The design shall have regard to materials used in construction, the purpose for which the equipment is intended, the working conditions to which it will be subjected and the environmental conditions on board. PSCO received report from onboard pilot that main engine was not responding to bridge commands.

FUEL WAS SURROUNDING ALL FOUR AUXILIARY GENERATORS, ON THE MAIN ENGINE AROUND AND BELOW ALL CYLINDERS, INCLUDING SOAKED FUEL PIPING AND DOUBLE JACKETED HOSES. OIL WAS CAKED ON THE DECK BELOW ALL HFO PURIFIERS AND A BUCKET OF FUEL WAS HANGING BEHIND THE NO. 2 HFO PURIFIER PUMP.
