Every line above is a count of records the US Coast Guard wrote, or a date it recorded. A blank resolution means none was recorded in this data; it does not establish that a deficiency is outstanding. Years with a record are calendar years in which a record exists, not years in service. 15 - Safety Management Systems (ISM) › N/A - No Subsystem › Emergency preparedness
Issued 20 September 2025 No resolution recorded
The Company should identify potential emergency shipboard situations, and establish procedures to respond to them. Since the emergency diesel generator is located in the main machinery space, the SMS does not address how to respond to a loss of power in the event of a machinery space casualty (fire, flooding, etc.) that renders the emergency diesel generator inaccessible. This causes a single point of failure that presents an unacceptable risk to the crew, vessel, and the environment. An external ISM audit is required. 74 SOLAS (24) IX/3.1 ISM Code 8.1 Code 30bc
Action required: 30 - Ship detained
15 - Safety Management Systems (ISM) › N/A - No Subsystem › Emergency preparedness
Issued 20 September 2025 No resolution recorded
The Company should ensure that all personnel involved in the Company's SMS have adequate understanding of the relevant rules, regulations, codes, and guidelines. The crew did not identify any issues with the emergency source of power being located in the main machinery space nor that it is impossible to stop the passage of smoke and flame from the galley or accommodation spaces to the bridge. 74 SOLAS (24)IX/3.1 ISM Code 8.1 Code 17bc
Condition: Invalid
Action required: 17 - Rectify deficiencies prior to departure
04 - Emergency Systems › N/A - No Subsystem › Location of emergency installations
Issued 20 September 2025 No resolution recorded
The emergency source of electrical power, associated transforming equipment...and emergency switchboard should be located above the uppermost continuous deck and should be readily accessible from the open deck. PSCO observed emergency generator located in the main machinery space below the uppermost continuous deck. CCSSC (1997) 2.34 Code 17bc
Condition: Improper Installation
Action required: 17 - Rectify deficiencies prior to departure
04 - Emergency Systems › N/A - No Subsystem › Location of emergency installations
Issued 20 September 2025 No resolution recorded
The emergency source of electric power, associated transforming equipment, if any, and emergency switchboard should be located above the uppermost continuous deck and should be readily accessible from the open deck. The emergency diesel generator is located in the main machinery space below the uppermost continuous deck. CCSSC (1997) 2.34 Code 17bc
Condition: Improper Installation
Action required: 17 - Rectify deficiencies prior to departure
07 - Fire Safety › N/A - No Subsystem › Ventilation
Issued 20 September 2025 No resolution recorded
Ventilation systems of each of the following groups of spaces should be entirely separated from each other: machinery spaces; galley; cargo spaces; and accommodation spaces and the control spaces. The ventilation system for the galley is not separated from the accommodation spaces or the navigation bridge as required by CCSSC. Thus, it is not possible to prevent the passage of smoke and flame between the galley, the accommodation spaces, and the bridge. CCSSC (1997) 3.16.6.1 Code 17bc
Condition: Improper Installation
Action required: 17 - Rectify deficiencies prior to departure
07 - Fire Safety › N/A - No Subsystem › Fire doors/openings in fire-resisting divisions
Issued 20 September 2025 No resolution recorded
Subject to provisions of 3.16.1.2 and 3.16.1.3, the fire safety requirements of bulkheads, decks, doors, and stairways should, as far and as practicable and reasonable in the opinion of the Administration, comply with the applicable requirements of 1974 SOLAS Convention for such bulkheads, doors, and stairways. A solid wooden door is the only boundary separating the accommodation spaces (on deck 1), also fitted with solid wooden doors, from the stairway leading to the navigation bridge (on deck 2). No doors separate the accommodation spaces from the galley (on the main deck). This design represents an unmitigated fire risk. CCSSC (1997) 3.16.1.1 74 SOLAS Cons. II-2/2 Code 17bc
Condition: Improper Installation
Action required: 17 - Rectify deficiencies prior to departure
07 - Fire Safety › N/A - No Subsystem › Fire prevention structural integrity
Issued 20 September 2025 No resolution recorded
Paints, varnishes, and other finishes used on exposed interior surfaces should not offer an undue fire hazard in the judgment of the Administration and should not be capable of producing excessive quantities of smoke. Wood finishes are on the walls of the accommodation spaces, and the crew was unable to provide evidence that they will not pose an undue fire hazard or produce excessive quantities of smoke. CCSSC (1997) 3.16.3.2 Code 17bc
Condition: Improper Installation
Action required: 17 - Rectify deficiencies prior to departure
11 - Life Saving Appliances › N/A - No Subsystem › Stowage and provision of Lifeboats
Issued 20 September 2025 No resolution recorded

Except as provided by this section, all rescue boats shall comply with paragraphs 4.4.1 to 4.4.7.4. Each lifeboat shall be fitted with a permanently affixed approval plate endorsed by the Administration or its representative, containing at least the following items: manufacturer’s name and address; lifeboat model and serial number; month and year of manufacture; number of persons the lifeboat is approved to carry; and the approval information required under paragraph 1.2.2.9.
