Fire-extinguishing systems shall be kept in good order and be available for immediate use. PSCO observed that the CO2 cylinders cutter pin in CO2 room was still installed after dry-dock. In accordance with vessel's "Intruction for Maintenance and Operation of Fire Fighting and Containment Equipment" manual (document no. 50-03-018-2-E, page 3)" THE SECURITY PIN AND THE STEEL WIRE MUST ALWAYS BE REMOVED WHEN A SYSTEM IS PUT INOT OPERATION" 74 SOLAS (1997) II-2/ Reg. 21

Ventilation systems should be fixed and, if of the negative pressure type, permit extraction from either the upper or the lower parts of the spaces. PSCO observed ventilation duct in compressor room to be rusted with holes in ducting approximately 3 to 5 feet from bottom of intake.
