If a defect is found that may affect the safety of a pressure vessel, it shall be hydrostatically tested at a pressure of 1.5 times the maximum allowable working pressure. Substantial rust was found on the side of the air receiver located in the "Puzzle Palace" space. The maximum allowable working pressure (MAWP) is also unknown because the data plate on the pressure was also ineligible. Provide U-1A form for the pressure vessel and hydrostatically test the vessel to 1.5 times the MAWP listed on the air receiver or replace air receiver that meets the requirements of 46 CFR 54.01-5.

The arrangement of the bridge lifebuoys are currently arranged that releasing them from its stowed position would allow them to strike the deck below. In addition, vessel shall ensure the lifebuoys meet requirements in LSA Code 2.1.1.7, weighing 4kg. Vessel shall make arrangements to allow the release of the bridge ring buoy to drop into the water from the deck on which it is stowed.
