No vessel may be navigated unless it has in its service, and onboard, the crew complement required by the COI; vessel does not currently meet the minimum manning specified on the COI. Vessel is currently short (01) 1st A/E from COI requirement. Vessel is permitted to transit from Gary, IN to Superior, WI to bring on board replacement 1st A/E.

A survey shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel. Damage to the #8 starboard ballast tank was discovered following an allision in January of 2026. 03 cracks were observed in the hull, and multiple frames on the inside of the tank were damaged and require crop/renew. Vessel is authorized to affect permanent repair to frames and temporary repairs to the hull via drill stop, gouge and weld as a temporary repair prior to departure for 2026 shipping season. Temporary repair is authorized until vessel enters dry dock in 2027 where permanent repair via crop/renew will be required.

Each lifeboat must be launched with its assigned crew and maneuvered in the water at least once every 3 months. Vessel was unable to execute drill because lifeboat was on the shore side of the vessel. Vessel is required to provide evidence of lifeboat drill within 30 days.

A survey shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel. During pressure testing of welds in cargo holds, water was observed leaking into the port side of cargo hold #1 from the adjacent ballast tank. It was discovered that a large portion of the area was substantially wasted beyond acceptable limits. Area will need to be surveyed and company will need to submit repair proposal. Vessel is authorized to temporarily leave dock to allow for the transit of another vessel and immediately return to the dock for survey and repair.

Vessel is required to have the minimum manning as specified on the COI. Vessel does not currently meet the minimum manning specified on the COI. Company is required to find a suitable replacement and notify the local OCMI when qualified crew reports for duty.

Master made notification to [name withheld], of crew shortage due to consent based discharge of AB Cepriano Matig, who departed the vessel on 12/12/2025. Vessel is authorized to operate with one less AB until arrival in Two Harbors, MN, on or about 12/15/25. If a replacement AB is not found, the Master shall contact the local OCMI to request further approval for temporary manning.

In addition to the emergency power source required under part 112 of this chapter, each self-propelled vessel must have at least two electric generating sources. Vessels reported the port generator breaker is not operating properly and will trip off without cause. Vessel to provide tech report to proper operation of breaker prior to departing the port of Duluth/Superior. 46CFR90.25-1 46CFR111.10-3

All materials to be used in any of the work specified in the various sections of this part must be free from injurious defects and must have a workmanlike finish. The construction work must be executed in a workmanlike manner with proper tools or equipment and must be free from defects which would impair strength or durability. There was a gap of about 1" between the boiler shell and the refractory near the burner nozzle on the top 1/2 to 3/4 of circle. Vessel must fill the gap per manufacture spec prior to operating the boilers. 52.01-40

The inspection shall be such as to insure that the vessel, as regards lifesaving appliances is in satisfactory condition and fit for the service for which it is intended. Unable to lower the lifeboat, or conduct a MOB drill on the vessel due to ice in the water. 46CFR91.25-10 46CFR97.15-35 46CFR199.45(b)(2)

The proper condition and operation of the survival craft and rescue boat launching appliances at loads ranging from light load to full load must be completed at every periodic and renewal inspection. The last load test was completed on 11AUG2020. Vessel is overdue for light load, and full weight test. 46CFR199.45(b)(1)

All piping, valves, and fittings shall meet the applicable requirements of subchapter F. The Fire main on deck, both port and STB side had a leak from flange fittings. 46 CFR 95.10-15(a)

Each section of firehose must be lined commercial firehose that conforms to Underwriters' Laboratories, Inc. Standard 19 or Federal Specification ZZ-H-451E. Four fire hoses were leaking and must be replaced. 46 CFR 95.10-10(n)(3)

A sufficient number of lifejackets must be carried for persons on watch and for use at remotely located survival craft stations. If watch stations, work stations, or work sites are remote from cabins, staterooms, or berthing areas and the immersion suits stowed in those locations, there must be, in addition to the immersion suits required under paragraph (a) of this section, enough immersion suits stowed at the watch stations, work stations, or work sites to equal the number of persons normally on watch in, or assigned to, those locations at any time. Vessels COI has a max of 33 persons allowed onboard, and states maximum number of lifejackets and immersion suits at 33. Immersion suits and lifejacket requirement should be 6 larger than the maximum number of crew because of watch station requirments onboard. Vessels max persons allowed reduced from 33 to 27. 46 CFR 96.06-1 46 CFR 199.273(b) 46 CFR 199.70(b)(1)(ii) 46 CFR 199.70(b)(2)(iv)

Each cargo vessel must carry an immersion suit approved under approval series 160.171 of an appropriate size for each person on board. Vessel's COI requires 33 immersion suits. 01 Immersion suit was found with a wasted air hose, and was removed from service. Vessel maximum persons allowed is reduced by 1. 46 CFR 96.06-1 46 CFR 199.273(a)

In general compliance of prime movers with the rules promulgated by ABS will be considered as satisfactory evidence of structural and operational efficiency of prime movers. Vessel reported that one of two ship service generators is inoperable due to a faulty lube oil pump. Identify cause of casualty & repair to satisfaction of ABS and attending Coast Guard marine inspector, prior to departing port of Duluth/Superior. While transiting the port of Duluth/Superior, an adequate tug escort is required until rectification of this deficiency. 46 CFR 111.10-5(a)-"Old J" AMENDED 29 May 23: In general compliance of prime movers with the rules promulgated by ABS will be considered as satisfactory evidence of structural and operational efficiency of prime movers. While transiting to St. Clair,MI utilize an adequate tug escort while in Duluth/Superior harbor and the St Clair river. Prior to departing St. Clair, MI, repair generator to satisfaction of ABS and attending Coast Guard marine inspector.

Piping whose temperature exceeds 150 degrees Fahrenheit, shall be suitably insulated. Steam piping missing insulation on forward bulkhead of engine room going to starboard heater. Insulate steam piping to the satisfaction of the attending MI prior to operation of auxiliary boilers. 46 CFR 96.03-1(a) 46 CFR 56.50-1(k)

Vessels must be equipped with independently driven fire pumps in accordance with Table 95.10-5(a). Vessel is over 1,500 gross tons and required to have a minimum of two fire pumps on board. Vessel master reported the strainer basket to their emergency fire pump froze over and cracked. Make permanent repairs to the satisfaction of attending Class (ABS) surveyor or Coast Guard marine inspector.

Vessel unable to launch lifeboat during annual fitout inspection due to hazardous ice conditions. Conduct MOB recovery drill and demonstrate launch, maneuvering, and recovery of the lifeboat. Drill to be witnessed by USCG Marine Inspector; or, provide date-stamped video of drill showing all aspects of evolution, to include operation of davit system components and on-water operations. The following staterooms had broken ceramic floor tiles, loose of broken floor tiles, rust scale in shower stalls: 3RD mate portside floor and surround in bathroom; C/E stateroom ceramic floor tile pieces around bulkhead drain piping; 3A/E shower rust scale and cracked ceramic floor tiles; S/A stateroom; Ab port aft main floor and bathroom floor chipped and cracked floor tiles; Maint/AB port fwd shower drain.

There are requirements for fluid power transmission and control systems and appurtenances for bow thruster systems. During transit, the vessel reported their bow thruster inoperable. Determine cause of failure, effect repairs and prove proper operation of the bow thruster within 30 days to the satisfaction of Class [name withheld] attesting to the bow thruster operation.

On the outboard auxiliary boiler, the control panel door's safety interlock in not operational.

On the inboard boiler, one of the fuel shutoff solenoids is missing the electrical cover.

Ventilation system in the accommodation spaces appears to be full of debris. Provide a plan to have the ventilation system professionally inspected. Provide inspection results to MSD Sturgeon Bay for further evaluation.

Fire hose on the reel behind the ECR is in need of replacing as per fire servicing company. Renew hose and reel if necessary.

Heater in stbd fwd corner of anchor windlass room is missing its cover. Renew the cover or heater before use.

Observed nylon electrical cable penetration installed in an A-0 bulkhead, located above the engine room stbd main deck door. Remove and installed an acceptable and or approved metallic cable penetration as required by 46 CFR 92.07-10 Rectify within 30 days

Observed in the main control room two monitoring cameras were not working. Repair or install an equivalent system which provides the engineering watch stander an automatic and remote control monitoring system which provides adequate means to monitor the engineering plant as required by 46 CFR 62.50-20. Rectify within 30 days

Noted several general alarms in the unloading tunnel were not working. Make repairs and demonstrate all general alarms in the unloading tunnel are working. As required per 46 CFR 113.25. Rectify before vessels departure.

Noted several emergency lights in the unloading tunnel were not illuminated, and several light fixtures missing covers. Make repairs and demonstrate all tunnel e-lights are proper illuminated and fixture covers are installed. As required by 46CFR 111.75-20. Rectify before vessels departure.

Noted emergency generator volt meter, when switched turned to B to C & A to C meter did not read proper phase voltage. Correct and demonstrate proper operation to CG [name withheld] with 30 days.

During the inspection of the accommodation spaces most of the staterooms had mold, rust, peeling paint, and in some cases broken tiles and cracked toilets. There is objective evidence that these substandard conditions are not being proactively managed. Clean mold and rust and renew broken tiles and cracked toilets.

The wipers on the bridge need renewed. Also make sure the springs and arm have enough pressure on the window to operate properly.

The navigation light indicator panel sounds a trouble alarm when the lights are working properly. Replace LED lights with incandescent bulbs to ensure proper operation of the panel.

Renew the handles on the aft anchor cats paw and stbd fwd butler plate.

Ensure proper navigation light operation is checked and logged in the official log book prior to sunset each day until the navigation light indicator panel is properly working.

Inflatable liferafts are stowed so that launching them requires them to be lifted more than 1 foot. Provide a plan to minimize the distance the life rafts need to be lifted iin order to be launched.

Observed hydraulic fluid leaking from the stbd inboard and port inboard steering gear pump discharge lines. Engineers stated they had conducted a test the day before, and no leaks were observed. Informed steering gear units were serviced during this past winter.

Temporary repairs were made to a halfmoon shaped crack, approx. 12" long, located under the port anchor pocket. Permanent repairs must be made to the satisfaction of the CG or Class at the next dry-dock, scheduled for NLT 04FEB2021.

Vessel was unable to perform full load and light load weight tests of lifeboat due to mooring arrangement with another vessel preventing lowering of boat. Perform lifeboat weight testing as required.

Both boilers (currently off-line) are due or fireside/waterside inspection and hydrostatic testing prior to bringing each boiler on-line for the season.

Fire dampers for engine room on the main deck port side and the one middle damper on the starboard side gaskets are wasted. Replace gasket material to the original state to ensure proper ventilation closer.

Make repairs to the #2 generator to ensure it can be placed in parallel mode with other generators to the satisfaction of U.S. Coast Guard Officer In Charge, Marine Inspection prior to departure from Silver Bay, MN.

Vessel was unable to lower the open Life Boat, as well as get the life boat underway during the Annual COI inspection due to location at the dock. Vessel is required to lower the life boat into the water and have crew maneuver the life boat in the water in the next 30 days.

All systems of a marine engineering nature shall be in accordance with the requirements of sub-chapter F. The vessels water cooled bearing on the port tail shaft is leaking more than expected water. Vessel secured port tail shaft in route to St. Claire. Make repairs and test to the satisfaction of U.S. Coast Guard Officer In Charge, Marine Inspection prior to departing St. Claire.

Prior to being placed back into service, Auxiliary Boiler #S-4600 must have its mounting removed for inspection, and have both a fireside and waterside inspection all conducted by a USCG Marine Inspector. Boiler is past due for these inspections.

Per D9 policy direction, USCG Marine Inspector shall witness the vessel's lifeboat launch, operate and be recovered in the water. Weather conditions prohibited this, at the time of inspection, from being safely conducted. Vesseul requested 30 day extension due to adverse weather conditions, extended additional 30 days.

Vessel may proceed to Superior, Wi under the power of 3 engines. Upon arrival effect permanent repairs to the starboard outboard quill shaft and pilot bearing. Repairs shall be completed to the satisfaction of an attending Coast Guard Marine Inspector. 46 CFR 50.05-10

Vessel original CO2 system drawings were approved with a 60 second time delay. At some point a 30 second was installed. Requirement is to replace with a 60 as approved in original drawing.

Make permanent repairs as original to damaged, inset area of bow section on centerline just below the anchor pockets, to include internal structure centerline longitudinal and transverse support shell framing.
