Licensees of radio stations are required to have current station documents. The vessel's station license is registered incorrectly as a General Mobile Radio Service. Update with proper registration and provide new CALL SIGN.

Every vessel subject to Part III of Title III of the Communications Act must have a detailed inspection of the radio installation by an FCC-licensed technician in accordance with § 80.59 once every five years. The FCC-licensed technician must use the latest FCC Information Bulletin, How to Conduct an Inspection of a Small Passenger Vessel. If the ship passes the inspection, the technician will issue a Communications Act Safety Radiotelephony Certificate. The vessel's Safety Radiotelephony Certificate needs corrected with proper Call Sign for vessel.

Each survival craft must be:(1) Secured to the vessel by a painter with a float-free link permanently attached to the vessel. Vessel painter line was not connected to vessel via weak link.

08199 - A visual indicator must be provided at the operating station to indicate when any automatic bilge pump is operating. The bilge indicator for the forward compartment was inoperable. 46 CFR 182.530(c) 16c

18299 - The marine employer must have a sufficient number of alcohol testing devices readily accessible on board the vessel to determine the presence of alcohol in the system of each individual who was directly involved in the SMI. There were no alcohol test strips on board the vessel. 46 CFR 4.06-15(a)(1) 16c

03199 - Except for engine exhausts, each inlet or discharge pipe that penetrates the hull below a line drawn parallel to and at least 150 millimeters (6 inches) above the deepest load waterline must have means to prevent water from entering the vessel if the pipe fractures or otherwise fails. There are 08 through hulls less than 6 inches above the waterline that need valves installed. 46 CFR 179.350(c) 50c

46 CFR 16.210 - Pre-employment testing requirements.No marine employer shall engage or employ any individual to serve as a crewmember unless the individual passes a chemical test for dangerous drugs for that employer. Vessel is not enrolled in drug and alcohol program, did not get drug tested for preemployment and not actively enrolled in a consortium program.

46 CFR 182.455 - Shutoff valves, installed so as to close against the fuel flow, must be fitted in the fuel supply lines, one at the tank connection and one at the engine end of the fuel line to stop fuel flow when servicing accessories. The shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located, preferably from an accessible position on the weather deck. If the handle to the shutoff valve at the tank is located inside the machinery space, it must be located so that the operator does not have to reach more than 300 millimeters (12 inches). Vessel did not have shut off valve located at the fuel tanks. Vessel is also missing labeling IAW with 46 CFR 185.608.

46 CFR 180.68 - A vessel on a lakes, bays, and sounds, or rivers route must carry: (1) Three hand red flare distress signals approved in accordance with § 160.021 in subchapter Q of this chapter, or other standard specified by the Commandant; and (2) Three hand orange smoke distress signals approved in accordance with § 160.037 in subchapter Q of this chapter, or other standard specified by the Commandant. Vessel only had 4 red hand flares.

46 CFR 180.70 - A vessel must carry one floating waterlight unless it is limited to daytime operations. Each floating waterlight must be approved in accordance with 161.010 in subchapter Q of this chapter. Each ring life buoy with a floating waterlight must have a lanyard of at least 3 feet in length but not more than 6 feet, securing the waterlight around the body of the life ring buoy with a corrosion resistant clip. The vessels life ring buoy did not meet the requirements stated above. *Daylight hours of operation only*

46 CFR 180.130 - Each survival craft must be secured to the vessel by a painter with a float-free link permanently attached to the vessel except that a float-free link is not required if the vessel operates only on waters not as deep as the length of the painter. Vessels life float did not have a painter or a float free link that meets the requirements of 46 CFR 180.137.

46 CFR 180.175 - Each life float must be fitted with a lifeline, pendants, two paddles, a painter, and a light. In addition to the missing painter (previously addressed), the vessel did not have paddles or a water light.

46 CFR 185.604 - The number of persons capacity must be marked or painted in clearly legible letters and numbers on each life float. Vessel’s life float did not have capacity marked on it as required.

46 CFR 182.4655(h) - Provisions must be made for closing all supply duct cowls or scoops and exhaust duct discharge openings for a space protected be a fixed gas extinguishing system. all closure devices must be readily available and mounted in the vicinity of the vent. Vessel did not have means to secure engine exhaust vents.
