OIL WAS CAKED ON THE DECK BELOW ALL HFO PURIFIERS AND A BUCKET OF FUEL WAS HANGING BEHIND THE NO. 2 HFO PURIFIER PUMP. 74 SOLAS (2001) II-2/15.2

THE ENGINE ROOM PORT OF THE CHECKLIST SHOWS PIPING SYSTEM AND STEERING GEAR WERE CHECKED FOR LEAKS WEEKLY IN NOVEMBER 2019. THE PIPELINES AND LAGGING IN THE ENGINE ROOM LED THE PSCO TO BELIEVE INSPECTIONS ARE NOT BEING CONDUCTED IN ACCORDANCE WITH THE MAINTENANCE PLAN. 74 SOLAS (2014) IX.3.1 ISM CODE 10.2.1
