Unless other means are provided to ensure that a potential waste receptacle fire would be limited to the receptacle, waste receptacles must be constructed of noncombustible materials with no openings in the sides or bottom. Plastic waste receptacle on board.

Each hull identification number must be carved, burned, stamped, embossed, molded, bonded, or otherwise permanently affixed to the boat so that alteration, removal, or replacement would be obvious. Hull identification number was posted on a removable wood plaque.

The discharge ends of fuel tank vent pipes must terminate on the hull exterior as high above the waterline as practicable and remote from any hull openings, or they must terminate in U-bends as high above the weather deck as practicable and as far as practicable from openings into any enclosed spaces. Vent pipes terminating on the hull exterior must be installed or equipped to prevent the accidental contamination of the fuel by water under normal operating conditions.

The owner, charterer, master or managing operator shall instruct each crew member, upon first being employed and prior to getting underway for the first time on a particular vessel and at least once every three months, as to the duties that the crew member is expected to perform in an emergency including, but not limited to, the emergency instructions listed on the emergency instruction placard (fire, heavy weather, or man overboard conditions) required by § 185.510 of this part. Drills are not being properly documented and crew failed to demonstrate knowledge of drill procedures.

Provisions shall be made by means of plugs, covers, ect to prevent admission of air into the space protected. Engine room ventilation cover inoperable.

Except when in the judgment of the cognizant OCMI the operation of a vessel does not present one of the hazards listed, the emergency instruction placard should contain at least the applicable portions of the “Emergency Instructions” listed in § 185.512. The emergency instructions must be designed to address the particular equipment, arrangement, and operation of each individual vessel. Emergency instruction placards do not address all requirements for vessels systems.

Each EPIRB must be tested monthly, using the integrated test circuit and output indicator, to determine that it is operative. EPIRB is not being tested monthly.

The owner, managing operator, or a qualified servicing facility as applicable must conduct the following inspections and tests on portable fire extinguishers:Monthly inspections required by NFPA 10 may be conducted by the owner, operator, person-in-charge, or a designated member of the crew. Crew is not conducting monthly inspections of fire extinguishers and is not documenting inspections as required.

A hatch cover must:(1) Have securing devices; and(2) Be attached to the hatch frame or coaming by hinges, captive chains, or other devices of substantial strength to prevent its loss. Steering space hatch not properly secured to prevent loss.

Watertight hatches must be marked in clearly legible letters at least 25 millimeters (1 inch) high: “WATERTIGHT HATCH—KEEP CLOSED”. Water tight hatches on main deck missing label.

Except on a vessel limited to daytime operations, the compass must be illuminated. Light on compass inoperative.

Each life float must meet the requirements in subpart 160.010 of this chapter - Each buoyant apparatus must have Type II retroreflective material meeting subpart 164.018 of this chapter on each side and end. Retro reflective material installed not the approved type.

When the flexible section of hose between fill and tank is a nonconductor of electricity, the metallic sections of the fill pipe separated thereby must be joined by a conductor for protection against generation of a static charge when filling with fuel. Wire joining fuel fill and tank broken.

Section 151.59 (Garbage Placards) applies to—(1) A manned U.S. ship (other than a fixed or floating drilling rig or other platform) that is 26 feet or more in length. Vessel is missing garbage placard.

Vessels subject to part III of title III of the Communications Act that operate in the waters described in § 80.901 must, at a minimum, be equipped as follows:(1) Vessels operated solely within 20 nautical miles of land must be equipped with a VHF-DSC radiotelephone installation meeting the requirements of § 80.1101(c)(2), except that a VHF radiotelephone installation without DSC capability is permitted until one year after the Coast Guard notifies the Commission that shore-based sea area A1 coverage is established. Vessels in this category must not operate more than 20 nautical miles from land. Vessels VHS-DSC is not programmed with vessel's MMSI# required by 47 CFR 80.103(b) (MMSI number programmed into DSC radios)-47 CFR 80.905(a)

A vessel must carry one floating waterlight. Each ring life buoy with a floating waterlight must have a lanyard of at least 3 feet in length, but not more than 6 feel, securing the waterlight around the body of the ring life buoy… [and] must be attached to the lanyard with a corrosion-resistant clip. 1) replace expired battery 2) place waterlight on a lanyard 3) attach the lanyard to the buoy with a clip. 46 CFR 180.70

An adult life jacket must be provided for each person carried on board a vessel. 1) obtain 10 missing life jackets 2) renew retro-reflective tape 3) renew faded labeling. 46 CFR 180.25-5

46 CFR 180.78 701c Each child size life jacket must be stowed in a location that is appropriately marked and separated from adult life jackets so the child size life jackets are not mistaken for adult life jackets. Mark lifejacket compartment and make it clear that the child size lifejackets are stowed separately. 46 CFR 180.78

Placards containing instructions for the donning and use of the life jackets aboard the vessel must be posted in conspicuous places that are regularly accessible and visible to the crew and passengers. Obtain and post placards for all types of life jackets on board (including all newly purchased life jackets).

Electrical equipment on a vessel must be installed and maintained to: Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Vessel electrical connections were exposed and subject to accidental contact and shock hazards requiring physical barrier to protect crew and equipment. 46 CFR 183.200(b)

An exposed hazard, such as gears or rotating machinery, must be properly protected by a cover, guard, or rail. Renew starboard engine belt guard. 46 CFR 177.960

Each device must have a placard suitable for posting on which is printed the operating instructions, safety precautions, and warnings pertinent to the device. The size of the letters printed on the placard must be one-eighth of an inch or larger. Obtain appropriate placarding for MSD.

Ring Buoy - provide a 60' lifeline and provide a clip for the water floatlight.

Found nuerous ground wires detached from fittings. Re-attach ground wires to appropriate grounds.

Found water egressing in way of port stern corner in lazarette. Make proper repairs to ensure water tight integrity is maintained.

Found numerous dead end wires in way of operating station. Remove all dead end wires.

A Certificate of Inspection is valid for 1 year for vessels carrying more than 12 passengers on international voyages. Vessel's COI annual exam expired on 7 Aug 2018. Vessel is limited to carrying 06 passengers until an annual inspection is completed. 46 CFR 176.107(a)
