Machinery and equipment installed on each vessel must be suitable for the vessel and its operation and for the purpose intended. On a vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in each of the following normally unmanned spaces ...Vessel's bilge alarms (03) were not labeled in order to properly identify. Item was corrected on the spot with label maker. 46CFR 182.530

Each distribution panel or switchboard must be totally enclosed and of the dead front type. The FWD engineroom panel was missing a blank. Vessel fabricated and installed a protecting cover for opening. Corrected on the spot. 46CFR 183.330.b.

Except as otherwise provided in this section, each space accessible to passengers or used by the crew on a regular basis, must have at least two means of escape, one of which must not be a watertight door. The fwd escape hatch for the main space was unable to be opened. Free and prove smooth and safe operation to the attending marine inspector. 46CFR 177.500.e.

Each child size life jacket must be stowed in a location that is appropriately marked and separated from adult life jackets so the child size life jackets are not mistaken for adult life jackets. Currently the child and adult PFD's are stored together in the main deck location. Redistribute PFD's to be in compliance with the above regulation. 46CFR 180.78.a.5.

Placards containing instructions for the donning and use of the life jackets aboard the vessel must be posted in conspicuous places that are regularly accessible and visible to the crew and passengers. PFD placard is flat mounted on main deck. Relocate for passenger visibility. 46CFR 185.516.a.

The following emergency broadcast instructions, when placed on a placard, will satisfy the requirement contained in §?184.506 for an emergency broadcast placard ...The vessel information and call sign shall be posted on the bridge IVO radios. 46CFR 184.510

A vessel is inspected for compliance with the standards required by this subchapter. ...The top tread for the upper deck stairwell is cracked on the underside in multiple locations, in need of repair. Provide tread that is to the satisfaction of the attending marine inspector. 46CFR 176.800.a.

On a vessel on which the number of passengers permitted on upper decks is limited by stability criteria, as indicated by the vessel's stability letter, the maximum number of passengers allowed on an upper deck must be indicated by a durable marking of at least 25 millimeters (1 inch) numbers and letters at the entranceway to that deck. The vessels signage has worn and in need of renewal. 46CFR 185.602.g.

The EPIRB test required by paragraph (a) shall be logged or otherwise documented, as applicable. Abandon ship and man overboard drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. The drill entry shall include the following information:(1) Date of the drill and training; and (2) General description of the drill scenario and training topics. Vessel did not have documented abandonship drills or EPIRB monthly tests. 46CFR 185.728.c. 46CFR 185.520.f.

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date: ...(2) U.S. Coast Pilot or similar publication; (3) Coast Guard Light List; (4) Tide tables. Vessel to provide up to date/current publications for the areas in which the vessel operates. 46CFR 184.420.a.

Flexible nonmetallic hose may be used in lube oil, fuel oil and fluid power systems, subject to the following requirements: (v) For the purpose of flexibility only, flexible hose installed in lengths of not more than 760 millimeters (30 inches) ...Vessel is able to comply with old 46CFR T on installations. Once the vessel repowered to the MTU mains (non replacement-in-kind), the vessel is required come up to the current regulations. Vessel to metallic pipe supply and return f/o lines from the f/o tank to the filter assembly/main engine. To be completed at or by next drydock to the satisfaction of the attending Marine Inspector. 46CFR 182.720

Each ring life buoy with a floating waterlight must have a lanyard ... but not more than 1,830 millimeters (6 feet). Lanyard on vessel was around 20 feet long. Vessel correct on the spot. 46CFR 180.70.d.2.

Each floating waterlight installed after March 11, 1997, on a vessel carrying only one ring buoy, must be attached to the lanyard with a corrosion-resistant clip. The clip must have a strength of at least 22.7 kilograms (50 pounds), and allow the waterlight to be quickly disconnected from the ring life buoy. Life buoy did not have a attached waterlight..

There must be a means of accurately determining the amount of fuel in each fuel tank either by sounding, through a separate sounding pipe or a fill pipe, or by an installed marine type fuel gauge. Unapproved sight glass was installed to the tank.

Potable water tank piping wasted. Make repairs in kind.

For alternating-current generators a means for measuring frequency shall be provided. The frequency meter was inoperable.

All cables installed where particularly liable to damage shall be specially protected by removable metal coverings. Cabling in engine room was attached using only plastic ties.

All electrical equipment must be constructed and protected that its uninsulated parts cannot be inadvertently touched. Switchboard was not fully enclosed.

1) Hand rail weld cracked port side forward main deck. 2) Grab rail inside house near galley weld crack in overhead.

3) Battery in Life float inoperable.

4) Fuel return line for stbd generator leak at tank on forward bulkhead.

5) Inadequate fire main pressure at pump. (30 psi).

An adult life jacket must be provided for each person carried on board a vessel. 05 kapok life jackets did not meet serviceability requirements. Reducing passenger count from 145 to 140 allowed until new life jackets are received.

Repair STBD side generator exhaust coolant water leak.

Prove proper operation of engine room escape hatch.

Clad weld marked areas local to keel in the 2nd void.

Crop and renew area in bottom plate under sewage tank in laz.

Make weld repairs to stbd rudder post IWO of attachements.
