Except as provided by 15.725 of this part, no vessel may be navigated unless it has in its service and on board the crew complement required by the COI. Vessel did not have a master with MMC.

The master shall conduct sufficient fire drills to make sure that each crew member is familiar with his or her duties in case of a fire. Marine inspector was unable to witness fire drill due to lack of crew members. Vessel must conduct satisfactory fire drill prior to carry passenger.

The master shall conduct sufficient drills and give sufficient instructions to make sure that all crew members are familiar with their duties during emergencies that necessitate abandoning ship or the recovery of persons who have fallen overboard. Marine inspector was unable to witness abandon ship or man overboard drills due to lack of crew member. Vessel must conduct satisfactory emergency drills prior to carry passengers.

An adult life jacket must be provided for each person carried on board a vessel. Vessel had 03 unserviceable adult life jackets and 01 unserviceable child life jacket. Vessel is limited to carrying 124 passengers and 12 child until life jackets are replaced.

A vessel that is exposed to salt water more than three months in any 12 month period since the last examination must undergo a drydock examination and an internal structural at least once every two years. Vessel exceeded two year interval on 01FEB2020. Vessel required to undergo satisfactory drydock examination prior to departure from port or carrying passengers.

Vessels shall be inspected for compliance with the standards required by subchapter T for items not covered by standards in this chapter and shall be IAW standards acceptable to the [name withheld] port shaft packing gland, located under brides stateroom and also under the grooms stateroom is leaking. Wood surrounding shaft log at the bulkhead and throughout bride and groom bilge that surrounds the port shaft show wastage that needs repair/treatment during the next drydock. Provide repair plan and make proper repairs during next drydock (Jan 2021). Vessel required to maintain log of space for leakage verification until proper repairs are made. Vessels route is restricted to LBS only. 46 CFR 176.25-1(a)(OLD T)

At each inspection managing operator must be prepared to have fire protection equipment ready for inspection and qualified servicing facility must conduct the following tests: fixed fire extinguishing system and provide satisfactory evidence of required servicing to the marine inspector. Operator was unable to provide evidence of required servicing for the fixed fire extinguishing system. 46 CFR 176.810 / 46 CFR 176.25-25 (OLD T)

Each vessel that operatess on the high seas, or beyond 3 miles from shore, must have an FCC type catergory 1, 406mhz EPIRB installed to float free/activate. Vessels EPIRB found to not be registered or active since 20018. 46 CFR 180.64

Each EPIRB must be tested monthly and shall be logged/documented. Vessel found to not have an EPIRB log or documentation of testing. 46 CFR 185.728

All enclosed spaces within the vessel shall be properly ventilated. Wheere such openings would endanger the vessel under adverse weather conditions, means shall be provided to close them. All deadlight covers, used to close portlight holes on both port and starboard sides of the vessel were found to be inoperable or seized in the open position. 46 CFR 177.20-1(OLD T)

Unless otherwise stated lifejackets must be stored in convenient places and container must not be capable of being locked and must allow lifejackets to float free. Found 10 lifejackets to be stowed under the main deck in a locked closet but were counted in the required 129 lifejackets on board. 46 CFR 180.78

Every vessel subject to part III of Title III of the communications act must have a detailed inspection of the radio installation by the FCC licensed technician every 5 years. Vessel FCC Safety certificate found to have expired in June 2020. 47 CFR 80.903

Operation of transmitters on an ship station must be performed by a person holding a commercial radio operator license or permit. Vessel master/operator does not have a valid operator permit. 47 CFR 80.155

Vessels shall be inspected for compliance with the standards required by subchapter T for items not covered by standards in this chapter and shall be in accordance with standards acceptable to the OCMI as good marine practice. Vessel's port shaft packing gland, located under port side bride's stateroom, was found to be leaking excessively. Vessel shall keep log, prior to and after each trip to verify leakage in space, until proper repairs are made. 46 CFR 176.25-1 (OLD T)

Adult life jackets must be provided for every passenger carried. Vessel missing 25 life jackets. Restricted to 104 passengers until required life jackets are provided.

A port light must have an inside hinged dead cover. Covers on galley port lights missing bolts and dogs. Replace to satisfaction of attending Coast Guard Inspector.

Life jackets must have retro in accordance with Coast Guard approval. Over 30% of life jackets have wasted retro. Replace in kind with solas approved retro.

Automatically actuated emergency lighting required in below deck passenger spaces. Vessel does not have operational emergency lighting. Provide proposal for new installation of emergency lighting in bride and groom spaces below deck. 46CFR 184.30-5(a)

Provide 8 adult sized type I lifejackets. 2 were unserviceable, and 6 were missing.

1. ENSURE THE FOLLOWING REQUIREMENT IS MET: "THE NAME OF THE VESSEL MUST BE MARKED ON SOME CLEARLY VISIBLE EXTERIOR PART OF THE PORT AND STARBOARD BOW."

Ensure forward starboard porthole is capable of being made watertight.
