Stairways and lift trunks that penetrate more than a single deck shall be surrounded by at least "A-0" class divisions and be connected by self closing doors at all levels. PSCO observed that the door going from the stairway to the ship's provisions was not self closing. 74SOLAS(04)II-2/9.2.3.4.1

Resolution: Crew installed self closing mechanism with spare parts on board. Door now fully self closes.

Means shall be provided whereby normal operation of propulsion machinery can be sustained or restored even though one of the essential auxiliaries becomes inopperative. Special consideration shall be given to the malfunctioning of: the sources of water pressure. PSCO observed the number one salt water cooling pump leaking from the pump casing. 74SOLAS(04)II-1/26.3.6
