Electric installations on vessels must ensure maintenance of services necessary for safety under normal and emergency conditions. Properly troubleshoot root cause of loss of propulsion and provide proof of repairs and class report to CG.

Lifeboats must be recoverable. Port Lifeboat motor controller transformer burnt up. Temporary installed and new transformer to be ordered.

13199 - Periodic Safety test procedure documents approved by the Coast Guard must be retained aboard the vessel. Test procedure documents must be in a step-by-step or checkoff list format. Each test instruction must specify equipment status, apparatus necessary to perform the tests, safety precautions, safety control and alarm setpoints, the procedure to be followed, and the expected test result. The Company should ensure that obsolete documents are promptly removed. Vessel's PSTPs available on board were frequently missing the required components of a periodic safety test procedure as per 46 CFR 61.40-10(a) (i.e. missing detailed procedure to be followed and expected result). Checklists made for PSTP verification makes reference to tests not available in the current PSTP on board. 46 CFR 61.40-1(c), 46 CFR 61.40-10(a), and ISM Code 11.2.

The International Anti-Fouling System Certificate needs to be updated to reflect the repainting from the last vessel dry docking. Int Convention on the Control of Harmful Anti-Fouling Systems on Ships. Reg. 4(1)(a)

A signboard should be permanently fitted in the machinery space containing gas-fuelled engines stating that heavy lifting, implying danger of damage to the gas pipes, should not be done when the engine(s) is running on gas. No signboard within the engineroom. Resolution MSC.285(86)5.6.10

A water spray system should be fitted for cooling and fire prevention and to cover exposed parts of gas storage tank located above deck. Numerous water spray nozzles on the water spray system for the superstructure were clogged, rendering them ineffective for fire prevention. MSC.285(86) 3.3.2.1

Starboard lifeboat davit found inoperable during abandon ship drill. Lifeboat incapable of being launched. Prove proper operation of lifeboat launching arrangement and completion of abandon ship drill to the satisfaction of the attending marine inspector. SOLAS III/Reg 16.2

The machinery, boilers and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended and shall be so installed and protected as to reduce to a minimum any danger to persons on board, due regard being paid to moving parts, hot surfaces and other hazards. The design shall have regard to materials used in construction, the purpose for which the equipment is intended, the working conditions to which it will be subjected and the environmental conditions on board. The salt water distribution header is 90/10 copper nickel 30" in diameter and 6'4" long and it is located under the salt water pump controllers. During the internal examination a band of corrosion was discovered - approx. 12" wide and 4'11" long centered in the 6 o'clock position. The hole approx. 1/8" was located 7" in from the flange at the 6 o'clock position. Properly repair or renew salt water distribution header to its original construction. Temporary repair accepted by the attending CG Marine Inspector for 90 days.
