A pre-engineered fixed gas fire extinguishing system must automatically shut down all power ventilation systems and all engines that draw intake air from within the protected space. Vessel has installed power supply ventilation system that does not shutdown when pre-engineered system is activated.

At each inspection for certification and at every other vessel inspection the bilges and other spaces may be examined to see that there is no excessive accumulation of oil, trash, debris, or other matter that might create a fire hazard, clog bilge pumping systems, or block emergency escapes. Vessels bilges were found with accumulated oil below the reduction gear and shaft hull penetration.

A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. For equivalent kits, the contents must be stowed in a suitable, watertight container that is marked “First-Aid Kit”. A first-aid kit must be easily visible and readily available to the crew. Vessel first aid kit was missing required contents.

Vessels constructed of wood must provide bilge level alarms in all watertight compartments except small buoyancy chambers. A visual and audible alarm must be provided at the operating station to indicate a high water level in each space. Vessel is constructed of wood and does not have a high level indicator forward of the collision bulkhead.

The owner shall, prior to receiving a Certificate of Inspection, submit for approval to the cognizant OCMI, at least two copies of the following plans, manuals, analyses, and calculations that are applicable to the vessel as determined by the OCMI; On sailing vessels: Rigging plan showing sail areas and centers of effort as well as the arrangement, dimensions, and connections of the standing rigging. Vessel shall provide copy of approved sail plan to MI.

On passenger decks of vessels engaged in a ferry or excursion type operation, rails shall be at least 42 inches high. The space below the rail shall be fitted with Bulwarks; Chain link fencing or wire mesh or equivalent. Vessel may meet new T standards: rails with openings of not more than 4 inches in diameter; or bars,slats, rail courses, or an equivalent spaced at intervals of not more than 100 millimeters (4 inches). Vessels deck rail courses are currently at intervals estimated to be 10 inches. Provide plan prior to installation to Marine Inspector for the installation of rail courses that meet the standard listed above.

Vessel was not able to perform a proper shutdown of engines using the fire suppression system. Repair fire extinguishing system prior to departure and prior to carriage of passengers to the satisfaction of an attending USCG Marine Inspector. 46 CFR 181.420 (a) (3)

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. Vessel experienced a loss of propulsion while getting underway from the dock due to catastrophic failure of the vessels transmission/reduction gear. Demonstrate proper operation of vessels propulsion to the satisfaction of attending CG Marine Inspector.

A vessel that is exposed to salt water more than three months in any 12 month period since the last examination must undergo a drydock examination and an internal structural at least once every two years. Vessel is overdue for dry dock exam due 01 December 2023. Vessel to complete satisfactory dry dock exam prior to carrying passengers. 46 CFR 176.600 (c) (1).

A drydock examination conducted in compliance with § 176.600 must be conducted while the vessel is hauled out of the water or placed in a drydock or slipway. During the examination all accessible parts of the vessel's underwater body and all through hull fittings, including the hull plating and planking, appendages, propellers, shafts, bearings, rudders, sea chests, sea valves, and sea strainers shall be made available for examination. Sea chests, sea valves, and sea strainers must be opened for examination. On wooden vessels, fastenings may be required to be pulled for examination. Vessel is overdue for fasteners to be pulled. Vessel must complete satisfactory fastener pull prior to carrying passengers. 46 CFR 176.610 (a)

All oceangoing vessels 40' or more must have a garbage management plan. No plan provided. Provide a garbage plan.

At each initial and subsequent inspection for certification, reinspection and at every other vessel inspection the marine inspector shall require that all observed unsafe practices and hazardous situations be corrected. Racor filter is leaking red dye diesel. Make proper repair to the satisfaction of an attending marine inspector.

Conductors in power and lighting circuits must be No. 14 AWG or larger. Replace 22AWG conductor for light above stove to satisfaction to attending marine inspector.

Provide plans to unstep mast to OCMI within 30 days.

Aft bilge high-water alarm was found to have only visual arm and audible alarm was inoperable. Existing vessels after 1999 must have both visual and audible high-water alarms. Provide audible alarm for aft bilge highwater alarm, in addition to the visual alarm.

Found vessel to be missing EPIRB log. EPIRB's monthly test must be documented. Provide appropriate EPIRB log.

Provide approved type battery for floating electric waterlight.

Company did not have documentation for employee background checks in accordance with 49 CFR 40.25.

Provide proper high water bilge alarm in way of the engine room.

No smoke detectors in overnight berthing areas. Master instructed to install recreational type smoke detectors.

The gray water overboard discharge hose on the port side head was not properly installed. The looseness caused water leakage which drained to the bilge.

EPIRB battery expired April 2003 under old ownership. Master required to replace and inform LOSMS via fax of purchase of new battery.

Provide bilge high level alarm for the anchor locker.

INSTALL PROPER RELIEF VALVE FOR HOT WATER HEATER (TEMP 200 F, PRESSURE 150 PSI)

MAKE LATEST CORRECTIONS AND CHANGES CHARTS AND COAST PILOT.

RECHARGE ONE B-II FIRE EXTINGUISHER PRIOR TO CARRYING PASSENGERS.

PROVIDE PERMANENT TOP COURSE FOR RAILING ACROSS STERN OF THE VESSEL. TOP COURSE SHOULD BE PLACED SO LIFE FLOATS MAY FLOAT FREE.
