During a review of the third-party service records, the facility's annual service report (dated May 2020) for lifesaving appliances noted deficiencies listed in annual narrative summary. CG/BSEE Inspectors observed defs had not been corrected and lifeboats remained in service. As a result, the CG cannot verify that the lifeboats have been maintained in good condition and are readily available for emergency evacuation as noted in the EEP. No additional personnel are permitted on board without approval of the Coast Guard. In order to rectify this deficiency the Operator must provide documentation that all deficiencies noted in the May 2020 servicing report for lifesaving appliances have been corrected.

In the course of the inspection the CG observed that no lifeboats had been launched within the previous 12 months. Prior to returning the lifeboats to service the Operator must ensure that an emergency evacuation drill has been successfully completed for each lifeboat.

IAW 33CFR143.120 each Floating OCS Facility must meet the requirements of 46CFR108.301 and act IAW their approved Stability Letter. During an evaluation of compliance with stability protocols in the MOM and on board operations, CG and BSEE Inspectors observed anomalies and warning alerts displayed on the Weight Report and TTMS Diagnostic portion of the facility's IMMS. As noted in the approved MOM, the system assists the Operator with maintaining stability of the facility. An open Condition of Class dated 31Oct2019 (see MISLE narrative). Currently, only 3 out of 36 load cells are active, but those 3 are unreliable due to phasing in and out of service. Currently the facility's COI requires 02 BCOs, which can be reduced to 01 BCO if ballast control operations do not exceed 12 hrs in a 24 hr period. Due to the current state of the TTMS, 02 dedicated BCOs are required to maintain the stability of the facility. Prior to reducing the required BCO compliment, the Operator must provide documentation demonstrating compliance with the stability letter.

While performing a review of the third party service records, the facility's service report (dated July 2020) for firefighting appliances notes that the foam concentrate stored at fire station #32 and #34 failed its sample analysis. CG and BSEE inspector observed that this deficiency has not been rectified and the foam concentrate remains in service. Replace foam concentrate at stations #32 and #34 prior to resuming production operations.

Tendon Tension Monitoring System (TTMS) is currently operating with tendons T1, T5 and T6 being manually calculated due to failed readings from the tendon load cells. Additionally, tendon 5 is completely failed with bridge 2 failure and no acceptable values on T5,B1. Estimated health of the TTMS is approximately 40% and is not operating within acceptable limits to provide validation of weight maintenance and monitoring per the approved Marine Operations Manual (MOM) and unit stability letter.
Until TTMS is restored, unit is prohibited from skidding on a drilling unit.
