No person may operate a vessel unless that person complies with all limitations in the endorsement on the vessel's Certificate of Compliance. Upon finding multiple electrical deficiencies in the cargo area, the vessel failed her COC examination and was expressly instructed not to conduct cargo operations. The deficiencies were issued on a Port State Control Report of Inspection and signed/stamped by the master. After the Coast Guard team departed, the vessel resumed their cargo operations which were prohibited by the attending examination team and acknowledged by the master. 46 CFR 154.1808

Electrical equipment, cable and wiring should not be installed in hazardous locations unless it conforms with the standards not inferior to those acceptable to the Organization. PSCO observed cabling for the cargo compressor junction box on both port and starboard cargo compressors to be loose and packed with silicone within the cable glands. Multiple bonding straps for the port cargo compressor piping were found wasted. Deficiency must be rectified prior to cargo operations and prior to departure. 74 SOLAS (14 Cons.) Each cargo pump discharge line and each liquid and vapour cargo manifold should be provided with at least one pressure gauge. Pressure gauge for the cargo-stbd condenser on the #1 tank top was found broken. Crew was able to replace. 74 SOLAS (14. Cons) Ch. VII/Reg. 13.1 IGC 93/Reg. 13.4.2

The machinery, boilers, and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended and shall be so installed and protected as to reduce to a minimum any danger to persons on board, due regard being paid to moving parts, hot surfaces, and other hazards. The design shall have regard to materials used in construction, the purpose for which the equipment is intended, the working conditions to which it will be subjected and the environmental conditions on board. Electrical installations shall be such that the safety of passengers, crew, and ship from electrical hazards will be ensured. PSCO observed emergency light fixture above the starboard cargo compressor space to be missing structural guard, leaving glass housing and fixture exposed to potential damage. 74 SOLAS (01 Cons.) Ch. The condition of the ship and its equipment should be maintained to conform with the provisions of the Code to ensure that the ship will remain fit to proceed to sea without danger to the ship or person onboard. After any survey of the ship under regulations 7, 8, 9 or 10 has been completed, no change shall be made in the structural arrangements, machinery, equipment and other items covered by the survey, without the sanction of the Administration. PSCO observed that a section of the sewage system piping in the engine room has been replaced with flexible hose as a temporary repair which has not been approved by the Administration. 74 SOLAS (01 Cons.)
