A vessel on a Great Lakes route must carry Six hand red flare distress signals approved in accordance with § 160.021 in subchapter Q of this chapter, and Six hand orange smoke distress signals approved in accordance with § 160.037 in subchapter Q of this chapter. A hand red flare distress signal approved in accordance with § 160.021 in subchapter Q of this chapter may be substituted for the hand orange smoke required. Vessel only had 3 had red flares meeting the 160.021 requirement. 46 CFR 180.68

Remote fuel shutoff stations must be marked in clearly legible letters at least 25 millimeters (1 inch) high indicating purpose of the valve and direction of operation. Pull rods under the helm were labeled as fuel shutoff, when they do not stop the flow of fuel. Label for fuel shutoff valves in the aft deck needs replaced. 46 CFR 185.608

(a) As appropriate for the intended voyage, a vessel must carry adequate and up-to-date: (1) Charts of large enough scale to make safe navigation possible; (2) U.S. Coast Pilot or similar publication; (3) Coast Guard Light List; (4) Tide tables; and (5) Current tables, or a river current publication issued by the U.S. Army Corps of Engineers or a river authority. (b) Extracts from the publications listed above for the areas to be transited may be provided instead of the complete publication. Vessel did not have physical copies of all pubs onboard. Captain had access to all references via phone, but nothing was physically onboard the vessel. Provide a plan to ensure access to required pubs is available at all times onboard the vessel when it is away from the dock. 46 CFR 184.420

(a) Marine employers must establish programs for the chemical testing for dangerous drugs on a random basis of crewmembers on inspected vessels who: (1) Occupy a position, or perform the duties and functions of a position, required by the vessel's Certificate of Inspection; (2) Perform the duties and functions of patrolmen or watchmen required by this chapter; or, (3) Are specifically assigned the duties of warning, mustering, assembling, assisting, or controlling the movement of passengers during emergencies. Vessel is required a deckhand per the COI, and not all of the persons filling the deckhand position are part of the vessels drug testing program. Persons filling the deckhand position must be enrolled in the drug testing program used by the company. 46 CFR 16.230
