The inspector shall examine all accessible parts of the vessel's hull, machinery, and equipment to be assured that it is in a satisfactory condition. Relief valve on hot water tank was not routed to the bilge. 46CFR56.50-25(c)

All deficiencies were to be rectified to the satisfaction of the USCG inspector and to be completed within 14 days. Deficiency #1: The deck under the Kings Room Bar is getting wasted, clean and determine the extent of rust/wastage. For all three deficiencies, determine the condition of the areas and make temp repairs for weather tightness and give a written repair proposal with time frame to make repairs.

All deficiencies were to be rectified to the satisfaction of the USCG inspector and to be completed within 14 days. Deficiency #2 Queens Room door in the "Syrup Room" at the bottom is wasted through and the deck in the room is questionable. Provide repair proposal for the wasted frame and determined the condition of the deck. For all three deficiencies, determine the condition of the areas and make temp repairs for weather tightness and give a written repair proposal with time frame to make repairs.

All deficiencies were to be rectified to the satisfaction of the USCG inspector and to be completed within 14 days. Deficiency #3: Top Deck Mardi Gras Bar outer blkhd fwd. Bottom of blkhd wasted through, provide repair proposal for this area as well. For all three deficiencies, determine the condition of the areas and make temp repairs for weather tightness and give a written repair proposal with time frame to make repairs.

17Jun25: On 16Jun25 it was reported by the Master and Chief Engineer of the vessel that the EDG Auto switch was not working properly and generator function was impaired. Make proper repairs to the unit prior to getting the vessel underway and demonstrate to the USCG when repaired.

A survey, either general or partial, according to the circumstances, shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel or the efficacy or completeness of its lifesaving appliances, fire-fighting or other equipment, or whenever any important repairs or renewals are made. The survey shall be such as to insure that the necessary repairs or renewals have been effectively made, that the material and the workmanship of such repairs or renewals are in all respects satisfactory, and that the vessel complies in all respects with the regulations in this subchapter. - 46 CFR 71.40-1(a) Vessel reported an allision with a partially submerged log that became lodged in the starboard side wheel, vessel immediately returned to home dock. Vessel must provide damage assessment and repair plan prior to movement.

§ 72.01-15 Structural standards. (a) In general, compliance with the standards established by the American Bureau of Shipping, see subpart 70.35 of this subchapter, will be considered satisfactory evidence of the structural efficiency of the vessel. However, in special cases, a detailed analysis of the entire structure or some integral part may be made by the Coast Guard to determine the structural requirements. There is possible deck wastage behind the Creole Room Bar by the void hatch. Cut away deck covering IWO the Rusted Deck. Clear all rust build up and provide a gauge report for the entire area around the hatch as discussed with the vessel master. Provide the report VIA email to [email withheld]. This is to be accomplished by 02Apr25. (Two Weeks). Possible repairs are pending the results of the gauging report.

Battery box located outside pilot house requires replacement. Currently it is holed at bottom and internally corroded excessively. 46 CFR 111.15-10(g)

#1 SSDG turbo charger oil hose and or other fittings leaking oil. Repair/replace hose and fittings on turbo charger. Complete repair(s) prior to operating the #1 SSDG. 46 CFR 61.20-3(a)

#1 SSDG F/O line leaking. Replace this F/O line, re-install lagging on turbo. Complete repair(s) prior to operating the #1 SSDG. 46 CFR 61.20-3(a)

Operating generators must provide a continuous and uninterrupted source of power for the ship's service load under normal operational conditions. Vessel's #1 generator became inoperable. Provide tech report attesting to proper operation of #1 generator to satisfaction of Marine Inspector.

Vessel is permitted one time voyage on 19SEP2024 with the following restrictions. 1. Vessel must notify VTS and confirm traffic prior to crossing river, departure and arrival. 2. Master must provide letter stating reduced power will not imped safe operation of vessel 3. Chief Engineer must provide letter stating vessel reduced propulsion speed will allow vessel to operate propulsion, vital systems, and hotel services on a single generator.

Each temporary source of emergency power must consist of a storage battery of sufficient capacity to supply the temporary emergency loads for not less than one-half hour. During loss of power, the public address system's battery backup was only able to provide 1 minute of use. Uninterpretable power supply (UPS) was showing a fault light. Repair or replace UPS with adequate battery life prior to departure. 46 CFR 112.37 (1982 Amended)

AIS must be maintained in effective operating condition. AIS was found with intermittent power loss and flickering. Troubleshoot and make appropriate repairs prior to departure. 33 CFR 164.46(d)(2)

Overcurrent protective devices must be installed so that a short-circuit on a circuit that is not vital to the propulsion, control, or safety of the vessel does not trip equipment that is vital. Electrical components under pilothouse console was found with multiple daisy-chained power strips and makeshift power cable tied into back of receptacle outlet. Furthermore, these powerstips were providing power to AIS, Throttle lights and miscalaneous non vital components such as microwave. Remove all unnecessary components such as daisy-chained power strips. Re-wire or move location of AIS and throttle control lights to a vital powers source. Survey all electrical components and breakers for overloaded circuits. Remove all stored items such as cleaning supplies from console.

Doors, other than those which are normally locked shall be of self closing type capable of closing against a 3.5 degree list. Generator room port and STBD escape trunks were found with missing self closing mechanism as per fire plan. Re-install self closing mechanisms.

Except as provided in paragraphs (b) through (d) of this section, this subpart applies to vessels with watertight doors in bulkheads that have been made watertight to comply with the flooding or damage stability regulations in this subchapter. Aft port steering pump room water tight door was found with hinge bushing failure and dogs unable to operate without the use of pipe. Top left dog was found loose and not making proper contact with seat. Make appropriate repairs to door.

Each general emergency alarm signal must be an electrically-operated bell, klaxon, or other warning device capable of producing a signal or tone distinct from any other audible signal on the vessel. General alarm bell on the identified port side bow of super structure was not working as designed.

A receptacle outlet must not have any exposed live parts with the plug opening uncovered. MI discovered an outlet missing a cover in the vessel’s lower galley. Install appropriate cover on the outlet and submit objective evidence to the satisfaction of the Marine Inspector.

Electric installations on vessels must ensure: Protection of passengers, crew other persons, and the vessel from electrical hazards. MI found an open space with a missing blanking plate in the oven circuit panel.

Each passenger vessel of less than 500 tons gross tonnage must carry at least one rescue boat approved under approval series 160.156. MI discover cracking/ splitting on a transom support bracket.

Double doors capable of independent operation and latching may have a clearance between the doors not to exceed 1/8 inch. However, if one door must always be closed first, a doorstop of at least 1/2 inch shall be provided for the second door. MI found fire doors with out the center latch mullion post installed.

Each item of lifesaving equipment carried on board the vessel in addition to those required by this part must— (2) Be accepted by the cognizant OCMI for use on the vessel. MI discovered adult life jacket location signs were blocked from the field of view.

835 issued: Replace broken window in the passenger compartment on the main deck Port side last window in the passenger seating area. Until repaired, mark off the area to prevent passengers from using the seating at the window.

The vsl shall have a satisfactory bilge pumping plant capable of pumping from and draining any watertight compartment. The vsl's [name withheld] were not sure which valve needed to be opened to dewater the forepeak. Vsl shall post a bilge diagram & label valves below the deckplates with tags or stenciling above the deckplates to show their location.

Pressure Vessels shall have detailed plans that include the information required by 46 CFR 54.01-18, the plans shall be certified by a registered P.E. to meet the design requirements of 46 CFR 54.01-5(d),(e) & ASME VIII. The vsl shall submit the plans to the local OCMI for review & approval.

Vsl to make repairs and/or provide for examination to the items as noted on the work list issued to the Master on 23 Jan 23.

The data shall be stamped directly on the pressure vessel. The data shall be legibly stamped and shall not be obliterated during the service life of the pressure vessel. This plate shall be securely attached to the pressure vessel. The nameplate shall be maintained in a legible condition such that it may be easily read. The data plate on the pressure vessel on the upper deck is no longer legible, replace pressure vessel during next dry-docking in January 2023. 46 CFR 54.10-20(c)

Whenever doubt exists as to the safety of an existing boiler, pressure vessel, or piping system, the marine inspector may require that it be gauged or checked to determine the extent of deterioration, and if necessary for safety may require the recalculation and reduction of the maximum allowable working pressure. The 02 pressure vessel in the bow thruster room were found with questionable corrosion, provide U1A form with specifications for corrosion allowance an conduct gauging of pressure vessel during next dry-docking in January 2023. 46 CFR 50.05-5(a)

During the test of the E-Gen shore power was not disconnected and the MI observed flashing and knocking at the E-Gen panel. Ch. Eng stated this was due to the shore power not being disconnected at the time of the switchover and was a result of residual power in line. Test was re-done successfully in which the shore line was disconnected and done on ship power only. Issued deficiency for an electrician to verify system is installed as originally designed and that there is no faulty equipment in system.

Vessel does not have an acknowledgment of enrollment of an ASP, as prescribed in 33 CFR 104.

vsl reported loss of propulsion.

46CFR71.65-5(d) (1 thru 7) Fire control. (1) Fire control diagram showing location and type of all required fire-screen insulation, including main fire zone and subdivisions, stairway and elevator enclosures, control space enclosures, etc., and type of all doors in such subdivisions and enclosures. (2) Comprehensive typical details of fire-screen insulation of both vertical and horizontal surfaces, including deck coverings where used, keyed by reference numbers to the “fire control diagram”. (3) Ventilation diagram including dampers and other fire control features. (4) Alarm systems. (5) Detecting systems. (6) Extinguishing systems, including fire main, carbon dioxide, clean agent, foam, and sprinkling systems. (7) Supervised Patrol Route. The vessels current fire control plan is an older issue in black and white with outdated symbols and missing important information such as deck and blkhd classing. Time has come to update the plan to include the new symbols and what is missing from this original plan IAW the above CFR requirement. To be Rectified prior to the next quarterly inspection. 33CFR164.35 Equipment on all vessels: 33CFR164.35(f)An illuminated rudder angle indicator in the wheelhouse. The rudder angle indicator located in the pilot house (2each) and the one located in the Engine room Control station are not reading the same rudder angle. At times the one in the pilot house constantly moves approx 5 to 7 degrees back and forth for a while. Correct the deficiencies between all indicators and determine the cause for the fluctuation in the pilot house that stops after a few minutes. This should be corrected within the next three days by 18Mar2022.

Firemain systems installed IAW 46CFR76.10 must be maintained in good working order. Currently several fire hydrant valves are leaking by excessively and on valve could not be opened. Make proper repairs and or replacements to these fire hydrant valves and demonstrate to the satisfaction of the USCG Marine Inspector. Prior to carrying and Passengers.

Diesel Fuel systems installed IAW 46CFR56.50-75 shall be maintained so that no leaks are present in the systems. Currently there are two Fuel lines leaking, one on the number 1 Main Generator and one on the number 2 generator. Replace these leaking lines and demonstrate to the USCG inspector the satisfactory operation of these engines. Prior to carrying passengers.

General. Each vessel must carry a life jacket for each person on board. While onboard, MIs condemned 41 adult lifejackets and 9 child lifejackets. Prove that the total required amount of lifejackets is onboard, prior to carrying more than 785 adults and 78 children as passengers.

The method of donning lifejackets. Replace life jacket donning instructions on the 2nd deck.

Each hand portable fire extinguisher shall be marked with a number and the location where stowed shall be marked with a corresponding number at least 1/2 inch high. Where only one type and size of hand portable fire extinguisher is carried, the numbering may be omitted. Several fire extinguishers are missing. Replace all fire extinguishers to their intended location.

Where extinguishing systems or equipment are not required, but are installed, the system or equipment and its installation must meet the requirements of this part. Remove or recharge the fire extinguisher on the main deck p/s adjacent to the midships bar.

Marine charts of the area to be transited, published by the National Ocean Service, U.S. Army Corps of Engineers, or a river authority that - (i) Are of a large enough scale and have enough detail to make safe navigation of the area possible; and (ii) Are currently corrected. Provide corrected charts for the area normally transited or prove current arrangement meets specification.

Safety or relief valves on pressure vessels: If your vessel's COI is renewed annually, the marine inspector must check the settings of the safety or relief valves on all pressure vessels, except cargo tanks, at each inspection for certification. Chief Engineer stated that relief valves have not been tested as per the regulation. Provide documentation attesting to the proper function of the relief valves or renew.

Test procedures documents must be in a step-by step or checkoff list format. Each test instructions must specify equipment status, apparatus necessary to perform the tests, safety precautions, safety control and alarm sets points, the procedure to be followed, and the expected test results. Chief Engineer conducted Jacket-water and lube-oil high temp alarms using a fry-daddy. Provide proper testing equipment as per manufacture recommendations.

Means shall be provided for stopping all fans in ventilation systems serving machinery and cargo spaces and for closing all doorways, ventilators and annular spaces around funnels and other openings to such spaces, from outside these spaces, in case of fire. Both port and starboard engine room ventilation closures were found missing gaskets. Replace all missing gaskets prior to departure.

Each Class 1 door must have a quick action closing device operative from both sides of the door. Bow thruster door handle was found severely loose, causing concern of falling off. CE stated that they may be able to tighten the handle. However it appears to have a shaft key type set up that may be causing the problem as well. Make appropriate repairs IAW manufacturers specifications.

Exhaust pipe installations must conform to the requirements of ABYC P-1 and part 1 of section 23 of NFPA 302 (both incorporated by reference). #3 generator turbo lagging was not designed for specific application on the turbo, exposeing many areas of turbo and exhaust. Install lagging to adequately cover turbo and exhaust such as the #1 and #2 lagging.

Fire Hose stowage bracket is mounted to the bow thruster door and was found with no adequate means of holding the fire nozzle. Because of this, the fire nozzle was found laying on the deck creating both a tripping hazard and possible future failure of nozzle assembly. Furthermore the spanner wrench was found sitting on an overhead beam creating a hazard to crew and possible loss of wrench falling into bilge. Create a bracket to hold both nozzle and spanner wrench.

Anchor line at anchor chain connection is growing green grass, cut back anchor line and re-splice line to the chain prior to getting underway.

2)Replace 06 Adult life jackets prior to carriage of full complement of passengers, currently only allowed (Non COVID Rules) 820 until new life jackets placed on board.

3)Gangway shackles loose and spins when gangway is moved by wave action. Properly fasten pin and shackle prior to getting underway.

4)Install an emergency light in the engineer’s workshop aft of the ECR room currently none in there.

5)During the test of the fire hoses one failed. All hoses were tested and found that only one failed, replace the failed hose. Prior to getting underway.

Vessel reported a loss of starboard drive to means of propulsion (paddle wheel) and moored safely at Poydras Dock. Vessel is not permitted to move until starboard drive and all associated equipment is inspected and tested to the satisfaction of a CG marine inspector, and a technicians report is provided detailing the cause of the casualty. CITE: 46 CFR 71.40-1

Currently the STBD SS/DG has a serious F/O leak at the H/P F/O pump distribution block. Repair all F/O leaks prior to using this SS/DG.

Currently the Center SS/DG has serious F/O leakage at the H/P F/O pump distribution block and at the #4 and #6 Cylinder head F/O line connections. Repair all F/O leaks prior to using this engine.

Currently the Port SS/DG has F/O lines that have worn rub spots significantly, Replace all worn F/O lines prior to using this engine.

A power-driven vessel of less than 12 meters in length may, in lieu of the lights prescribed in paragraph (a) of this Rule, exhibit an all-around white light and sidelights. Rescue boat under 12 meters on board was found with no navigation lights of any kind.

Grab line around life floats is deteriorated. Replace with new line.

47 adult and 6 child life jackets are deteriorated. Replace with new ones.

Maintain main deck galley fire doors free from obstructions.

Fuel fill coffer dams missing plugs. Provide appropriate plugs to prevent fuel discharge.

Vent closure sensor on main deck engine intake vents not properly installed. Install sensor correctly.

Emergency remote fuel shutoff valves (Port and Starboard) not properly marked and missing the tools required to access and operate them. Provide proper markings and tool for access and operation.

Readjust watertight door to make water tight. Make available a cheater bar to tighten dogs. Pipe was missing from door.

Renew missing door handle from fire screen door on the aft exit from the 02 caterers locker.

The 2nd deck bar has excessive water building up on the deck, determine the source of the water and correct problem.

Stairway both decks the bottom "Ginger Bread" railing missing, whats left is and HAZD bracket, remove HAZD or repalce "Ginger Bread" railing section.

The 2nd Deck aft bar electrical wires are all taped up, determine why they are and make repairs.

There are dead end wires aft 2nd deck bar in cabinet, properly dead end wires.

On the aft bar 2nd deck the sink that is mounted to the ice machine is not mounted properly and is a hazd. Properly repair the sink mounting.

The room that is next to fire station number 7, there are dead end cables located in that room, properly dead end these cables in junction boxes.

In the store room next to fire station number 7 there is missing fire protection insulation on the blkhd, replace missing insulation.

In the 2nd deck galley stbd side behind refer machinery, a dead end cable is sitting on the deck in water. Properly remove the wire or dead end in junction box.

Fire damper fwd second deck stbd side in cabinet is tight when operated, free up the damper to operate properly.

Exterior doors missing door handles, they were removed for painting and have not been installed, missing screws to install them.

The ventilation openings above the CO2 protected bow thruster room can not be closed. Examine and make repairs so dampers be closed, 03 each one Plrt and 1 stbd, then 1 fwd.

Main deck port side fwd cabinet two electrical boxes (LIVE) not mounted and one with live wire pulled out of the box. Repair same and properly mount the boxes.

Labels identifying # of lifejackets are missing from starboard side 02 deck.

Determine where insulation and fire protection is required in the liquor room.

Remove dead end wiring behind transformer behind lower deck kitchen. Make transformer termainal connections available for inspection.

Fire dampers in steering room, fwd Creole kitchen (lower deck), port top inoperable, also near fire station #7. (As identified to ship's crew.) Make fire dampers operable.

Stuffing tubes leading from SCR to generator room (port side of door) are not being kept up properly. Has dead end wires and holes where wires were removed. Clean up, remove wires, and make vapor tight.

Door missing from breaker box in tool room. Replace.

Area behind panels in SCR being used as storage. Remove all unneccessary equipment from area.

Life ring stowed outside of EDG is not marked with vessel name. Mark with vessel name.

Housekeeping in tool room is inadequate: items being stowed on top of transformers & excessive amount of flammable liquids stowed inside space. Clear off transformer and remove any flammable liquid in excess of what is needed for immediate use.

Overhead light covers in tool room & SCR are in poor condition (fractured) & secured with zip ties. Replace.

Replace primary stern light.

Label all circuit breaker panels on bridge for ship's equipment.

Dead end wire on wheelhouse deck aft must be removed, tagged out, or connected properly.

Replace #3 generator fuel gauge.

Replace #3 generator expansion joint as shown to Chief Engineer.

Install caps/plugs on all generator low lube oil testing valves.

Replace zip ties in overhead wire runs located throughout the engine room.

1. In the [name withheld] upper deck, FWD BLKHD on the deck, there are two 1.5" holes that have been recently cut in the deck to pass new audio and visual cable through. These are improper penetrations in the deck. Properly install wires through approved through BLKHD penetrations. Compliance 27Dec2017.

2. [name withheld] upper deck, FWD BLKHD where a cut out in the BLKHD was found where there are questionable wire repairs done. Remove electrical tape and determine the type repair completed and make proper approved repairs to these wires. Compliance date 27Dec2017.

3. Upper deck OVHD AFT on the open deck, there are permanent extension cords installed. Remove same and install proper wire and outlets. Compliance date 27Dec2017.

4. Queen Room Galley, there is an extension cord being used as permanent wire. Install proper outlet on the BLKHD where needed for the portable oven and remove the extension cord. Compliance date 27Dec2017.

Replace wiring cable to govenor on #1, #2 and #3 generator.

To Be completed prior to 31 Oct 2014: After the installation of the new generator automation monitor and alarm system, the current automation procedures new require updating. Provide updated automation procedures for the newly installed monitoring system for the generators and bilge alarms.

1) Bowthruster room requires a auto discharge although 46CFR76.05-1(a) states that the room does not require a CO2 system, 46CFR76.15-(a)(b) says if installed with CO2 bottles in the space protected it must have auto discharge. Provide proposal to instal auto discharge. Extended until 17Aug2014. Vessel lay up period.

2) Conduct engineering auto test procedures IAW the on board procedures.

3) The Alarm master box for the #2 generator circuit system is fried. Currently #2 generator circuit is wired in the spare circuit system. Provide proposal to fix/repair/replace the alarm panel in the SCR room. Extended from 26June 2014 until 17Aug2014. Fixed during their lay up period. T. Alho

4) Provide proposal that will keep the Creole Room Galley panel box stbd side oven disconnect from constantly being blocked by carts. Serious safety hazard.

6. To be completed prior to 17Aug2014: #2 generator high lube oil temp alarm does not work on alarm panel, Repair/Replace IAW item #4 issued on 16Jun2014.

7. To be completed prior to 17Aug2014: Bilge alarm for the SCR and the engine room will not alarm on the SCR alarm panel or in the pilot house as designed. Repair/Replace IAW item #4 issued on 16Jun2014. Removed the extra rounds of spaces requirement for bilge monitoring due to a temp. alarm box installed for the failed bilge alarms.

Prove proper operation of the steering alarm panel. If operating on #1 steering pump, provide live watch in the steering room.

Prove pooper operation of the bow thruster bilge alarm (Visual indicator is operational but not the audible alarm.)

Provide blank for port side electrical panel in galley, repair/replace wasted panel door.

Renew bow thruster room WTD gasket.

Adjust aft control room WTD.

Stencil Ax on bow and bow thruster room.

Under all bars replace all outlets with weather-tight fixtures. Current fixtures are corroded and pose a possible fire hazard.

Provide structural fire protection plans to OCMI, to determine where/what fire doors are required as per 46CFR72.05-25(g).

Replace sealant around piping through fire boundary on 02 deck between caterers locker and dining area above the drop ceiling.

Make repairs to steering system prior to carrying passengers

Replace 179 life jackets found to be severely degraded. Until such time total persons onboard shall not exceed 821 persons.

Renew rescue boat falls and weight test to 110% of full weight. New falls to have a 6:1 safety ratio for full weight. 46CFR199.190(j): 199.153(c). Weight test to be conducted in the presence of USCG inspector.

Emergency generator will not automatic start and take the load. Prior to carrying passengers. Prove proper operation of the emergency generator.

Replace missing A-60 fire protection in overhead in emergency generator space.

Forward engineroom water tight door (WTD) alarm indicator is not illuminating on the bridge when the WTD is open. Prove proper operation of WTD alarm.

The #4 and #5 high pressure fuel lines are rubbing together because of improper installation and a missing bracket, which keep the fuel lines from coming in contact with each other. Replace the #4 & #5 fuel lines and properly install with the correct bracket.

Vessel must provide 25 lifejackets. Vessel may not operate carrying more that 975 total persons until the 25 missing/ condemend lifejackets have been replaced.

Rudder indicators in the SCR room and Pilot house are reading 5 deg. different at max right rudder. Calibrate both rudder indicators.
