The deficiency issued on 01MAR2026 under MISLE activity #8318126 for expired pyrotechnics located in all 18 lifeboats and bridge provide evidence of a failure of the implementation of the vessels' SMS IAW Chapter 1, Marine Procurement, section 1.02 Emergency Purchase Procedure that describes as an emergency requisition any "good and services needed to avert a potential immediate and direct negative impact to: safe, reliable and compliant ship operation". The purchase order for pyrotechnics was placed 13NOV2025, but not processed until 07JAN2026; and not identified as an emergency requisition. An internal ISM audit and record of corrective actions, including actions to prevent recurrence IAW ISM 9, is required.
