Each enclosed space must be vented or ventilated. NA KIKA reported to OCS OCMI that ventilation fans are damaged and needing to be replaced due to fan motors being grounded and J-Box motor not being supported properly. Unit conducted additional investigation of ventilation units installed and determined SW shaft ventilation and the SE oil pump room are both without functioning ventilation systems, as required. MI request that proper repairs be made to bring units back into normal operation. Until final repairs completed, unit will ensure all personnel must receive permission prior to entry, entering personnel shall have properly calibrated portable gas detector to monitor O2 and LEL, any increase in LEL will require immediate evacuation of personnel and re-establishment of ventilation. Unit shall keep MI updated on any changes in mitigating factors and notify MI or OCS OCMI when all repair are completed. 46CFR1018.181(a)

Each unit must have draft marks for each foot of immersion if the unit is column stabilized. No draft marks were present on any columns. Make suitable repairs.

The P.A. system must be installed with regard to acoustically marginal conditions. The PA system is not audible at LS stations (muster), and other locations. Make suitable repairs.

Each crane must be designed in accordance with API Spec 2C. Multiple deficiencies identified in 2018 Seatrax report remain outstanding. Make suitable repairs.

ESS-2 panel, titled "Receiving Module West," accounts for majority of all safety function located in the NW column and west side of the facility, such as, fire water (deluge, sprinkler, and monitors) header blowdown, ESD, and ventilation. The water inside the panel activited emergency shutdown procedures.

Each lifeboat must be a fire protected lifeboat approved under series 160.135 and have a self contained air system. Multiple lifeboats on the unit have air systems leaking at the regulator. Replace/renew to original approved condition.

Provide lifeboat familiarization procedures from Nakika's SEMS program that demostrates every person on the facility is required to muster under emergency conditions and is provided with adequate training on how to embark lifeboats to include proper entry.
