All vessels must have navigation lights that are in compliance with the applicable sections of the International and Inland Navigation Rules. MI observed the anchor light to be inoperable and the degree of coverage for the stern light to be in excess of the 135° coverage requirement. Vessel is restricted to daylight and fair weather operations. Cite: 46 CFR 183.420

On a vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in each of the following normally unmanned spaces:(1) A space with a through-hull fitting below the deepest load waterline, such as a lazarette;(2) A machinery space bilge, bilge well, shaft alley bilge, or other spaces subject to flooding from sea water piping within the space; and (3) A space with a non-watertight closure, such as a space with a non-watertight hatch on the main deck. MI observed the audible alarms for all spaces to be inoperable. Hourly checks of the affected spaces are to be conducted while underway. Cite: 46 CFR 182.530(a)

A visual indicator must be provided at the operating station to indicate when any automatic bilge pump is operating. MI observed the required visual indicator in the aft compartment to be inoperable. Hourly checks of the affected space is to be conducted while underway. Cite: 46 CFR 182.530(c)

Each launching appliance and each survival craft and rescue boat on a vessel must be in good working order and ready for immediate use before the vessel leaves port and at all times when the vessel is underway. MI observed the 8-man life float's safety net to be deteriorated and missing a required paddle. Cite: 46 CFR 185.700(a)

A vessel must have one or more ring life buoys as follows:(2) A vessel of more than 7.9 meters (26 feet) in length, but not more than 19.8 meters (65 feet), must carry a minimum of one life buoy of not less than 610 millimeters (24 inches) in diameter. MI observed the ring life buoy's grabline to be deteriorated.

Each launching appliance and each survival craft and rescue boat on a vessel must be in good working order and ready for immediate use before the vessel leaves port and at all times when the vessel is underway. MI observed the 25 person life float to be severely deteriorated and the 8 person life float to have wasted grab lines. Restricted route to Lakes, Bays, and Sounds until corrected, however, once the grab line is replaced on the 8 person life float, the vessel may go on limited coastwise trips with a total personnel allowance of 16.

A vessel on an oceans, coastwise, limited coastwise, or Great Lakes route must carry six hand red flare distress signals approved in accordance with § 160.021 in subchapter Q of this chapter, or other standard specified by the Commandant, including, but not limited to, approval series 160.121. MI observed only three hand red flares on board. Restricted route to Lakes, Bays, and Sounds until corrected.

Aft bilge visual indicator is inoperable. Forward bilge alarm and indicator inoperable. Repair bilge alarms and visual indicators.

A vessel must be provided with bilge pumps in accordance with Table 182.520(a). A second power pump is an acceptable alternative to a hand pump if it is supplied by a source of power independent of the first power bilge pump. Individual power pumps used for separate spaces are to be controlled from a central control point and must have a light or other visual means at the control point to indicate operation. Visual bilge pump indicators are inoperable. Repair light indicating operation of bilge pumps.

Except as noted in § 176.114 of this part, each vessel inspected and certificated under the provisions of this subchapter must, when any passengers are aboard during the tenure of the certificate, be in full compliance with the terms of the certificate. Vessel is certified to carry a total of 52 people. Vessel only had 30 lifejackets onboard and a life float capacity of 30. Vessel restricted to carrying 30 total people onboard.

On a nonmetallic vessel, the enclosures and frames of electrical equipment must be bonded to either a common ground by a normally non-current carrying conductor. During inspection, marine inspector found broken/ loose bonding cables in engine compartment (stbd side) and on rudder shaft (port). Replace/fix loose and disconnected bonding cables.

Vessel must have at least three 2.5 Gallon fire buckets with the words "Fire Bucket" stenciled in a contrasting color on each bucket. Vessel did not have the words "Fire Bucket" stenciled on 01 of the fire buckets. Label fire bucket.

On a vessel of at least 26 feet in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in each unmanned space. Vessel's aft high water visual indicator is extinguished. Replace/repair visual indicator for aft high water alarm.

Each life float must be fitted with a lifeline, pendants, two paddles, a painter, and a light. A vessel must carry one floating water light in approved in accordance with 160.010 in subchapter Q, or other standard specified by the Commandant, including, but not limited to, approval series 160.110. Vessel had 02 floating water lights with expired batteries. Replace batteries in 02 floating water lights.

Radiotelephone installation must be continuously associated with the ship even though a portable installation is used. Bridge radio had different MMSI from Radio station certificate. Acquire new station license with correct MMSI. 47 CFR 80.1003

All vessels must have navigation lights that are in compliance with the applicable sections of Internation and Inland Navigation Rules. Port running light and stern navigation lights were extinguished. Replace/ repair lights.

Any vessel of at least five net tons which engages in Coastwise trade must have a Certificate of Documentation. Vessel does not have a valid COD after ownership change. Provide proof of valid COD.

Each vessel must be equipped with at least two operable portable battery lights. One of these lights must be located at the operating station and the other at the access to the propulsion machinery space. Portable light at the operating station was inoperable. Repair/replace light.

Vessel must comply with FCC radio requirements. Radio station authroization is expired, O/O must provide evidence of current certification.

Rotating machinery must have a proper protection. Belt drive on main engine in the engine compartment is unprotected.

The design of all exhaust systems must ensure minimum risk of injury to personnel. Exhaust gas must not leak from the exhaust piping.

Windows must be capable of withstanding weather conditions. Starboard main deck window is missing.

Vessel must have up-to-date U.S. Coast Pilot, C.G. Light List, and Tides and Currents on board. Vessel provided updated publications prior to finishing the exam.

Life float must be kept apart by spacer. Vessel provided wood spacer for life float.

Provide first aid kit in compliance w. 46 CFR 160.041 or replace expired items as needed IAW 46 CFR 184.710.

Replace life ring buoy light lanyard with UV resistant/dark line of a length or 3 to 6 feet and life ring buoy life line of 60 feet IAW 46 CFR 180.70

Replace retro tape on life floats with SOLAS tape IAW 46 CFR 185.604

Provide life float tags to verify compliance and replace life float waterlight lanyard with 3/8" UV resistant line of at least 18 feet IAW 46 CFR 180.150

Failed to undergo Certificate of Inspection examination prior to expiration of previous COI.

Engine room bilge pump light not working and needs labeled.

Anchor light does not work.

Fuel shut offs need proper valve handel installed.

Vessel was due for a dry-dock/hull exam by March 26th, 2016; to date (as of April 13th, 2016) this required inspection had not been scheduled or completed. Vessel is prohibited from operating with passengers on-board until a satisfactory dry-dock/hull exam is completed by an attending U.S. Coast Guard Inspector. IAW 46 CFR 176.600
