Whenever a vessel is deprived of the service of a member of its complement, and the master or person in charge is unable to find appropriate credentialed personnel to man the vessel, the master or person in charge may proceed on the voyage, having determined the the vessel is sufficiently manned for the voyage. The vessel is allowing the Second Mate to depart vessel for a family emergency making the vessel sailing short of a second mate. Replace the open second mate position and contact this Office.

A survey, either general or partial, according to the circumstances, shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel or or whenever any important repairs or renewals are made. The survey shall be such as to insure that the necessary repairs or renewals have been effectively made, that the material and the workmanship of such repairs or renewals are in all respects satisfactory, and that the vessel complies in all respects with the regulations. During the drydock extension survey, marine inspector found substantial damage to the frames and plating on the port stern corner of the vessel where the port steering gear flats rooms is located, which is below the vessel's waterline when loaded. Vessel is authorized to conduct temporary repairs as long as they are approved by an ABS surveyor/CG marine inspector, but shall enact permanent repairs to the satisfaction of a CG marine inspector during next drydock. 46 CFR 91.30-1 46 CFR 92.01-10

Plans and specifications for new construction and major alterations showing the respective piping systems must be submitted. Prior to installation aboard ship, diagrams of the following systems must be submitted for approval: Bilge and ballast piping, and Sanitary drains, soil drains, deck drains, and overboard discharge piping. Vessel's OWS piping system and sewage piping system does not currently match original drawings. Sewage tank is currently interconnected with bilge piping system and allows for oily waste/water to be pumped overboard completely by-passing OWS. Vessel shall submit plans to Marine Safety Center (MSC) within 90 days to ensure current arrangement is allowed by regulations and lock out any valves that interconnects both system and are not part of the approved drawings until further review has been conducted by MSC.

M/V GREAT REPUBLIC is experiencing water ingress in its #5 STBD ballast water tank. Repair prior to departing Toledo, OH.

For inspection procedures of Lifesaving appliances and arrangements, see subchapter W of this chapter. Each lifeboat and rescue boat must be plainly marked as follows:(1) Each side of each lifeboat and rescue boat bow must be marked in block capital letters and numbers with— (i) The name of the vessel;and(ii) The name of the port required to be marked on the stern of the vessel to meet the requirements of subpart 67.123 of this chapter. Rescue boat lacked name of vessel and name of port. Mark rescue boat. 46 CFR 91.25-15 46 CFR 199.176(a)

Each vessel must have posters or signs displayed in the vicinity of each survival craft and the survival craft's launching controls that— (a) Illustrate the purpose of controls; (b) Illustrate the procedures for operating the launching device; (c) Give relevant instructions or warnings; (d) Can be easily seen under emergency lighting conditions; and (e) Display symbols in accordance with IMO Resolution A.760(18). Forward liferaft found without launching instructions. Provide instructions at forward stowage location. 46 CFR 91.25-15 46 CFR 199.90

Where inflatable liferafts are substituted for lifeboats, unless freeboard at embarkation point is such that embarkation devices are not necessary, suitable arrangements shall be made for embarkation which shall include sufficient ladders or other suitable devices to facilitate embarkation into the inflatable liferafts when waterborne. Permanent railing located at the port side liferaft embarkation ladder is preventing suitable embarkation into the liftrafts. Temporary solution allows embarkation ladder to be deployed to waterline. Make permanent solution by end of next winter layup period. 46 CFR 94.50-7

Within the accommodation and service areas the following conditions shall apply: Corridor bulkheads in accommodation spaces shall be of the “A” or “B” Class intact from deck to deck. Discovered numerous service bulkheads and accommodation bulkheads with drilled holes through bulkhead. Numerous service lockers in house found with new electronic cable through bulkhead without fireblocking. Fill holes with approved fire retardent material and fireblock new cable penetrations. 46 CFR 92.07-10(d)(1)

ach general emergency alarm signal must be marked “GENERAL ALARM—WHEN EMERGENCY ALARM SIGNAL RINGS GO TO YOUR STATION” in red letters at least 1/2 inch high. General alarm singals in cargo tunnel corridor found without markings. Provide markings.

Drills must, as far as practicable, be conducted as if there were an actual emergency. Each fire drill must include— During the fire drill, Marine Inspectors witnessed crew difficulty donning fire suits and responding to fire in incinerator room. 46 CFR 97.15-35 46 CFR 199.180(c)(1) 46 CFR 199.180(f)

Abandon-ship drills must include: Lowering of at least one lifeboat after any necessary preparation for launching and Starting and operating the lifeboat engine. Due to rescue boat being pier side, a complete abandonship drill could not be conducted during the annual inspection. Conduct drill with USCG inspector. 46 CFR 199.180(d)

Wires must be in an enclosure. Multiple dead end wires found exposed in conveyor tunnel aboard vessel. 46 CFR 96.05-1 46 CFR 111.60-11

Each fixture globe, lens or diffuser must have a high strength guard or be made of high strength material. Light fixture in conveyor tunnel was missing a high strength guard. 46 CFR 96.05-1 46 CFR 111.75-20(b)

The material, design, construction, workmanship, and arrangement of main propulsion machinery and of each auxiliary, directly connected to the engine and supplied as such, must be at least equivalent to the standards established by the ABS Steel Vessel Rules. Vessel reported inoperable port main engine due to a casualty related to the turbo. Troubleshoot/Identify the cause of the casualty and propose repair plan to class and local OCMI prior to departing Duluth/Superior. Furthermore, a tug escort is required within the harbor of Duluth/Superior.

Master made notification to [name withheld], of crew shortage due to consent based discharge of an Able Bodied Seaman (AB), while in Duluth, MN. The vessel and company is unable to find a replacement crewmember. Vessel is authorized to sail with one less AB, for 14 days, and must continue to make efforts to find a replacement at each port of arrival.

Each lifeboat must be launched with its assigned operating crew aboard and maneuvered in the water at least once every 3 months during an abandon-ship drill. Vessel was unable to get the lifeboat underway during the annual inspection due to its location at the dock. Vessel is required to lower the lifeboat into the water and maneuver when able.

All emergency lights shall be marked with a letter "E" at least 1/2 inch high. Vessels emergency lights throughout the tunnel were not labeled. Label all emergency lights.

Each cable installation must meet section 25 of IEEE. Cables are to be secured with metal banding in wire runs. Vessel had new installed wires in the tunnel that were not secured in cable runs.

Instructions shall be posted in the steering engine room, relating in order, the different steps to be taken in changing to the emergency steering gear. The instructions shall indicate each clutch or pin to be “in” or “out” and each valve or switch which is to be “opened” or “closed” in shifting to any means of steering for which the vessel is equipped. Steering gear compartment did not have instructions for changing to the manual operation of emergency steering and the line diagram did not reflect their current arrangement.

The regulations of this Subpart are based on the assumption that the nature and stowage of the cargo, ballast etc., are such as will secure sufficient stability if the vessel an avoid excessive structural stress. Vessel is to locate source of water ingress between #5 Port Ballast Tank and make permanent repairs, to the satisfaction of the local OCMI or Class, prior to conducting commercial operations.

It was reported that damage and or wastage to the port kort nozzle seal is allowing water intrusion into the aft peak ballast tank. The vessel is required to conduct repairs to the satisfaction of ABS and/or the cognizant OCMI.

Vessel was unable to lower rescue boat due to mooring arrangement of vessel during the annual inspection. Vessel is required to lower and maneuver the rescue boat in the presence of a CG MI within 45 Days.

Locate source of water ingress in the #1 Port Ballast Tank and make permanent repairs, to the satisfaction of the local OCMI, prior to conducting commercial operations.

Vessel reported 5 cracks approximately 5-18 inches long on the bulkhead between the #3 and #4 ballast tanks. Vessel is to make temporary repairs in Grand Haven, MI. and then proceed to Toledo, OH. for permanent repairs.

Vessel identified ballast water migration between the #3 and #4 PORT ballast tanks. Further investigation revealed 04 separate cracks on the bulkhead between #3 and #4 tanks. Cracks were reported by ABS Class Surveyor to be any where from 10 inches to 5 feet in length. Temporary repairs are to be conducted while moored in Burns Harbor, IN. Vessel is then authorized to transit to Grand Haven, MI then proceed to Detroit, MI to make permanent repairs. Any deviation of this proposed plan is to be communicated to the nearest [name withheld] are be made to the satisfaction of the local OCMI and/or attending Marine Inspector.

It was reported that damage to the hull is allowing water intrusion into the starboard #2 ballast tank. The vessel is required to conduct repairs prior to departure from Carmeuse - Cedarville to the satisfaction of ABS and/or the cognizant OCMI.

Vessel sustained damage to Ballast tanks 4S and 5S, temporary repairs made in 4S (frames 52-54)on 01AUG21 that will be permanently repaired during lay-up in winter of 2021-22. Frames 63 and 64 in 5S will be permanently repaired prior to vessel movement.

Concrete Box Patch fitted in #1 Port Ballast Tank to bottom plate two frames inboard from the turn at frame 16. Permanent repairs to be conducted a vessels next scheduled dry-dock. 31MAR25

Vessel reported water intrusion in the number 1 port ballast tank. Upon examination, found erosion on one tab connection of a longitudinal frame to the bottom plate two frames inboard from the turn at frame 16. Vessel is capable of pumping water and maintaining 6" of water in tank. Vessel to proceed to Toledo to effect repairs. Captain verbally told to notify USCG of any changes while enroute and to monitor the tank. All repairs to be to the satisfaction of the local OCMI.

Vessel unable to lower Rescue Boat to the water and operate during COI Inspection. Vessel Must be able to Lower Rescue Boat into the water and operate while USCG Witnesses with in 30 Days.

Fire Extinguisher Bracket on the bulkhead next to the rescue boat must be replaced with the appropriate bracket for the extinguisher with in 30 Days.

A fire detection sensor tripped and caused the unintended release of C02 into the incinerator space.

A 72LB C02 bottle was inadvertently discharged into the vicinity of the incinerator. Dishcarged Bottle shall be replaced.

Did not conduct drills to maintain social distancing practice (COVID-19) during inspection. Drill to be conducted in presence of Coast Guard at a future date.

Holes drilled in to the bulkhead of the escape tunnel from the engine room with electrical wires running through the holes with no fire block or fire protection for the penetrations. Repairs to made to the bulkhead so that the penetrations meet the A class structural fire protection of the bulkhead.

Emergency generator prime mover radiator leaking from hole in the top right corner. Replace radiator in kind within 30 days.

Stbd anchor pocket sustained ice-impact damage IWO the upper outboard aft corner resulting in a split corner weld seam, and concave inset, for approx 11". Crop and renew as original. Permanent repairs to be completed prior to departure from Port of Duluth, MN.

Install alarm to signal loss of autopilot (loss of CDU test function 4.1 in DVTP). Until such alarm is installed and tested a helmsman shall remain a the helm at all times while underway. 46CFR58.25-25(c)

Make repairs and prove operation of broken fire main stop valve on D Deck.

Vessel sustained an approximate 3 to 4 inch crack in its bow hull plating, in the fore-peak, just inboard of the first starboard vertical frame and about three transverse frames up. Permanent repairs shall be made IAW ABS Class rules.

Submit PSTP's for approval to USCG Marine Safety Center in reference to newly installed SSDG's over 2017 winter lay up season.

Vessel has temporary patch on port bow. Complete final repair to the satisfaction of Marine Inspectors in Toledo, OH.

Vessel sustained hull breach and internal structure damage following an allision. Complete temp repairs to the satisfaction of attending Marine Inspector prior to transiting to Toledo for permanent repairs.

Jumper wires were installed on steering control unit circuit board by a tech rep as a temporary repair (pump #2 circuits 9 & 10). System shall be tested in accordance with approved plans and manufacturers drawings.

Make permanent repairs to fractured shell plating, frames, and strings within the #6 Port Ballast tank, in way of the bilge knuckle at frame 72. Repairs & Tests shall be verified by a CG MI per 46 CFR 91.30 & 91.45-5

Open, inspect and make permanent repairs to correct Stern Thruster oil leak. Repairs & Tests shall be verified by a CG MI per 46 CFR 91.30 & 91.45-5

Helmsman shall remain on watch at the main steering console at all times when underway until Design Verification Test Procedures (DVTPs), in regards to the additon of an auto-pilot failure alarm, are corrected to the satisfaction of the OCMI. (46 CFR 58.25-5(c))

Submit a Qualitative Failure Analysis and Design Verification Test Procedures as required by 46 CFR 62.20-1 and 62.20-3 for all microprocessor-based systems in regards to new C32 SSDG's.

Provided updated steering gear instructions on the bridge and local steering station as required by 46CFR97.37-33

Install means to energize magentic compass light as required by 33CFR164..35(b)

Replace expired hydrostatic release for EPIRB as required by 46CFR199.510(b).

Bulkheads of galleys shall be of "A" class construction. Vessels galley has a permanent vent installed on fwd bulkhead with no means of closure to maintain "A" class integrity.

Each rescue boat must be approved underal approval series 160.156 and equipped as specified in 199.175 of 46 CFR. Rescue boat does not have approved charging equipment installed.

Automation test procedures must be submitted to USCG for approval. Vessel installed new SSDG's without approved ATP's.
