STBD HULL: Observed wood decay in multiple locations in way of the two aft-most chainplates on the stbd side. Excavate to good material and repair to the satisfaction of the attending Marine Inspector.

STBD HULL: Observed wood decay in the coverboard and shear strake in way of the aft chainplates stbd side. Excavate and repair to the satisfaction of the attending Marine Inspector.

The design, materials, and construction of masts, posts, yards, booms, bowsprits, and standing rigging on a sailing vessel must be suitable for the intended service. The hull structure must be adequately reinforced to ensure sufficient strength and resistance to plate buckling. The cognizant OCMI may require the owner to submit detailed calculations on the strength of the mast, post, yards, booms, bowsprits, and standing rigging to the Marine Safety Center for evaluation. The vessel reports that the masts have been unstepped and removed from the vessel for further rot investigation and repair. Vessel may continue to operate with passengers as a mechanically self-propelled vessel only. Vessel is restricted from carrying passengers while any of the masts are on board and may not operate under sail by any means. All COI fire-fighting, lifesaving and manning requirements are still in effect. In order to become a passenger sail vessel again the -------MISLE character limit-see uploaded 835V---------------

The number and identification of the items stowed inside, and their sizes, must be marked in clearly legible letters and numbers on each container for lifejackets. MI observed markings were not at least 2 inches high, not on all containers, and did not have the correct number of items stowed. 46CFR185.604(f)

Gears, couplings, flywheels and all machinery capable of injuring personnel shall be provided with adequate covers or guards. MI observed two diesel engine belts exposed without guards. 46CFR58.01-20

Vessels that have overnight accommodations for passengers must submit plans to the cognizant OCMI by 28Mar2022 for the installation and use of a monitoring device to ensure the wakefulness of the watchmen. Submit detailed plans for your proposal to include general arrangement/locations and bill of materials for the installation to include cable sizes/length for plan approval and install the system prior to 30Sep2022. plan approved 30DEC22, pends install.

WEEKLY, MONTHLY INSPECTION OF SURVIVAL CRAFT SHALL BE CARRIED OUT. NO INSPECTIONS DOCUMENTED OR LOGGED.

THE MASTER SHALL ENSURE EPIRB IS TESTED MONTHLY, TEST SHALL BE LOGGED. NO EPIRB TEST LOGGED.

ON A VOYAGE OF OVER 24 HOURS PASSENGERS SHALL BE REQUESTED TO DON LIFE JACKETS AND GO TO ASSIGNED EMBARKATION STATIONS. OVERNIGHT PASSENGER ORIENTATION DOES NOT SPECIFY DONNING OR DEMONSTRATION OF LIFEJACKETS AT EMBARKATION STATION.

EACH DISTRIBUTION PANEL SHALL BE TOTALLY ENCLOSED AND BE OF THE DEAD FRONT TYPE. ENGINE ROOM DC DISTRIBUTION PANEL HAS OPEN BREAKER HOLES, AND A MODIFICATION FOR AN INVERTER THAT COMPROMISE ITS DEAD FRONT, POSE ELECTRICAL SHOCK HAZ.

All life ring buoys are wasted, replace (3) ring buoys.

(2) Life floats are not properly attached with line or week link. Install weak link & line properly

Not all drills for the season have were accounted for. Vessel owner must instill monthly MOB & FF drills during the operating season.

No monthly test of the EPIRB were accounted for. Vessel owner must ensure EPRIB is test and logged monthly.

Lableing for life jacket stowage needs to be updated to reflect actual amount of life jackets in stowage area

Vessel is short (06) adualt life jackets, provide 6 new life jackets to meet COI requirments

(3) Smoke detectors in each overnight accomandations shall be re-knewed prior to any overnight trips.

Thru-hull fitting for the engineroom is weeping. Tighten packing (stbd side)

Secondary bilge hose driven by stbd engine is leaking

Saloon biloge strainer is wasted, replace in-kind

Life float needs to have spacers between each life float

Provide a repair proposal to MSD Cape Cod to repair the leaking bottom plank and contact MSD Cape Cod prior to placing the rescue boat back in service.

Provide a satisfactory rescue boat prior to carrying passengers (photos are acceptable).

Vessel has not been conducting MOB and fire drills. Vessel is to conduct a drill, log, and provide to CG.

02 lifefoast on vessel were found needing repairs to seems. vessel is to repair both port bottom lifefloat and bottom lifefloat found midships.

The back stay on the starboard skiff thimble is worn. Repair or replace.

The back stay bolt on the starboard side IWO of the pulley is wasted. Repair or replace.

The starboard quarter lifebuoy waterlight battery was found expired. Provide an unexpired battery.

Vessel does not have a lifebuoy with a lifeline attached. Provide a lifeline to 01 lifebuoys.

01. EPIRB registration is expired. Provide EPIRB registration.

02. Retro reflective material does not work as designed on the vessels life floats. Provide new retroreflective material.

03. Starboard side aft mast shroud #3 & #4 nuts look severly wasted. Replace nuts in kind.

04. Port side anchor chain looks severely wasted. Remove anchor chain and have availbe to CG for inspection.

05. Vessel does not have preventative maintenance plan for rigging inspection regime as per NVIC 02-16. Provide PMP to USCG.

06. Vessel does not have sail plan on hand. Provide a current sail plan to USCG.

07. Vessel paddles for the life floats need to have a quick removable line to allow the paddles to be released. Provide a different line.
