On a vessel of at least 26 ft in length, a visual and audible alarm must be provided at the operating station to indicate high water level. Newly installed high bilge alarm did not visually activate. 46 CFR 182.530

For fixed gas fire extinguishing systems, the inspections and tests required by Table 176.810(b) of this section...the operator must provide satisfactory evidence of the required servicing to the marine inspector. Servicing report for fixed system not available for inspection. 46 CFR 176.810

Each ring life buoy must be marked with type II retro reflective material. The arrangement of the material applied after March 11, 1996 must be as specified by IMO Resolution A.658(16). Retro materials of a sufficient width (approximately 5 cm) should be applied around or on both sides of the body of the lifebuoy at four evenly spaced points. Onboard life ring retro reflective material not marked in accordance with the requirements. 46 CFR 185.604

An adult life jacket must be provided for each person carried onboard a vessel. Two life jackets found unserviceable and taken out of service. Vessel restricted to 20 passengers until lifejackets are replaced. 46 CFR 180.71

Each survival craft must be: secured to the vessel by a painter with a float free link permanently attached to the vessel...stowed so that when the vessel sinks the survival craft floats free and, if inflatable, inflates automatically. Three IBA's stowed onboard in a manner that will not allow automatic deployment and release in the event of sinking. 46 CFR 180.130

There must be an individual holding an appropriate license as or a valid MMC with endorsement as master in command of each of the following vessels:(2) Every self-propelled inspected vessel. Vessel master was found with an expired medical readiness card. Vessel master must obtain an updated medical readiness card to the satisfaction of the Marine Inspector. 46CFR15.805(a)(2)

A vessel not required to have a power driven fire pump by § 181.300 must have at least three 9.5 liter (21/2 gallon) buckets, with an attached lanyard satisfactory to the cognizant OCMI, placed so as to be easily available during an emergency. The words “FIRE BUCKET” must be stenciled in a contrasting color on each bucket. Vessel was missing proper markings and attached lanyard on fire buckets. Install proper lanyards and markings to the satisfaction of the Marine Inspector. 46CFR181.610

Lakes, bays, and sounds, and rivers routes. A vessel on a lakes, bays, and sounds, or rivers route must carry:(1) Three hand red flare distress signals approved in accordance with § 160.021 in subchapter Q of this chapter, or other standard specified by the Commandant; and(2) Three hand orange smoke distress signals approved in accordance with § 160.037 in subchapter Q of this chapter, or other standard specified by the Commandant. Vessel's distress flares were found expired. Replace all distress flares to the satisfaction of the Marine Inspector. 46CFR180.68(b)

The owner, charterer, master or managing operator shall instruct each crew member, upon first being employed and prior to getting underway for the first time on a particular vessel and at least once every three months, as to the duties that the crew member is expected to perform in an emergency including, but not limited to, the emergency instructions listed on the emergency instruction placard required by § 185.510 of this part and, when applicable, the duties listed in the station bill required by § 185.514 of this part. Vessel has no records of training for crew members. Establish a training template for all new and current crew members to the satisfaction of the Marine inspector. 46CFR185.420(a)

Abandon ship and man overboard drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. The drill entry shall include the following information:(1) Date of the drill and training; and(2) General description of the drill scenario and training topics. Vessel was found with no abandon ship and man overboard drills. Conduct and log drills to the satisfaction of the Marine Inspector. 46CFR185.520(f)

Fire fighting drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. The drill entry shall include the following information:(1) Date of the drill and training; and(2) General description of the drill scenario and training topics. Vessel was found with no fire drills. Conduct and log drills to the satisfaction of the Marine Inspector. 46CFR185.424(d)

Each item of lifesaving equipment determined by the marine inspector to not be in serviceable condition must be repaired or replaced. 6 kapok lifejackets were found to be not servicable and need to be replaced. Vessels max allowable people on board is reduced to 19 until lifejackets are replaced.

A visual indicator must be provided at the operating station to indicate when any automatic bilge pump is operating. There is no visual indicator at the forward inside operating station to indicate when the bilge pump is operating.

A vessel must carry either a first aid kit approved under series 160.041 or a kit with equivalent contents and instructions. Vessel had several expired items in first aid kit and shall renew expired items.

All applicants for original officer endorsement must present to the Coast Guard a first aid and safety course. Master did not have first aid or CPR. 46 CFR 11.201

Marine employers shall establish programs for the chemical testing for dangerous drugs on a random basis of crewmembers on inspected vessels. Vessel has consortium but did not have list of crewmembers from the consortium that are enrolled in the program or the latest MIS report.

An enclosed space containing machinery powered by gasoline must be equipped with a flammable vapor detection device. Vessel did not have a flammable vapor detection device in the forward fuel tank space. Require installation of flammable vapor dection device in forward fuel tank space during next dry dock. 46CFR182.410(c)

At each initial and subsequent inspection for certification, the marine inspector shall examine all accessible parts of the exterior and interior of the hull. Starboard side forward of the forward operating station the inside planking is broken and the exterior planking is water damaged. Require repairs be made during next dry dock. 46CFR176.25-5(a)(1)[Old T]

A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. First aid kit did not contain Aspirin or Ammonia inhalants. 46CFR184.710 code 16(c)

The employer shall provide an Employee Assistance Program (EAP) for all crewmembers. The employer may establish the EAP as a part of its internal personnel services or the employer may contract with an entity that will provide EAP services to a crewmember. Drug and alcohol program for vessel does not include company policy with EAP. 46CFR16.401 code 16c

Exhaust Piping non metalic hose is not rated for the operating temperatures produced by the engines. Hose smokes while underway due to excessive heat. 46 CFR 182.200

Submersible Electric Bilge Pumps are not UL Rated. 46 CFR 182.520 (e)(1).

Forward Bilge Wiring was found coiled with several loops and laying in bilge. 46 CFR 183.340

Shafting from Engines were exposed and did not contain covers or guards around rotating machinery. 46 CFR 177.35-15

Currently the boat is outfitted as an open hull (no watertight integrity), but there is no documentation attesting to how the stability test was completed. This can be addressed in 3 ways: 1. Provide documentation from the stability test showing the test was conducted under the constrains of an open hull, 2. Make all hatches/access points into the hull watertight, or 3. Conduct a new stability test classified as an open boat. Please notify Sector San Francisco of your intended option.

Vessel has expired Annual Inspection; vessel must satisfactorily complete Annual Inspection prior to carrying passengers. - 46 CFR 176.500(b)(1).

Install IBA to be in a float free arrangement.

Pulled and inspected 4 keel bolts to include #2,#4,#10,#14, All four had cracked lower fasteners, owner to make remainder of keel bolts ready for inspection prior to credit dry dock.

Properly dead end or removed identified electrical cable IAW 46 CFR 183

Repair leak on the port side heat exchanger.

Vessel shall not operate with passengers until an underway evaluation of hull repairs is conducted by attending marine inspector.

Update first aid kit.

Remove type II child PFD's from vessel.

POST AND MAKE VISIBLE TO PASSENGERS: COI, STABILITY LETTER, COAST GUARD SAFETY DECAL, EMERGENCY CHECKOFF LIST, PFD DONNING PLACARD.

INSTALL SECONDARY BILGE HIGH LEVEL ALARM AT AFT OPERATOR'S STATION.

PROVIDE "EMERGENCY EXIT - KEEP CLEAR" PLACARD IN WAY OF FORWARD ESCAPE HATCH.

A SEA TRIAL MUST BE CONDUCTED TO PROVE THE WATERTIGHT INTEGRITY OF THE HULL. THE SEA TRIAL MUST BE TO THE SATISFACTION OF THE ATTENDING CG MARINE INSPECTOR.

PROVIDE RUDDERS INSTALLED & READY FOR INSPECTION.

PROVIDE VESSEL FOR INSPECTION AFTER SUFFICIENT AMOUNT OF TIME TO ALLOW FOR SWELLING OF PLANKS.

PROVIDE PROPELLERS INSTALLED & READY FOR INSPECTIOIN.

REPLACE CUTLASS BEARINGS.

PROVIDE MSD THAT DOES NOT DISCHARGE OVERBOARD. CURRENT UNIT, LECTRA SAN 159.15/100/3/I IS A TYPE I. IN ADDITION, GREY WATER FROM SINK DRAINS SHOULD NOT GO OVERBOARD. MSD SHOULD BE A TYPE III - HOLDING TANK TYPE. SECUREPRESENT UNIT.

REPAIR/REPLACE 4 LEAKING BUTT BLOCKS. I PORT SIDE, FORWARD OF ENGINE AT "O" OF OFFICIAL NUMBER, 1 STBD SIDE AT AFT END OF SETTEE, AND 2 ,PORT AND STBD, AT FWD STEERING STATION.

SHOW THAT HALON SYSTEM INSTALLED IN ENGINEROOM IS CG APPROVED. HALON SYSTEM WAS INSTALLED IN LIEU OF CO2 SYSTEM YEARS AGO. SCHEMATIC DIAGRAM OF SYSTEM SHOULD BE SUBMITTED FOR APPROVAL.

REPAIR RETRO-REFLECTIVE TAPE ON 4 ADULT AND 2 CHILD LIFEJACKETS. VESSEL IS RESTRICTED TO 20 PASSENGERS TOTAL, UNTIL THIS ITEM IS COMPLETED.
