At each inspection the hull shall be examined. Forepeak deck stbd side, first frame outboard of deck clamp wood is wrotten.

All machinery shall be checked and operational. Port generator is inoperable. 46CFR176.25-10

The condition of the vessel and equipment shall be acceptable. Port exhaust leak and protective lagging missing. 46CFR182.430

At each inspection the hull and its through fittings shall be examined. Sewage overboard discharge line leaking into the lazerrete space. 46CFR177.800

The condition of the vessel and it's equipment shall be acceptable to the [name withheld] leak identified outboard STBD engine. 46CFR176.01-10

Switchboards shall be dry and adequately protected. Breaker panel on the bridge does not lock and has an opening on the cover. 46CFR183.05-15

The inspector shall examine and test all items in the ship's outfit. Vessel's hydraulic line for the anchor has a leak in the forepeak compartment. 46CFR176.25-40

Stowing of life floats must allow easy launching. Life float painter lines were tangled and not readily accessible for inspection. 46CFR180.20-1

A ship station license is granted for operation of all maritime servieces transmitting equipment. VSL's ship station license is expired. 47CFR80.13

Oil pollution prevention placard required to be posted. Vessel missing placard. 33CFR155.450

For each marine casualty required to be reported, the owner, master or PIC of the vessel shall determine whether there is any evidence of alcohol or drug use by individuals directly involved in the incident. Vessel is required to have means to test 03 crewmembers for alcohol. Vessel's alcohol test strips are expired. 46CFR185.210

At each inspection the bilges and other spaces shall be free from excessive accumulation of oil, trash, debris, or other matter that might creae a fire hazard. Hydraulic oil accumulated under operating station where batteries, motors, and many electrical wires are located. O/O clean this space free of hydraulic fuel, remove soiled material, and have ready for inspection. 46CFR176.830

A power driven fire pump shall be installed with a minimum capacity of 50 gallons per minute at a pressure not less than 60 psi. Vessel's water intake valve frozen. O/O shall prepare fire pump, valve and piping for MI to verify proper operation. 46CFR181.10-1

All escape hatches and other emergency exits shall be marked on both sides using at least 1 inch letters: "Emergency Exit, Keep Clear". 46CFR185.30-15

All electrical installations shall be to the satisfaction of the [name withheld] acid leak in the STBD E/R battery bank, and corrosion in the Port E/R battery bank. O/O shall make E/R battery installation corrections ot the satisfaction of the attending MI. 46CFR183.01-15

All vessels shall be provided with suitable steering apparatus. Vessel's steering hydraulic line has a hydraulic leak in the lazarette space. 46CFR182.30-1

Exhaust piping shall be gas-tight throughout its length. Vessel's port engine water cooled exhaust pipe has a water leak at a coupling connection. 46CFR182.20-20

At each inspection the inspector shall examine and test all items which are required by the regulations. Vessel's sewage discharge line overboard is leaking fluid in the in the aft compartment outboard the sewage holding tank. 46CFR176.25-40

At each inspection the MI shall require that all ovserved unsafe practices and hazardous situations be corrected, and that there are no accumulation of oil that may create a fire hazard. Port generator had a fuel leak at the fuel filter. 46CFR176.25-50

Each vessel that operates on the high seas must have on board a FCC Type Category 1, 406 MHz EPIRB, installed to automatically float free and activate. Vessel's EPIRB battery and HRU unit expired. 46CFR180.64

A vessel great than 65' in length must carry a minimum of 3 24" ring life buoy with suitable grab lines and name of the vessel in capital letters. Vessel's ring life buoy mounted on the stern did not have the vessel's name. The grab line was deteriated and needs replaced. 46CFR180.70, 46CFR185.604

Placards containing instructions for donning and use of life jackets must be posted in a conspicuous place. Vessel has two types of lifejackets and currently has donning instructions for one of them. 46CFR185.516

The COI and Stability Letter shall be framed under glass and posted in a conspicuous place. MI noted vessel did not have COI nor Stability Letter posted in a conspicuous location. 46CFR176.01-40, 46CFR176.306

Rubber shaft bearings must be renewed when any water groove is found to be half the original depth. Both port and starboard shaft bearings have excessive wear and require replacement. 46CFR176.670

A hatch exposed to the weather must be watertight. Hatch leading to steering compartment missing gasket. 46CFR179.360

A power-driven fire pump shall be installed on each mechanically propelled which is permitted to carry 49 or more passengers. Vessel’s fire pump was in operable.

When operating a vessel on a body of water where the discharge of untreated sewage is prohibited by the Environmental Protection Agency under 40 CFR 140.3, the operator must secure each Type III device in a manner which prevents discharge of sewage. Acceptable methods of securing the device include -(1) Closing each valve leading to an overboard discharge and removing the handle;(2) Padlocking each valve leading to an overboard discharge in the closed position; or(3) Using a non-releasable wire-tie to hold each valve leading to an overboard discharge in the closed position. Vessel’s type III MSDS holding tank was found open. Install locking devices to prevent the discharge of sewage prior to operating within 3nm.

No major repairs or alterations affecting the safety of, the vessel with regard to the hull machinery, or equipment, shall be made without the knowledge and approval of the Officer in Charge Marine Inspection. Proposed alterations shall be approved by the Officer in Charge Marine Inspection, before work is started. Drawings, sketches or written specfications describing the alterations in detail may be required by the Officer in Charge, Marine Inspection. Vessel had a generator installed without prior approval. Submit drawings and written specifications for installation of starboard generator prior to operating starboard generator.

At each initial and subsequent inspection for certification and reinspectien the marine inspector shall examine and test the main and auxiliary steering apparatus to the extent necessary to determine that its condition is satisfactory and that it is fit for the service intended. The vessel’s starboard rudder packing leaks excessively when observed hard over.

Each vessel that operates on the high seas, or that operates beyond three miles from the coastline of the Great Lakes, must have on board a FCC Type Accepted Category 1, 406 MHz EPIRB, installed to automatically float free and activate. Vessel’s EPIRB battery and HRU are expired. Vessel limited to no more than three nautical miles from shore.

The marine employer must have a sufficient number of alcohol testing devices readily accessible onboard the vessel to determine the presence of alcohol in the system of each individual who was directly involved in the Serious Marine Incident (SMI), unless the required alcohol tests can be accomplished within 2 hours from the time of occurrence of the SMI. Vessel's COI authorizes operations on an Oceans Route and the alcohol testing devices provided are expired. Vessel is limited to a route and time where alcohol testing can be completed within 2 hours of an SMI.

No marine employer shall engage or employ any individual to serve as a crewmember unless the individual passes a chemical test for dangerous drugs for that employer. Vessel’s crew member does not have pre-employment testing results.

Electrical installations and equipment that will result in a vessel that is reasonably safe from fire caused by and/or originating in electrical equipment, and reasonably safe from eleotrical shock-of personnel. Numerous dead end wires exist on the vessel. In the third compartment dead end wires in and around the CO2 bottles, in the engine room, dead end wires above the starboard generator, block heater on port engine, and in the steering room dead end wires on the vessel’s ventilation fan. Remove all dead end wires to the satisfaction of the attending marine inspector.

The owner, charterer, master or managing operator shall instruct each crew member, upon first being employed and prior to getting underway for the first time on a particular vessel and at least once every three months, as to the duties that the crew member is expected to perform in an emergency including, but not limited to, the emergency instructions listed on the emergency instruction placard required by § 185.510 of this part and, when applicable, the duties listed in the station bill required by § 185.514 of this part. (c) Crew training shall be logged or otherwise documented for review by the Coast Guard upon request. The training entry shall include the following information. (1) Date of the training; and (2) General description of the training topics. Vessel crew was unable to produce documentation of emergency training within three months.

Machinery shall be installed in such a manner as to afford adequate protection against personnel injury and fire. The vessel’s port engine has a fuel leak and the engine’s exhaust installation wrap has some exposed/open areas. Correct leak and replace insulation prior to carrying passengers.

The master shall conduct sufficient drills and give sufficient instructions to make sure that all crew members are familiar with their duties during emergencies that necessitate abandoning ship or the recovery of persons who have fallen overboard. Vessel’s crewman had only conducted man overboard drills verbally and was unprepared to respond to a man overboard drill.

Piping for vital systems must be composed of ferrous materials. Marine Inspector observed in the tank space the most port side fix bilge pipe has a significant length of rubber hose installed beyond what is needed for flexibility. Make permanent repairs to the fixed bilge pipe with ferrous materials. 46 CFR 182.710 (c)(1)

Sea valves and strainers shall be of sufficient scope to determine that these items are in a satisfactory condition for the service intended. Marine Inspectors observed the starboard fire main uptake and generator uptake valves were difficult to operate. Make appropriate repairs. 46 CFR 176.25-5

The provisions of Part III of Title III of the Communication Act require United States vessels which transport more than six passengers for hire while such vessels are being navigated on any tidewater within the jurisdiction of the United States adjacent or contiguous to the open sea, or in the open sea to carry a radiotelephone installation complying with this subpart. Vessel's Radio Safety Certificate expired in June of 2020. Vessel is restricted to a lakes, bays, and sounds route not more than 1000 feet from shore. 46 CFR 184.502 47 CFR 80.901

Means shall be to taken ensure the vessel is reasonably safe from fire caused by and/or originating in electrical equipment and reasonably safe from electrical shock of personnel. Several dead end wires were found in the E/R and one attached to the CO2 pressure switch. Additionally, the outlet on the aft E/R bulkhead is missing a cover. 46 CFR 183.01-05

An ammeter shall be provided for measuring current of each generator. The ammeter is inoperable. 46 CFR 183.10-5(g)

Fuel supply piping shall have a minimal pipe thickness of .035 inches. The high pressure fuel delivery line had developed a crack causing fuel to discharge into the vessel bilge. 46 CFR 182.20-40

The part of the exhaust system between the point of cooling water injection and the engine manifold shall be water jacketed or adequately insulated. Insulating wrap aft of the turbo is loose or insufficiently wrapped on both engines. 46 CFR 182.15-20

A voltmeter and ammeter shall be provided for measuring voltage and current of each generator. The ammeter is inoperable. 46 CFR 183.10-5(g)

Flexible hose installed in lengths of not more than 30 inches and subject to pressures of not more than 5 psi may be used if it meets subparagraphs A,B and C of this cite. Flexible fuel return hose (Parker 801-8) isi not USCG approved hose. 46 CFR 182.720 / 46 CFR 182.115(b)

Throughhull fittings shall be in satisfactory condition for the service intended. Port rudder post packing is leaking beyond a reasonable amount. 46 CFR 176.15-5

At each inspection watertight closures in the hull shall be tested by operation. Both through hull valves for port and stbd engine's raw water cooling were seized in the open position. 46 CFR 176.25-5

At each inspection hydrostatic release units shall be checked for compliance with the servicing and usage requirements of 46 CFR 185.740. The vessel's hydrostatic release unit for the EPIRB is expired. Vessel is restricted from operating beyond three miles of shore until verified this items is corrected.

Cylinders for the CO2 fixed fire extinguishing system shall be securely fastened. The bracket for the stbd CO2 bottle is unsecured. 46 CFR 181.20-30

At each inspection electrical cable shall be examined for safe electrical condition. Exposed wires were found at the end of a conduit pipe used to support a flood light that is no longer installed. 46 CFR 176.25-15

Where passengers permitted on the upper deck is limited by stability criteria, the maximum number of passengers allowed must be indicated by a one inch in height durable marking. The marking shows 30 passengers are permitted while the stability letter addendum authorizes 25. 46 CFR 185.30-3

The part of the main engine exhaust system between the point of cooling water injection and the engine manifold shall be water-jacketed or suitably insulated. Found exhaust wrap loose and/or missing small sections on both engines. 46 CFR 182.20-15

Provide up-to-date navigation charts for area of operations.

Secure hydraulic steering lines where lying on main engine exhaust.

Seal all open penetrations in underdeck bulkheads.

Remove oil soaked absorbent pads and oily water from engine room bilges.

Secure excessive water leak at both rudder post packings.

Make high level bilge alarm system provide audible alarm.
