REPAIR BILGE PUMPS: A VESSEL MUST BE PROVIDED WITH BILGE PUMPS IN ACCORDANCE WITH TABLE 182.520(a)(e). AT TIME OF INSPECTION MACHINERY SPACE BILGE PUMPS DID NOT OPERATE. 46 CFR 182520(a)

PROVIDE PROPER CABLE TIES: Cable and Wiring must be installed with metal supports and spaced not more than 24” apart in such a manner to avoid chaffing and other damage. Plastic tie wraps are limited to bundling and not support. 46CFR 183.340 (b) (4)

Secure batteries and battery boxes: code Each battery must be located as high above bilge as practicable, secured to protect against shifting with roll and pitch of vessel and free from water splash or spray 46CFR 183.352 (b)

Provide lanyard for life ring light and attach to life ring: Each ring life buoy with floating water light must have a lanyard 3 feet in length but not more than 6 feet securing light around the body of the ring life buoy (3) Each floating water light on a vessel carrying only one ring buoy, must be attached to a lanyard with a corrosion resistant clip. A break strength of at least 50 pounds that can be quickly disconnected.46CFR10.70 (6)(d)(2)

Provide repair schedule for repairing as origional construction: -main deck plank replacement and beam repair aft of main engine including port carlins, from embarcation to stern. -Repair/Replacement of deck house sheathing of rotted areas around operating station to include kitchen area aft of operating station. -Repair /Replacement of operating station rotten windows and window frames -Repair/Replacement of of rotten operating station roof to prevent water intrusion. The listed required repairs are to be completed no later than 31 March 2024. A list of materiels and repair schedule is to be provided to cognizant OCMI pror to commencing work. (completed Repairs must be to satisfaction of cognizant OCMI and undergo a satisfactory sea trial prior to operation with passengers.)46 CFR 177.300

Repairs and alterations to hull, machinery, or equipment that affects the safety of the vesselmust not be made without the approval of cognizant OCMI. -Replace / Repair as origional dunnage boards IWO Fore Peak, Main Hold and Machinery Space. -Arrest fungus and moldIWO fore peak, and main hold under sides of recently installed / replaced deck beams and planks. -Repair as origional fore peak deck beam IWO rot on starboard side. Provid Plan to install access hatch for main hold. The deck was recently replaced and no approved hatch installed. Repairs and approved hatch install to be completed prior to 31 March 2024.

Service Life floats: AT TIME OF INSPECTION LIFEFLOATS WERE NOT SERVICED AND NOT IN ACCEPTABLE CONDITION FOR OPERATION IAW COI. A life float approval series 160.027 or other standard specified by commandant. In good serviceable condition. In Accordance with 46CFR180.15 (d) & 180.207 (a) (1) Lakes, Bays ad Sounds: wood vessels in cold water are required LF with 100% aggerate capacity, 46CFR180.207 2 (c) A vessel on L,B & S route in Warm Water is not required to carry survival craft. 46CFR180.207 (2) (d)A vessel certificated to operate on L,B & S route within one mile of land is not required to carry survival craft. Service life floats and lights for life floats. 46CFR180.200 (a) (2)

46 CFR 182.720(e) Where flexible nonmetallic hose is permitted for use in piping systems by theis section, it must meet SAE J-1942 or be specifically approved by the Commandant. Vessel supply, return and vent hoses were not required J-1942 hoses.

46 CFR 182.20-40(b)(3) The shutoff valve at the tank shall be operable from outside the tank compartment, preferably from an accessible position on the weather deck. Vessel stbd fuel tank remote shutoff valve was inoperable. Vsl only allowed to operate off of port fuel tank until repaired.

46 CFR 182.20-30(d) When the flexible section is a nonconductor of electricity, the metallic sections of the filling pipe separated thereby shall be joined by a conductor for protection against static spark when filling. Install bonding wire on stbd fill pipe.

Provide Repair Proposal detailing continuation of deck plank repair and beam repair including carlins, port & starboard from embarcation to stern,Deck House Sheathing repair aft of operating station, Weather seal and sash repair to kitchen area windows, roof repair and recovering to prevent water intrusion. To include any materials used and construction application.

Prep exposed fasteners heads in hull with proper "bungs" including fastener heads improperly sealed. 46 CFR 177.300 NVIC 7-95

Re-seal rabbit seam at the keel the length of the vessel. 46 CFR 177.300 NVIC 7-95

Reef/Re-caulk and re-seal side planks port and starboard from plank 29 to stern Chine to Sheer. 46 CFR 177.300 NVIC 7-95

Remove Copper covering from planks 36&37 and properly seal planks to prevent water intrusion. 46 CFR 177.300 NVIC 7-95

Seal machinery space to provide proper enclosure to prevent FFF from escaping when discharged. Fire damper closures do not operate or close effectively. The openings and surrounding area of superstructure have rot and wastage preventing closures to work properly 46 CFR 177.300, 181.400, NVIC 7-95

Provide proper junction box for shore tie connection. 46 CFR 183.390

Provide repair plan for approval to install proper hatch in main deck fwd of mast. Unauthorized cut out to install hatch was not accepted by attended inspector. 46 CFR 177.300 NVIC 7-95

Provide Plan for approval to repair main hold hatch to include rotted adajacent deck beams and fore and aft carlins. Unauthorized repair and hatch covering install was not accepted by attending inspector 46 CFR 177.300 NVIC 7-95

Provide plan for approval to replace fungus infested and rotten deck boards and deck beams IWO fore peak, main hold, machinery spcae, pilothouse, kitchen area and aft of kitchen above transom. 46 CFR 177.300 NVIC 7-95

Provide plan for approval to repair / replace as origional dunnage boards IWO fore peak, main hold, machinery space. 46 CFR 177.300 NVIC 7-95 This requirement to be completed by end of calendar year 2022.

(a) The owner or managing operator shall make a vessel available for dry dock, underwater examination or internal structural examination. In accordance scheduled intervals and NVIC 7-95 guidance wood vessels are to undergo a routine inspection of fasteners. Vessel scheduled fastener pull was due by 31 March 2022. An initial scheduled date of 01 April 2022 was canceled due to vessel not out of water and made ready for fastener examination. The vessel is restricted from carrying passengers until the fastener examination can be completed. Officer in Charge Marine Inspection (OCMI) can provide a extension to the examination interval. A written request to [name withheld] requesting an extension is required. 46 CFR 176.610(a), 46 CFR 176.675, NVIC 7-95

Repair/replace as origional rotten hand rails IWO port & starboard from stem to embarcation ports. 46 CFR 177.300 / 46 C FR 177.900 NVIC 7-95

Each item of lifesaving equipment determined by the marine inspector to not be serviceable condition must be repaired or replaced. Aft port and starboard life buoys found in unserviceable condition, including dry-rotted grab lines. 46 CFR 176.808(b)

All vessels shall be provided with a satisfactory arrangement for draining any watertight compartment under all practical conditions. Discharge piping for starboard bilge pumped cracked and returning water back to bilge after pumping. 46 CFR 182.25-1 (Old T)

A visual indicator must be provided at the operating station to indicate when any automatic bilge pump is operating. Operating station indicator light was inoperable for the starboard automatic bilge. 46 CFR 182.530(c)

The marine employer must have a sufficient number of alcohol testing devices readily accessible on board the vessel to determine the presence of alcohol in the system of each individual who was directly involved in the SMI. All alcohol testing strips on board the vessel were expired. 46 CFR 185.212 46 CFR 4.06-15(a)

A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. For equivalent kits, the contents must be stowed in a suitable, watertight container that is marked “First-Aid Kit”. First-aid kit contents expired.

Each battery used in an item of lifesaving equipment, except inflatable survival craft equipment, must be replaced during the annual inspection if the expiration date of the battery has passed. Battery for floating waterlight attached to starboard ring buoy found expired. 46 CFR 185.726(d)

Each ring life buoy must be marked with Type II retroreflective material approved in accordance with 46 CFR 164.01 and be applied as specified by IMO Resolution A.658(16), which requires application around or on both sides of the body of the lifebuoy at four evenly-spaced points. Retroreflective material is missing from two points on the forward life buoy. 46 CFR 185.604(i)

Each ring life buoy with a floating waterlight must have a lanyard of at least 3 feet in length, but not more than 6 feet, securing the waterlight around the body of the ring buoy. Floating waterlight lanyard was approximately 1 foot in length after being secured around the body of the ring buoy. 46 CFR 180.70(d)(2)

All cable and wire must be installed with metal supports spaced not more than 24" apart and in such a manner as to avoid chafing and other damage. Stern light wiring in lazarette improperly supported by failed plastic 3M wire ties instead of metal supports every 24". 46 CFR 183.340(b)(4)

A vessel on lakes, bays, and sounds or rivers must carry 3 hand red flare distress signals approved to 46 CFR 160.021 and 3 hand orange smoke distress signals approved to 46 CFR 160.037, with substitutions as authorized in 46 CFR 180.68(c). Distress signals found expired or of a type approved for recreational vessel under 33 CFR 175.110. 46 CFR 180.68(b)

ROTTEN DECK BOARDS IWO AFT & FWD OF MAST. 2. ROTTEN DECK PLANKING BETWEEN DECK HOUSE AND FWD HOLD. 3. ROTTEN AREAS IWO PORT & STARBOARD SIDE OF DECK HOUSE IN VICINITY OF VENTILATION SHUDDERS. 4. DECK HOUSE MOUNTING TRIM IWO DECK HOUSE PLATE ATTACHMENT INCLUDING DECK BOARD BELOW SAME. 5. ROT ON DECK HOUSE SUPPORT BEAMS BOTH PORT AND STARBOARD ENDS. 6. FUNGUS & MOLD: DECK BEAMS(S) IWO DECK BEAMS AND DECK BOARDS PORT SIDE FWD. HOLD, STARBOARD SIDE MAIN HOLD IWO SHEER CLAMP AND SHELF. ENGINE SPACE OVERHEAD IWO DECK BEAMS AND DECK PLANKING. 7.DECK PLANK ROT IWO ENGINE SPACE OVERHEAD PORT & STBD S 8. FIRRING & ANGEL HAIR AROUND BASE OF MAST STEP AND MAST PLATE BOLTS.

A satisfactory U/W check ride is req for credit Dry-dock

Any vessell of at least five net tons which engages in the fisheries on the navigable waters of the United States must have a Certificate of Documentation bearing a valid endorsement appropriate for the activity in which engaged. Vessel was unable to provide the Certificate of Documentation for review. 46 CFR 67.7

Electrical Equipment on a vessel must be installed and maintained to protect passengers, crew, other persons and the vessel from electrical hazards, including fire caused by or originating in electrical equipment and electrical shock. Remove/repair exposed wires from the electric bilge pump in the Engine Room 46 CFR 183.200

On a vessel of at least 26 feet in length, a visual and audible alarm must be provided at the operating station to indicate high water level in the vessel. The High Water alarm is inoperable. Repair or replace high water alarm on vessel. 46 CFR 182.530(a)

Repairs or alterations to include repairs or refastening of decks must not be made without approval of the cognizant OCMI. Provide a repair proposal for the restoration of the areas identified by the attending Marine Inspector IWO the STDB side Operating Station paneling and deck boards and the area IWO the Port Side railing and post. 46 CFR 176.700(a)

Captain's license expired, Renew and provide to attending marine inspector.

Provide proof of enrollment in drug consortiu for entire crew.

Provide 02 alcohol test kits on board.
