All escape hatches and other emergency exits used as means of escape must be marked on both sides in clearly legible letters at least 50 millimeters (2 inches) high: “EMERGENCY EXIT, KEEP CLEAR”, unless such markings are deemed unnecessary by the cognizant [name withheld] observed no emergency marking in accommodation space & on emergency exits for engine room. Vessel operator installed emergency marking during inspection. 46 CFR 185.606

Be installed with metal supports spaced not more than 610 millimeters (24 inches) apart, and in such a manner as to avoid chafing and other damage. The use of plastic tie wraps must be limited to bundling or retention of multiple cable installations, and not used as a means of support, except that on vessels of not more than 19.8 meters (65 feet) in length, installations in accordance with paragraph 14.h of ABYC E-8 and paragraph 15.h of ABYC E-9 (both incorporated by reference; see 46 CFR 175.600) are acceptable as meeting the requirements of this section. Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment and electrical shock. Outlet behind the concession stand was not properly secured. 46 CFR 183.200 (b)

Each ring life buoy must be marked with Type II retroreflective material approved in accordance with 46 CFR 164.018 or other standard specified by the Commandant. The arrangement of the retroreflective material applied after March 11, 1996, must be as specified by IMO Resolution A.658(16). As per IMO Resolution A.658(16), Retro-reflective materials of a sufficient width (approximately 5 cm) should be applied around or on both sides of the body of the lifebuoy at four evenly-spaced points. The ring life buoy only had three pieces of retro-reflective tape. 46 CFR 185.604(i) IMO RESOLUTION A.658(16)

Where flexible nonmetallic hose is permitted for use in piping systems by this section, it must meet SAE J-1942 (incorporated by reference; see 46 CFR 175.600) or be specifically approved by Commandant. Fuel hoses were found to be deteriorated & outer covering dry rotted on port main diesel engine.

The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection giving consideration to provisions for a seaworthy hull, protection against fire, means of escape in case of a sudden unexpected casualty, guards and rails in hazardous places, ventilation of enclosed spaces, and necessary facilities for passengers and crew. Vessel must repair damage to the forward bow hand rail, both sides of the spray rail, and ensure bolts for hull splash guard are water tight prior to carrying passengers. 46 CFR 177.100

All vessels must have navigation lights that are in compliance with the applicable sections of the International and Inland Navigation Rules... Vessel's mast head light lens obstructs light.

Vessels operated beyond the 20 nautical mile limitation specified in paragraph (a)(1) of this section, but not more than 100 nautical miles from the nearest land, must be equipped with a MF-DSC frequency transmitter meeting the requirements of § 80.1101(c)(3) and capable of transmitting J3E emission and a receiver capable of reception of J3E emission within the band 1710 to 2850 kHz. Vessel does not have an MF-DSC radio installed on board. Vessel will need to install one, or apply for a FCC wavier IAW SPV Checklist (revision 2019.05.16) Until this is rectified, the vessel will be restricted to operating under 20 NM from land.

Provision must be made for closing all supply duct cowls or scoops and exhaust duct discharge openings for a space protected by a fixed gas extinguishing system. The engine room natural ventilation covers do not close sufficiently.

The discharge ends of fuel tank vent pipes must be fitted with removable flame screens or flame arresters. The flame screens must consist of a single screen of a single screen of corrosion resistant wire of a least 30x30 mesh. The fuel tank vent does not have a screen.

As appropriate for the intended voyage, a vessel must carry adequate and up to date: U.S. Coast Pilot or similar publication and Coast Guard Light list. Coast Pilot and light list are not current (2019)

Each survival craft on a vessel must be in good working order and ready for immediate use before the vessel leaves port and at all times when the vessel is underway. All of the vesel's lifefloats had cinched covers over the tops of them while passengers were embarked.

Vessel had cut corroded bilge pipe in #2 hold and repaired with soft patch. Repair was made prior to OCMI approval. Vessel is required to submit repair plan prior to completion.

Aft port side gel coat and fiberglass damage needs repair.

Fire hose burst during hydrostatic testing. Replace with an approved fire hose to the satisfaction of the attending CG Marine Inspector prior to carrying passengers.

The pressure guage for the fire pump was inoperative. Replace to the satisfaction of the attending CG Marine Inspector.

Found dead end wires with no terminal connectors under the steering console. Install pressure type connectors on each dead end wire to the satisfaction of the attending CG Marine Inspector.

Found expired items in the first aid kit. Replace expired items to comply with 46 CFR 160.041 to the satisfaction of the attending CG Marine Inspector.

Found incomplete first aid kit. renew kit to conform to 46 CFR 160.041 to the satisfaction of a CG Marine Inspector.

Mechanical ventilation in engine room did not shut down upon actuation of pre-engineered fixed firefighting system. Demonstrate proper auto shutdown to the satisfaction of CG Marine Inspector.

Pre-engineered fixed firefighting system’s bottle overdue for hydrostatic test. Hydo bottle or perform the optional 5 yr visual exam by a DOT authorized servicing company with a Requalification Identification Number.

Provide Letter of Designation of Senior Deckhand(s) as required by vessel's current valid COI in accordance with NVIC 1-91.

Install visible alarms at the operating station for automatic bilge pump.

Replace holed fire hose w/ approved UL19 fire hose.

Provide pressure gage on output side of fire pump.

Replace painters on all three stacks of life floats.

Replace all grab lines, pendants on all life floats as necessary.

Install LF floating light lanyards on all seven life floats. Lanyards are too short and not of approved material.

Replace five broken sets of lifefloat paddles.

Install suitable spacer material between all LF's.

Provide accurate labeling of mismarked bilge high water alarms.

Install exhaust lagging on port engine (outboard exhaust pipe) fwd of cooling line.

Provide metallic supports for cabling in engine room, above both engines.

Mark all lifefloats w/ vessel's name and number of persons.

Vessel restricted to 152 persons until two additional adult lifejackets placed on board.

Provide proof that navigation charts are current to the satisfaction of the Marine Inspector

Install forepeak keeper device, and gasket material to satisfaction of marine inspector.

Install machinery guard over rotating fire pump belts.

Replace two cracked port and starboard windows in pilothouse.

Make salon storage compartment, and lazarette compartment bulkheads watertight. (6 bulkhead penetrations need to be sealed)

Provide proper stenciling of Number of persons on each life float.

Provide two 4' paddles properly stenciled and stowed for each life float.

Provide 3'-6' lanyard with clip for lifebuoy.

Repair fuel leak on return line generator.

Replace outdated hydro release.

Provide a cable on the stbd emergency shutoff valve. Its currently located under a refridgerator in the concession area. The energency shutoff also needs to be properly labeled.
