire Boy bottle of 400 CUFT installed while COI states 475 CUFT and old bottle was 475 CUFT. Drawings also indicate the original bottle was 475 CUFT. Area of engine rooms could not be confirmed. Provide drawings/plans of engine room area to confirm pre-engineered system of 400 CUFT will cover space. 46 CFR 181.420

A vessel with more that one passenger deck and a vessel with overnight accommodations must have the public address system operable from the operating station. PA system not operational. Provide operational PA system. 46 CFR 184.610(c)

At each inspection the owner shall be prepared to have the vessel ready for inspection of the standing rigging in accordance with their Preventative Maintenance Plan. Furler disassembly to inspect standing rigging has not been completed in this calendar year. Make standing forestay rigging ready for inspection and provide report of condition. 46 CFR 176.802

Emergency instruction list missing. Post emergency instruction list.

Waste management plan missing. Post waste management plan.

Alcohol testing devices expired. Provide valid alcohol testing devices.

Life jacket storage marking missing. Provide marking for both child and adult storage of life jackets.

Lens/Light for 01 life float removed from service. Provide 01 new life float light with a lanyard of at least 18 feet in length.

Portable and pre-engineered fixed fire fighting have not been serviced in past year. Provide service certificate of fire fighting appliances.

Compass light not operational. Provide compass light.

Current Light list, Coast Pilot, and Tide Tables not on vessel. Provide local extracts of publications for area to be transited.

Oil change pump hot-wired onto vessel battery. Remove unapproved wiring that leads directly from battery to final load source.

Natural ventilation has mechanical closure that is not allowing for full closure. Make repairs to natural ventilation closures to ensure full closure of natural ventilation.

Running rust noted where chain plate meets port side of furler. Remove paint for further visual exam. NDT Dye-pen may need to be performed to rule out cracking based on visual inspection. Complete at upcoming dry dock.

Sail Rigging Preventative Maintenance Plan (PMP) not being utilized as designed. Daily, weekly, monthly, and semi-annual inspections not being conducted and/or logged as plan states in the past year. Provide full rigging survey at upcoming dry dock. Provide full rigging inventory of all old and new rigging components. Begin to log all inspections in accordance with vessel PMP. Complete at upcoming dry dock.

Port engine shutdown solenoid failed to secure engine. Repair/renew solenoid in accordance with 46 CFR 176.804(b)

Port and stbd forward berth overhead emergency escapes lacked sufficient means of access. Provide satisfactory ladder in accordance with 46 CFR 177.500(j). Passengers are not allowed in forward berths.

Interior side of escape hatches in forward and aft port/stbd accommodation spaces were not marked. Mark in accordance with 46 CFR 185.606.

Generator compartment Fireboy did not secure generator when tested. Prove correct operation of Fireboy in accordance with 46 CFR 181.420.

Port and stbd engine compartment ventilation closures did not close completely. Prove correct operation in accordance with 46 CFR 176.810.

Vessel has only 52 satisfactory PFDs. Limited to 52 total persons in accordance with 46CFR 180.71.

Fire detection system required by 46CFR 177.410(3) is inoperable. Rounds of spaces serviced by system must be conducted every 15 minutes while passengers are onboard.

Safety Certificate required by 47 CFR is expired. Provide current certificate onboard.
