Shutoff valves, installed so as to close against the fuel flow, must be fitted in the fuel supply lines, one at the tank connection and one at the engine end of the fuel line to stop fuel flow when servicing accessories. The shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located, preferably from an accessible position on the weather deck. If the handle to the shutoff valve at the tank is located inside the machinery space, it must be located so that the operator does not have to reach more than 300 millimeters (12 inches) into the machinery space and the valve handle must be shielded from flames by the same material the hull is constructed of, or some noncombustible material. Electric solenoid valves must not be used, unless used in addition to the manual valve. Shut off valve was located Approx. 2 1/2' into the compartment. Install a remote pull for the emergency fuel shutoff valve.

The Coast Guard will issue a medical certificate to a mariner meeting the medical and physical standards for merchant mariners. Vessel Master could not produce the medical license or documentation had been issued. Provide proof to the satisfaction of the attending Marine Inspector. 46 CFR 10.301

A vessel's hull must not carry current as a conductor. Grounding wires were found to be corroded and disconnected from the vessel's rudder plate.

Remote fuel shutoff stations must be marked in clearly legible letters at least 1 inch high indicating purpose of the valve and direction of operation. Letters were faded to the point of illegibility.

A vessel must undergo an annual inspection within the 3 months before or after each anniversary date. Vessel continued to operate beyond its 90-day window.

Vesel's B-II fire extinguishing bracket is severly wasted. Replace with a new bracket.

Lifebuoy did not have the proper length of line for the water light. Provide the a new line that is the proper length.

Vessel does not have their child and adult lifejackets stowage properly labeled with amount of each type. Properly label the stowage compartment with the proper amount of each lifejacket.
