A 6 inch crack was found at the top left corner of the forward door to the ECR and 4 inch crack was found on the top left corner aft door to the ECR, compromising the integrity of the ECR fire boundary, 74 SOLAS (14 cons) II-2/9.3., 40AC.

2520: The company should ensure that any non-conformity is reported, appropriate action is taken and records of these activities is maintained. The SMS should ensure hazardous situations re reports to the company. The crew is using unsafe procedures to test the smoke detector over the main engine. The heat detector in the E/R workshop has an incorrect smoke detector label near it. See Narrative.
